A co-constructed practice model for supporting parents of children in conflict with the law

- Description: The unique historical stressors linked to South Africa’s apartheid legacy, continues to manifest in the form of economic exclusion, social exclusion, inequality and poverty, with parents being subjugated to service users and extenders rather than included as service advocates, particularly in the child justice system. Furthermore, policies and practices do not include, engage and support parents on an intra and interpersonal level. Parents of children in conflict with the law, experience their children’s charge or arrest as well as their subsequent journey through the child justice system as a crisis resulting in their need for emotional, informational, practical and professional support during the child justice process. The similarities and differences between the contexts of child protection and child justice in supporting parents illuminates the existing gaps in child justice legislation, policy and practice resulting in a lack of support for parents during the child justice process. In the Children’s Act 38 of 2005, it is recognised that parents have a legal responsibility towards their children and that in fulfilling this responsibility parents can access support services to assist them when they face challenges in fulfilling this responsibility. In contrast, despite 80 percent of children in conflict with the law being released into parental care and parents expressing the need for support in fulfilling their parental responsibility, in this regard the Child Justice Act 75 of 2008 fails to make provision for parents to access support services. The child justice system’s narrow focus on parents as service extenders contributes to parents’ being excluded from targeted support services resulting in parents’ support needs not being addressed. The lack of programmes and services aimed at supporting parents highlight the need for coordinated services that address the multiple stressors parents are exposed to. To this end, this study was aimed at coconstructing a practice model for supporting parents of children in conflict with the law. The theoretical lenses employed in the current study namely; the Ecological systems model and the Buffering effect model describes the various systems parents need support from and the type of support they need from their family, community and professionals. Guided by a qualitative approach, the present study integrated applied research, in particular intervention design and development with participatory action research as it allowed systematic collaboration during the research process to ensure rigour. This study involved participants from two research sites namely, the Nerina One- Stop Child Justice Centre in Port Elizabeth and the Reception, Assessment and Referral office at the Uitenhage magistrates’ court. Employing a non-probability purposive sampling method, this study facilitated the participation of parents of children in conflict with the law and child justice officials who met the inclusion criteria, in the co-design and development of a practice model for supporting parents of children in conflict with the law. Participants assumed an expert and collaborative role, which enabled the co-construction of knowledge, meaning and innovation of the practice model. Qualitative data collection methods namely, twelve focus groups and thirty two participant observations, were used to explore, co-construct, describe and design a practice model for supporting parents of children in conflict with the law during the child justice process. Thematic analyses was employed to condense the data, search for codes, categories, themes, relationships and patterns in the data. Due to the research approach and design, data analysis was ongoing and informed design and development of the practice model. Based on the thematic analysis and synthesis both descriptive and analytic themes emerged. To ensure trustworthiness, this study employed various strategies to strengthen commitment, rigour, transparency and coherence. In addition, the participative research process, the inclusion of multiple forms of qualitative inquiry and the significance of the study contributed to the validity and quality of the study. Ethical considerations applicable to the study included participants’ voluntary participation, their informed consent and ensuring participants’ privacy or maintaining confidentially. Various strategies were employed to prevent or minimise risk to participants. The findings showed that formal sources of support, in particular, offer opportunities for parents to access individual and family counselling, parenting advice, and peer support. The study also highlighted the importance of recognising parents as a subsystem in the child justice system offers potential opportunities for inclusion of parents as co-facilitators of parenting programmes or support groups, as peer supporters during the child justice process. Parents’ inclusion as a partner in the child justice system is highlighted as an opportunity for parents to be able to vi participate in child justice fora, oversight committees and accreditation committees to influence policy, services and budget allocations for services to support parents of children in conflict with the law. This study’s contribution to the existing body of knowledge is an integrated, multidisciplinary, multi-phase co-constructed practice model that would enable inclusion of, and support for, parents of children in conflict with the law prior to, during and after the child justice. The co-constructed practice model (a) involves a continuum of parent-centred support for and inclusion of parents prior to, during and after the child justice process; (b) advances an inclusive and collaborative child justice system that views parents as important stakeholders in determining the type of services they need and being involved in developing practice; and (c) promotes parents as equal partners in decision making and policy making to influence legislation, policy and practice in the child justice system.

An assessment of school food and nutrition environments for strengthening the integrated school health policy in the Eastern Cape, South Africa

- Description: The 2012 Integrated School Health Policy (ISHP) offers a framework for adequate school food environment. It aims to contribute to the improvement of the general health of school-going children and to address health barriers to learning in order to improve education outcomes. This study assessed the school food and nutrition environment and critically appraised the position of nutrition within ISHP in order to make recommendations for best practice and promotion of healthy-eating in quintiles 1 – 3 secondary school learners of the Eastern Cape, South Africa. Areas evaluated included the school food environment (National School Food Nutrition Programme, NSNP, tuck-shop, vendor, lunch box, and school vegetable garden); eating practice; nutrition knowledge; and nutrition status. A quantitative and qualitative cross-sectional design was applied in data collection. Data was collected from 1,357 learners at 18 secondary schools in three districts: Buffalo City Metropolitan Municipality, Chris Hani and OR Tambo, using semi-structured questionnaires. Five major food items provided in the school food and nutrition environment included: beans, pap, sour-milk, rice, and samp (maize) (NSNP); chips, sandwich, pap, biscuits, and cake (tuck-shop); chips, sandwich, pap, cake, and pop-corn (vendor); and samp (maize), noodles, fat-cake, chips, and Russian/Viennas (lunch boxes). Only three of the 16 schools had school vegetable gardens which produced vegetables and no fruits. Five most frequently eaten foods for breakfast were porridge, rusks, stiff-pap, bread-with-spread, eggs, and polony. Learners from quintile 3 (60.8percent) significantly ate more cooked porridge than learners from quintiles 1 and 2 (51.9percent) (p = 0.015). Leaners from quintile 3 (59.4percent) significantly ate more brown bread-with-spread than learners from quintiles 1 and 2 (50.4) (p = 0.022). The majority of learners (72.7percent) ate breakfast; more male learners (24.4percent) than female learners (29.1percent) skipped breakfast. The majority (62.2percent) of learners had poor nutritional knowledge; significantly, more male (35percent) than female (27percent) learners had poor nutritional knowledge (p = 0.003). Nutritional knowledge increased with age; significantly more grade 8 learners had poor (42.6percent) nutritional knowledge than grade 12 learners (14.1percent) (p < 0.001). Leaners from quintile 3 had good (23.5percent) nutritional knowledge than learners from quintiles 1 and 2 (15.7percent) (p < 0.001). School lessons, books and television were the most important source for nutritional information to learners. The most significant source was lessons (72.0percent; p < 0.001); followed by television (69.4percent; p < 0.001) and books (67.8percent; p = 0.014), in that ranking order. The majority of learners (64percent) had normal body weight. The prevalence of underweight, overweight and obesity was 13.0percent, 15.0percent and 8.0percent, respectively. The prevalence of overweight and obesity was higher in females than males (21.3percent vs 5.3percent; overweight; p < 0.001)) and (11.6percent vs 1.2percent; obesity; p < 0.001), respectively; while the prevalence in terms of underweight was significantly higher in male (21.3percent) learners than in female learners (8.1percent); p < 0.001. The study indicated that the school food environment was not supported by ISHP. The Integrated School Health Policy did not include on-site package for nutritional assessment, thus, there is need to offer nutritional interventions in schools.

An exploration of HIV and aids disclosure among HIV-serodiscordant married couples in the Eastern Cape Province, South Africa

- Description: Sub-Saharan Africa has the highest prevalence and incidence of HIV infection worldwide, mostly attributed to heterosexual transmission. Transmission in HIV-serodiscordant couples who have received joint voluntary counselling and testing ranges from 3% to 7% per year and only about 20% know that they are living in a discordant relationship. Disclosure is seen as a cornerstone for the prevention of HIV transmission between in HIV-serodiscordant married partners. It is argued that numbers of HIV-serodiscordant couples are most likely increasing; however, limited or no support is given to these couples as most because counselling interventions in South Africa do not provide for married heterosexual HIV-serodiscordant couples. The aim of the study was to identify the impact of disclosure of HIV and AIDS status on partners in HIV-serodiscordant married relationships. The second aim was to synthesize the findings to guide the development of contextualized counselling guidelines for married couples in HIV-serodiscordant relationships. Five HIV-serodiscordant married couples and 6 HIV and AIDS counsellors from a variety of settings in Nelson Mandela Bay, participated in this study. In this interpretive qualitative study, non-probability criteria, purposive sampling was used. Both participant groups are isiXhosa-speaking adults between the ages of 21 and 65 years and include both males and females. Qualitative data were gathered by means of audio-recorded semi structured interviews. Thematic analysis was used to analyse the data, while Guba’s guidelines were used to enhance the trustworthiness of the research. Semi-structured interviews with HIV-serodiscordant married couples revealed 5 common themes and were: (a) confused environment of couples in HIV disclosure, (b) inconsistent delivery of healthcare information to HIV-serodiscordant couples, (c) relationship predicaments post disclosure, (d) partner support on sexual intimacy for married couples and (e) reactions of family, children and friends towards the HIV disclosure. The HIV and AIDS counsellors 3 common themes and were: (a) basic HIV and AIDS counselling skills, (b) organisational resources support of HIV counsellors and (c) suggestions to support HIV counsellors.. This study supports the view that further research be done in other related areas (gay, lesbians, bi-sexual relationships) and demystify this unknown or misunderstood notion needs to take place at a larger scale and probably incorporate quantitative surveys to supplement the current qualitative research. Implications for practise and avenues for research are considered in this study.

- Description: The influence of culture in coping with Posttraumatic Stress Disorders (PTSD) has been highlighted in literature. However, these culture specific coping strategies are not as yet, extensively explored. The research sought to explore and describe the culturally-informed coping strategies of PTSD among the black Africans in the Greater Accra Region of Ghana and black Africans in the Eastern Cape of South Africa. A qualitative methodological approach was used to explore the phenomenon under study. Purposive non-probability sampling was employed to obtain access to participants who could inform the objectives of the study. Data was collected using semi-structured interviews with traumatised individuals and focus group discussions with cultural leaders. All interviews were audio-recorded, transcribed, translated and analysed using thematic content analysis to facilitate the description and comparison of the similarities and distinctive features emerging from the two African communities. The findings indicated that a number of cultural factors influence interpretation of symptoms of PTSD and the adopted coping strategies. Participants’ understanding of symptoms of PTSD relied heavily on Christian Religious and African cultural understandings. It was evident that almost all explanations were purely spiritual and minimal knowledge was expressed on cognitive interpretations. A great deal of emphasis by the Ghanaians and isiXhosa of South Africa, was on dreams and such interpretations were deferred to an authoritative individual (especially the traditional healer). An obvious hierarchy for coping emerged with the traditional healer playing a major role for the Ghanaian and isiXhosa participants. Performing rites and the use of plants were essential to healing for these group of individuals. Whereas, the Coloured-Afrikaans made reference to a higher power “God”. An outstanding observation was the extent to which Ghanaian and isiXhosa participants have incorporated Christian and traditional practices (i.e. praying to God, performing rites and using herbs or plants). Even where an individual did not wish to mix practice because of upbringing, they acknowledged the existence and effectiveness of these practices. Colonisation was drawn on as an explanation for such mixed practices. The advent of Christianity was explained as having established Western culture (specifically Christianity) that is incorporated by participants. The Coloured-Afrikaans on the other hand, were consistent with the Christian practices “prayers to God”. A great deal of consistency however existed among all participants. Social support was considered very important. Talking with family and friends were relevant to the healing process as it gets rid of distorted thought patterns and avoids isolation considered as a serious current threat to healing. While there was an element of cognitive understanding, Western based treatment using cognitive restructuring was not a treatment option. Clinical practitioners or Psychologists that come into contact with the black Ghanaians and black South Africans should consider collaborative treatment strategies using the draft checklist as a guide.

The parental rights of unmarried fathers have evolved in South Africa, especially since the Children’s Act 38 of 2005 (RSA, 2006) now recognises that unmarried fathers should have inherent right of contact, guardianship and care. These rights are, however, not automatic and need to be acquired through a process of mediation. While this legislation aims to facilitate the parental involvement of fathers by giving them rights as per Section 21 of the Children’s Act 38 of 2005 (RSA, 2006), the reality is that many unmarried fathers still remain challenged in this regard despite the change in their legal position as fathers. Unmarried fathers are often compelled to take the matter further through protracted court processes. Unmarried fathers face unique challenges with parental involvement after separation and they often need to negotiate their involvement with the mother after separation. Given that mediation between unmarried parents is in its infancy in South Africa, very little research has been done in this area. It was envisaged that this study would highlight the important aspects of mediation which need to be considered in order for mediation outcomes to be more successful and to enhance the co-parenting relationship. The study employed a qualitative research approach that was exploratory, descriptive and contextual in design. The research study consisted of three phases. Phase 1 employed a multiple case study design in order to develop an understanding of the experiences and perceptions of unmarried fathers and mothers, as well as mediators, regarding the parental responsibilities of unmarried fathers in addition to their experiences of mediation. The data collection process comprised in-depth semi-structured interviews with three sample groups, namely unmarried fathers, and mothers, and mediators. The data collected from the interviews were transcribed, coded and analysed by the researcher through the use of two techniques, namely: within-case analysis and cross-case synthesis. An independent coder was utilised to verify the coding and analysis process. The analysis of the data in Phase 1 revealed that the construction of the role of unmarried fathers is influenced by various factors such as cultural and religious influences; childhood experiences of fatherhood; societal constructions of the father’s role, and the perception of the legal rights of unmarried fathers. The new legislation pertaining to the responsibilities and rights of unmarried fathers, as contained in the Children’s Act 38 of 2005 (RSA, 2006), has resulted in increased parental conflict between parents after separation owing to their lack of awareness and confusion regarding the changes in legislation pertaining to the rights of unmarried fathers and the role of customary law in this regard. This has resulted in misconceptions about the legal rights of unmarried fathers, which have influenced unmarried parents’ attitudes and receptiveness to mediation. A significant barrier to father involvement emanating from the findings was relationship conflict experienced after separation, which was also identified as a significant challenge in mediation. The discussion of the themes reflected both positive and negative experiences of mediation for the study participants. While the parents derived educational and therapeutic benefits from mediation; there were also several factors such as relationship conflict, cultural barriers, influences of the extended family, and the approach and skills of the mediator, which influenced the mediation process. In Phase 2 of the study an integrative literature review and synthesis of the literature was conducted and critically appraised in terms of emergent themes and relevant research in the area of family mediation, which would inform the process of developing the descriptive guidelines for mediation with unmarried parents. The empirical findings from the study were synthesised with the findings from the integrative literature review, thus enabling conclusions to be drawn in order to generate descriptive guidelines for mediation. Phase 3 of the study describes the guidelines for mediation which emanated from the synthesis of the findings in Phase 1 and Phase 2 of the study. As part of a qualitative evaluation, the guidelines were presented to an expert panel for review, after which the guidelines were finalised. The proposed guidelines for mediation to enhance the parental involvement of unmarried fathers is based on the hybrid model of mediation, which incorporates elements from the Transformative approach, the African centred approach and the co-mediation model. The guidelines are further described in terms of a phased approach to mediation.

The anti-proliferative activity of drimia altissima and a novel isolated flavonoid glycoside against hela cervical cancer cells

- Description: Cancer is one of the leading causes of mortality worldwide. About 44% of all cancer morbidity and 53% of all cancer mortality occur in countries with a low to medium Human Development Index (HDI). Thus, cancer is rapidly emerging as a serious threat to public health in Africa and most especially, sub-Saharan Africa. The International Agency for Research on Cancer (IARC) projects that there will be 1.28 million new cancer cases and 970 000 cancer deaths in Africa by the year 2030 owing to the increase in economic development associated lifestyles. The dominant types of cancer in Africa are those related to infectious diseases such as Kaposi’s sarcoma and cervical, hepatic and urinary bladder carcinomas. The main challenge to cancer treatment in Africa is the unavailability of efficacious anticancer drugs. This is because most developing countries can only afford to procure the most basic anticancer drugs, which are also frequently unavailable due to intermittent supplies. This results in patients progressing to more advanced cancer states. One way of combating this African problem is to focus on research that aims at discovering efficacious and cost effective cancer therapies from available natural resources within the African continent. This study investigated the potential anti-proliferative activity (against HeLa cervical cancer cells) of four plants (Adansonia digitata, Ceiba pentandra, Maytenus senegalensis and Drimia altissima) commonly used in the African traditional treatment of malignancies. After in vitro bio-assay screening using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) assay, M. senegalensis root extract (MS-R) and D. altissima bulb extract (DA-B) showed anti-proliferative activity against HeLa cervical cancer cells with IC50 values of 25 μg/mL and 1.1 μg/mL respectively. By possessing the strongest anti-proliferative activity among the tested extracts, D. altissima was selected for further studies. Liquid-liquid partitioning of the Drimia altissima bulb extract with n-hexane, ethyl acetate, and n-butanol, yielded partitions 79a – d, with the n-butanol fraction, 79d, exhibiting the strongest cytotoxic activity (IC50 = 0.497 μg/mL). Through High Content Analysis (HCA) screening, fraction 79d was found to induce marked early mitotic cell cycle arrest. Fractionation of 79d using Diaion® HP-20 open column chromatography and a stepwise gradient of reducing polarity (water-methanol-ethanol-ethyl acetate) yielded cytotoxic fractions 82b, 82c, 82d and 82e, all with significant anti-proliferative activities at the tested concentrations of 0.1, 1.0 and 10 μg/mL. Bio-assay guided fractionation of 82c (the most effective fraction at the lowest tested concentration of 0.1 μg/mL) using Sephadex® LH-20 open column chromatography and 50% MeOH led to the isolation of compound 3.17. After structural elucidation using 1D and 2D Nuclear Magnetic Resonance spectroscopy (NMR), High resolution Mass spectrometry (HRMS), Fourier-Transform Infrared spectroscopy (FT-IR), ultraviolet spectroscopy (UV) and Circular Dichroism (CD), compound 3.17 was identified as a novel C-glucosylflavonoid-O-glucoside, 6-C-[-apio-α-D-furanosyl-(1→6)-β-glucopyranosyl]-4′, 5, 7-trihydroxyflavone (Altissimin, 3.17). Compound 3.17 exhibited a dose dependant anti-proliferative activity with an IC50 of 2.44 μM. The mechanism of action for compound 3.17 was investigated through cell cycle arrest, phosphatidylserine translocation (PS), caspase activation and mitochondrial membrane depolarization. The mechanism of cell death elicited by compound 3.17 in HeLa cells was found to involve the induction of M phase cell cycle arrest with consequent activation of apoptotic cell death which was evident from annexin V staining, mitochondrial membrane potential (ΔΨm) collapse and the activation of caspases -8 and -3. In silico computational techniques were employed to virtually determine potential biological targets of compound 3.17. Target fishing using the Similarity Ensemble Approach (SEA) target prediction gave human aldose reductase (hAR, AKR1B1) the highest ranking with a p value of 2.85 x 10-24, a max Tc of 0.35 and a Z-score of 41.8217. Using AutoDock4 and the AutoDock tools suite (ADT), molecular docking of compound 3.17 in the hAR binding pocket was successfully achieved with a lower ΔG free energy binding (-9.4 kcal/mol) than that of positive control ligand 393 (-8.7 kcal/mol). In conclusion, this study identified the genus Drimia and particularly D. altissima as a potential source for novel cytotoxic compounds. The discovery of altissimin (3.17), the first flavonoid glycoside to be isolate from D. altissima, enquires into the possible existence of similar compounds within the species. In addition to the observed in vitro cytotoxic activity against HeLa cells, the potential of altissimin (3.17) as a hAR enzyme inhibitor opens up the possibility of its use as an adjunct to increase cancer cell sensitivity to chemotherapy. Thus, altissimin (3.17) shows promise as a potential anticancer agent.

- Description: A comprehensive academic support at the public nursing college is important. Despite this, there is no such support in public nursing colleges in the Eastern Cape Province of South Africa. The aim of the study was to develop an academic support programme for students in order to improve their success in Higher Education. The study endeavoured to answer the following questions: What are the academic support needs of students at the public nursing college in the Eastern Cape of South Africa? What conceptual framework can be developed for the academic support programme of students? What academic support programme can be developed for students at the public nursing college? What guidelines can be developed for implementation of academic support programme of students? The objectives were: to describe academic support needs of students at the public nursing college in the eastern Cape Province of South Africa; to develop a conceptual framework for the academic support programme for students at the public nursing college in the Eastern Cape of South Africa; to develop an academic support programme for students at the public nursing college in the Eastern Cape in south Africa and lastly to describe guidelines for implementation and evaluation of the academic support programme. The research design used in this study was quantitative, qualitative, descriptive, evaluative, and theory generation. The study was conducted according to four phases. Phase one focussed on academic supports needs of the student. Phase two focussed on development of conceptual framework for the students at the public nursing college in Eastern Cape Province. Phase three focussed on the development of the academic support programme for the students, whereas phase four focussed on description of guidelines for implementation and evaluation of the academic support programme. The population of the study consisted of students undergoing training in a four-year programme at the Public Nursing College. The target population was the students in the same programme from level one to level four of training. The sampling technique used was a probability sampling with multi-stage and cluster sampling techniques being utilized. The total sample was n=333 participants. A quantitative self-developed questionnaire was used to collect empirical data. The questionnaire consisted of eight sections. Descriptive data was analysed using Statistic Package of Social Sciences (SPSS) version 24. Exploratory factor analysis was conducted on the descriptive data collected in phase one. Concept analysis and identification were done through the utilisation of exploratory factor analysis. The conceptual framework for the academic support programme was identified through the analysed factors. Fifteen factors were derived from the descriptive data. The conceptual framework of the academic support programme discussed in Chapter 5 was used also in the development of the academic support programme. It is in this chapter that assumptions, context, stakeholders, process, outcome, dynamics and, lastly, the recipient were discussed, along with a description of the programme. The programme was evaluated in accordance with the criteria for programme evaluation. The recommendations brought forward for the academic support programme were: The guidelines have to be tested for the implementation of the programme; benchmarking should be done to compare with other institutions of higher learning regarding their academic support programmes; an academic brochure regarding academic support must be available in the college and should be given to students on arrival as part of a package; and neutral researcher should conduct the same research in the same college.

Despite this, there is no such support in public nursing colleges in the Eastern Cape Province of South Africa. The aim of the study was to develop an academic support programme for students in order to improve their success in Higher Education. The study endeavoured to answer the following questions: What are the academic support needs of students at the public nursing college in the Eastern Cape of South Africa? What conceptual framework can be developed for the academic support programme of students? What academic support programme can be developed for students at the public nursing college? What guidelines can be developed for implementation of academic support programme of students? The objectives were: to describe academic support needs of students at the public nursing college in the eastern Cape Province of South Africa; to develop a conceptual framework for the academic support programme for students at the public nursing college in the Eastern Cape of South Africa; to develop an academic support programme for students at the public nursing college in the Eastern Cape in south Africa and lastly to describe guidelines for implementation and evaluation of the academic support programme. The research design used in this study was quantitative, qualitative, descriptive, evaluative, and theory generation. The study was conducted according to four phases. Phase one focussed on academic supports needs of the student. Phase two focussed on development of conceptual framework for the students at the public nursing college in Eastern Cape Province. Phase three focussed on the development of the academic support programme for the students, whereas phase four focussed on description of guidelines for implementation and evaluation of the academic support programme. The population of the study consisted of students undergoing training in a four-year programme at the Public Nursing College. The target population was the students in the same programme from level one to level four of training. The sampling technique used was a probability sampling with multi-stage and cluster sampling techniques being utilized. The total sample was n=333 participants. A quantitative self-developed questionnaire was used to collect empirical data. The questionnaire consisted of eight sections. Descriptive data was analysed using Statistic Package of Social Sciences (SPSS) version 24. Exploratory factor analysis was conducted on the descriptive data collected in phase one. There are strong voices arguing for churches and other Faith-Based Organisations to be regarded as strategic in contributing to addressing the challenge of poverty as agents of social development. Yet, as church congregations multiply throughout South Africa, they appear to be less responsive to the challenge of poverty facing South African communities, contrary to the rich history of churches in the past. The need for churches to be involved in sustainable community-driven development initiatives to fight poverty in their local communities cannot be denied nor be met with just rhetoric and scholarly dissertations by theologians and academics. The vicious cycle of poverty in our communities must be eradicated as it represents a social injustice in terms of South Africa‘s Constitution and speaks to deep structural inequalities inherent in our society. Whilst researchers have made several proposals for churches to move from social welfare and relief, there appears to be little research with a clear practical strategy which congregations can implement to enhance their social service delivery role from social relief to sustainable community development. This has left a considerable gap which the current research sought to address. This study thus responds to the call by previous research for churches to implement strategies other than mere relief. Hence, this study contributes to the religious development discourse in South Africa, by developing a strategy to enhance Church-based social service delivery initiatives from social relief efforts which are not sustainable, to community development efforts which are sustainable and which will be consistent with the current social development policy context and contextually relevant to the socio-economic challenges of poverty and its consequences in South Africa. The study was conducted in two phases. During phase one, the main purpose was to gain a deeper understanding of the historical and current involvement of the Church in providing social services which address poverty in communities, to enhance understanding of the factors which influence the provision of social services by churches, as well as to understand participants views of how church-based social service delivery can be improved in order to fight poverty. For phase two, the goal was to develop a strategy to enhance church-based social service delivery from being social relief to being community development oriented. The following objectives guided the research process: To review selected and applicable literature to gain a deeper understanding of the historical and current involvement of the Church in the delivery of social services in order to fight poverty; To explore and describe the perceptions of church leaders and congregation members of the historical and current involvement of the Church in the delivery of social services in order to fight poverty; To explore and describe the factors which influence Church-based social service delivery in fighting poverty; To explore the views of church leaders and congregation members on how church-based social service delivery can be improved in their churches to inform the development of a strategy to enhance church-based social service delivery in fighting poverty. This study employed a qualitative research approach grounded in the interpretivist social science paradigm. The qualitative research approach necessitated an exploratory, descriptive and contextual method of inquiry. A descriptive case study method was also used, with the Methodist Church of Southern Africa selected as the main case of focus. The target population in this research comprised of Christian churches with a historical involvement in social service delivery in pre-apartheid South Africa. Purposeful sampling of 5 churches was undertaken, namely; Roman Catholic Church (RCC), Anglican Church (AC), Dutch Reformed Church (DRC), KwaSizabantu Mission (KSB) and Methodist Church of Southern Africa (MCSA). Data collection was conducted in two phases. Phase one comprised a literature review; Phase two comprised gathering empirical evidence by conducting face to face semi-structured interviews with clergy and from conducting one focus group interview with congregants. The information obtained from participants was transcribed, coded and analysed using content analysis and by a computer-assisted qualitative data analysis software (CAQDAS), namely ATLAS.ti. Findings revealed a marked difference between the historical and current churchbased social service delivery suggestive of decline, accompanied by the lack of empowerment opportunities for current church leaders and congregation members in community development facilitation. Further evidence suggest the lack of cooperation between church leaders and congregation members; as well as the lack of sustainability of interventions owing to an absence of resources and the lack of capacity to create sustainable economic opportunities. Findings also revealed that church congregations are rich with people with various assets - skills, strengths, capabilities, passions, gifts, talents in various fields, which they can share with one another as congregations and communities. The discussion of findings reflected that historically, Church-based social service provision involved holistic intervention mechanisms, empowerment, collaborations and sustainability of interventions suggestive of second generation strategies of community development, while currently, Church-based social service provision involves mostly social relief and social welfare efforts suggestive of first generation strategies. The findings suggest a holistic strategy for ―reawakening the co-operativeness‖ of congregations to facilitate sustainable Church-based social service delivery towards fighting poverty in local communities. The strategy which was then developed and described with its sub-strategies, namely: Establishment of collaborative relationships Creation of empowerment opportunities Facilitation of holistic interventions The goal of the strategy is to guide church leaders and congregation members in facilitating sustainable Church-based social service programmes to fight poverty.

More specifically, the study developed a divorce-stress-adjustment model for divorced black South African men. The theoretical framework underpinning this study was that of Symbolic Interactionism that was complemented and enhanced by Erikson’s Psychosocial Theory, focusing specifically on identity development in adulthood. This was a qualitative study using an Interpretative Phenomenological Analysis (IPA) as both the research design and data analytic theory and process. The eight participants were volunteers who were recruited purposively. In accordance with IPA guidelines, data for the study was collected using biographical questionnaires and semi-structured interviews. The emerging themes were grouped into four superordinate themes, that is, pre-divorce experiences, experiencing divorce, adjustment process and post-divorce experience. Each superordinate theme had corresponding subordinate themes and subthemes. The themes were then used to develop the divorce-stress adjustment model indicating that the experience of divorce is an interconnected process. Weed’s recommendations for interpretative synthesis of interview data were used.

- Description: The removal of the uterus, medically referred to as a hysterectomy, is a permanent and irreversible procedure that potentially results in a number of immediate and significant effects. Performing a hysterectomy is at times the only medical option to a long and traumatic history of pelvic pains and surgeries. The decision to undergo this very costly, painful, and life-changing procedure can have an influence on the way women view themselves. Women’s narratives of womanhood before and after hysterectomy need to be understood within the context of women’s socially constructed identities. Gendered identity has been closely linked to genital and/or reproductive organs and the removal of these organs may influence the way women and societies define womanhood.The interpretive paradigm of constructionism with a narrative inquiry design was used as the lens to examine the women’s experiences. In-depth interviews were analysed through thematic analysis. The findings of this research revealed negative experiences resulting from severe symptoms before hysterectomy, exposed multi-layered structures of power and dominance during treatment, and indicated positive outcomes of hysterectomy. The findings should inform medical and allied practitioners regarding hysterectomy as a treatment option and add to the theoretical debate on the relationships between hysterectomy, womanhood, and identity.

- Description: An expanding body of sport psychology theory and research has highlighted the central role of the coach-athlete relationship (CAR) in the performance, satisfaction and well-being of coaches as well as athletes. Models have been developed that identify the key dimensions of CAR quality and the interpersonal behaviour that maintains it, but there remains a need for interventions that support coaches and athletes to optimise their interpersonal skills. This can be done by harnessing recent advances in personality theory which emphasise a more holistic and developmental view of the person and provide a basis for greater mutual understanding and adaptive interpersonal behaviour change in the CAR. The aims of this study were to develop the Coach-Athlete Relationship Enhancement (CARE) intervention through an integration of the Enneagram personality typology with current models of the CAR, and then to evaluate the impact of the CARE intervention on CAR quality. Sixty-two university-level athletes and their nine coaches participated in the study. Quantitative and qualitative results indicate that the CARE intervention significantly enhanced CAR quality, including direct and meta-perceptions of closeness, commitment and complementarity, as well as co-orientation. Coach-athlete relationship quality was further enhanced by re-constructing coaches’ and athletes’ personal narratives based on self-descriptions of Enneagram type strengths, and a redefinition of individuals’ core motivations to incorporate adaptive interpersonal behaviour change.

- Description: An expanding body of sport psychology theory and research has highlighted the central role of the coach-athlete relationship (CAR) in the performance, satisfaction and well-being of coaches as well as athletes. Models have been developed that identify the key dimensions of CAR quality and the interpersonal behaviour that maintains it, but there remains a need for interventions that support coaches and athletes to optimise their interpersonal skills. This can be done by harnessing recent advances in personality theory which emphasise a more holistic and developmental view of the person and provide a basis for greater mutual understanding and adaptive interpersonal behaviour change in the CAR. The aims of this study were to develop the Coach-Athlete Relationship Enhancement (CARE) intervention through an integration of the Enneagram personality typology with current models of the CAR, and then to evaluate the impact of the CARE intervention on CAR quality. Sixty-two university-level athletes and their nine coaches participated in the study. Quantitative and qualitative results indicate that the CARE intervention significantly enhanced CAR quality, including direct and meta-perceptions of closeness, commitment and complementarity, as well as co-orientation. Research findings confirmed the presence of primitive defense mechanisms associated with the lower level pathologies described by Kernberg within Hughes’ patterns. Findings confirmed the defense mechanisms as alienating others. Learnings produced were presented in a model to be used in clinical practice to support others in interaction with narcissistic individuals. - Description: Background: South Africa has the highest number of people living with human immunodeficiency virus (HIV) infection in the world. In 2014, an estimated 10.2% of the population was HIV-positive which amounted to 5.51 million people. Efavirenz forms part of the triple therapy backbone used in South Africa and is part of the firstline treatment for HIV. Efavirenz has been strongly associated with causing neuropsychiatric side effects in at least 50.0% of patients to whom it is prescribed. These side effects cause hesitation amongst healthcare professionals to prescribe this agent to patients with active mental illnesses. Aim: The aim of the study was to evaluate the neuropsychiatric side effects of efavirenz in HIV-positive psychiatric and non-psychiatric patients and to determine whether this drug may be recommended for use in an HIV-positive psychiatric patient population. Method: The study was divided into two parts, namely a quantitative portion and a qualitative portion. The quantitative study was a prospective drug utilisation study, while the qualitative portion consisted of semi-structured interviews carried out with healthcare professionals working with people living with HIV/AIDS (PLWHA). The study included five municipal clinics in the Nelson Mandela Metropole as well as two public-sector psychiatric facilities in the Eastern Cape where medical records were reviewed to obtain the information required. Patients were followed in both instances for a period of 24 weeks with follow-up assessments carried out at two, four, 12 and 24 week intervals. In terms of the qualitative study, nurses at the clinics and doctors at the hospitals were contacted and appointments for interviews were made. The interviews were recorded using a voice recorder and were transcribed and analysed using theoretical framework analysis. Results: The review of 126 medical records at the clinics revealed that no patient had suffered from or complained of a neuropsychiatric side effect. This may indicate that patients were not suffering from clinically significant side effects, side effects were not being adequately recorded by healthcare staff, or the healthcare staff were not questioning patients regarding side effects. A total of 26 hospitalised patients were followed for 24 weeks in the psychiatric facilities. Almost half of the patients using efavirenz experienced an improvement in symptoms to the extent that they were iii discharged from the facility. The majority of patients (66.7%) not on an efavirenzcontaining regimen did not improve to the point of discharge. Healthcare staff were vague when providing a definition of neuropsychiatric side effects. There were conflicting ideas on whether or not efavirenz should be used in patients with an active psychiatric illness. Conclusions: Further studies need to be performed in public-sector institutions to obtain a clearer picture of the side effects experienced by patients using efavirenz. Healthcare staff need to be encouraged to keep complete records to allow for meaningful analysis. The further integration of mental health services into existing HIV programmes is essential for holistic treatment. Patients in psychiatric hospitals demonstrated that even patients with psychiatric disorders on efavirenz can experience positive outcomes and stabilisation of psychiatric symptoms, which may indicate that these may not have due to efavirenz use. Further elucidation concerning the use of efavirenz in patients with psychiatric disorders, a description of the neuropsychiatric side effects, as well as management strategies must be provided in subsequent HIV guidelines.

Experiential learning in an undergraduate BPHARM programme: impact of an intervention on academic achievement

- Description: The extended role of today’s pharmacist with the emphasis on patient-focused care has highlighted the need for increased exposure of undergraduate pharmacy students to experiential learning in patient-centred environments, and additional skills development in therapeutics, problem solving and clinical decision making. At the Nelson Mandela Metropolitan University (NMMU), final year pharmacy students complete a university-coordinated, hospital-based, structured experiential learning programme (ELP) for the exit-level module, Pharmacology4. However, the students consistently experience difficulties in the application of pharmacological knowledge during the transition from lecture-based learning to the patient-focused clinical setting. The student population at the NMMU is diverse, with varied cultural, ethnic, language and secondary level education backgrounds, as well as different learning preferences and approaches. The extent to which these factors affect academic achievement in the experiential learning environment is unknown. Central research question The central research question for this study was therefore, “What would be the effect of an intervention aimed at supporting undergraduate pharmacy students during clinical placements, on academic achievement in, and student attitudes towards, experiential learning programmes (ELP)?” In order to explore the research question, several factors which may influence academic achievement in ELPs were investigated, namely: academic achievement (pre-university, in the BPharm programme and, in pharmacology); the admission route into the BPharm programme and the subsequent rate of academic progression; English reading comprehension ability; learning styles; problem solving ability; the extent to which students are prepared for application of knowledge in the ELP, in terms of assessment methods used prior to the final year and previous pharmacy work-based experience. In addition, the students’ lived experience of the ELP was explored, and the need for, and nature of, an intervention was determined. The research was based in a pragmatic paradigm, using an advanced mixed methods approach. An intervention-based, two-phase, quasi-experimental design was employed with an initial exploratory Preliminary Phase (in 2013) preceding the larger experimental framework (Phases One and Two, in 2014 and 2015 respectively). The research design was primarily quantitative, with pre- and post- testing conducted before and after the ELP. The ELP was completed by the comparator cohort in Phase One and the experimental cohort in Phase Two. Supplementary qualitative data was collected before, during and after the ELP. The intervention, in the form of supplementary academic support sessions, was developed from the qualitative data using an iterative approach, and implemented during the ELP in Phase Two. Attitudes and expectations of the students towards the hospital-based ELP were generally positive and realistic. Areas of concern included the difficulties experienced in the application and integration of pharmacological knowledge, both in the clinical setting and the clinical case study-based assessments; students feeling overwhelmed, inadequate and inferior in the clinical environment, compounded by an absence of clinical pharmacists as role models; and feeling unprepared for patient-focused care. The qualitative data strongly supported the need for supplementary academic support sessions. The intervention was developed and implemented in Phase Two, using patient case-based, active learning strategies. The majority of students (91.0%; n = 104) reported improved case analysis skills. A statistically significant (p = .030, Cohen’s d = 0.34) improvement was noted in the summative Pharmacology4 assessment marks obtained by the experimental cohort post-intervention, although of small practical significance. Predictors of academic achievement in the ELP were found to be language, specifically English reading comprehension skills, academic achievement in the BPharm programme and pharmacology, the university admission score, the rate of academic progression, and problem solving ability. Previous pharmacy-based work experience and assessment questions requiring application of knowledge were also found to influence achievement in the ELP. The need for an intervention in the form of supplementary academic support sessions was confirmed. - Description: Historically psychobiography has focused to a large extent on the study of pathology in human development. In recent years, scholars in the field of psychobiography have recommended that studies with a eugraphic focus be undertaken, thereby investigating optimal human development. Milton Hyland Erickson (1901-1980) was an American psychologist and psychiatrist. An extraordinary creative individual who conducted pioneering work in the field of hypnosis. Whilst vast publications of his academic work exist and his work is continued through the Milton H. Erickson Foundation and its global subsidiaries, in comparison, his life story has not been the focus of much study. The psychobiographical merit of Erickson as the subject of the study was based on his involvement in interesting and important events, as well as his resilient attitude towards life, during even dire moments. The focus of this psychobiography was to conceptualise Erickson’s life in terms of the principles of Adler’s Individual Psychology and Peterson and Seligman’s Character Strengths and Virtues Classification. Through this process it was aimed to provide descriptions and interpretations of Erickson’s personality in order to illuminate aspects such as his creativity, service to others, and optimism. The study is written from the first person perspective and is rooted in the interpretive paradigm. It utilises a qualitative research approach in order to gain an in-depth understanding of Erickson’s finished life and thereby taking into account contextual factors. The two theoretical frameworks guided the iterative data collection and data analysis processes and the data were organised into a descriptive framework. The method of data analysis was idiographic thus describing the uniqueness of Erickson life. Strategies such as the triangulation of data sources and theory, focusing on aspects of saliency, trustworthiness, and ethics were employed to ensure reliable data extraction and interpretation. The study revealed that both theoretical frameworks were appropriate in their description and interpretation of Erickson’s personality and character strengths. Of significance was the protective role that creativity, social interest, hope, and optimism played against trauma and these emerged as central themes in Erickson’s life in terms of his psychological wellbeing. The findings on creativity and social interest are supported by existing research and theory but also highlight perspectives for future research. Creativity is a key component of knowledge production and therefore research in this area could improve our understanding of how it can be nurtured and developed across the lifespan. Whilst the construct of social interest was integral to this study its validity in terms of providing adequate explanations of how individuals balance their own needs against the needs of others was examined and suggestions of where the theory can be elaborated were highlighted.

- Description: Historically psychobiography has focused to a large extent on the study of pathology in human development. In recent years, scholars in the field of psychobiography have recommended that studies with a eugraphic focus be undertaken, thereby investigating optimal human development. Milton Hyland Erickson (1901-1980) was an American psychologist and psychiatrist. An extraordinary creative individual who conducted pioneering work in the field of hypnosis. Whilst vast publications of his academic work exist and his work is continued through the Milton H. Erickson Foundation and its global subsidiaries, in comparison, his life story has not been the focus of much study. The psychobiographical merit of Erickson as the subject of the study was based on his involvement in interesting and important events, as well as his resilient attitude towards life, during even dire moments. The focus of this psychobiography was to conceptualise Erickson’s life in terms of the principles of Adler’s Individual Psychology and Peterson and Seligman’s Character Strengths and Virtues Classification. Through this process it was aimed to provide descriptions and interpretations of Erickson’s personality in order to illuminate aspects such as his creativity, service to others, and optimism. The study is written from the first person perspective and is rooted in the interpretive paradigm. It utilises a qualitative research approach in order to gain an in-depth understanding of Erickson’s finished life and thereby taking into account contextual factors. The two theoretical frameworks guided the iterative data collection and data analysis processes and the data were organised into a descriptive framework. The method of data analysis was idiographic thus describing the uniqueness of Erickson life. Strategies such as the triangulation of data sources and theory, focusing on aspects of saliency, trustworthiness, and ethics were employed to ensure reliable data extraction and interpretation. The study revealed that both theoretical frameworks were appropriate in their description and interpretation of Erickson’s personality and character strengths. Of significance was the protective role that creativity, social interest, hope, and optimism played against trauma and these emerged as central themes in Erickson’s life in terms of his psychological wellbeing. The findings on creativity and social interest are supported by existing research and theory but also highlight perspectives for future research. Creativity is a key component of knowledge production and therefore research in this area could improve our understanding of how it can be nurtured and developed across the lifespan. Whilst the construct of social interest was integral to this study its validity in terms of providing adequate explanations of how individuals balance their own needs against the needs of others was examined and suggestions of where the theory can be elaborated were highlighted.

In general, it consists of a combination of two central elements: biography and psychological theory, which aim to explain the particular individual‟s psychological development. This particular study serves to explore the extraordinary life of renowned singer and songwriter, Ian Kevin Curtis (1956-1980), who died by suicide at the early age of 23. The basis for this investigation will take the form of notable biographical accounts of the subject‟s life, together with the application of Thomas Joiner‟s (2005) interpersonal-psychological theory of suicide which identifies factors that lead to suicidal ideation - in an attempt to understand the psychological circumstances that contributed to Curtis‟s suicide. In doing so, Adler‟s (1929) theory of Individual Psychology was applied to the life of Curtis in an attempt to build on Joiner‟s theory. This led to the concept of control being introduced and contributed to the development of an intake form to identify those at risk for suicide. It is hoped that exploring the psychological circumstances that contributed to Curtis‟s suicide and their interpretation by the subject will bring about an understanding of the risk factors that may induce suicide and, by extension, will highlight the relevance of this psychobiographical study as a tool for investigating and promoting preventative measures concerning suicide. The psychobiographical data collection and analysis for this research thesis will be guided by Yin‟s (2003) theory of „analytic generalisation‟ which uses a theoretical framework in selecting relevant data which develops a matrix as a descriptive framework for organising and integrating that data, and Alexander‟s (1988) analytical model which focuses on lifting out themes through principal identifiers of salience.

Research indicates that individuals do experience meaningful online romantic love relationships and that these relationships often lead to face to face (FTF) relationships. However, limited research has been done on exploring the experiences of those who are/were involved in online romantic love relationships. Furthermore, research conducted on online love romantic relationships generally fails to investigate how people experience passion online, hereby ignoring this integral component of romantic love. The primary aim of the research study was to explore young adults’ experiences of romantic love relationships in virtual space. More specifically, the study explored how young adults experienced intimacy and passion as elements of romantic love online. The study was viewed from an interpretative paradigm and made use of a qualitative approach. The researcher conducted in-depth, semi-structured interviews with seven participants which were transcribed, and analysed using Interpretative Phenomenological Analysis (IPA). Four superordinate themes were identified, namely, Online Intimacy, Online Romance and Passion, Online Love, and Social Exchange Online. The participants experienced romantic love online and reported that these relationships were very significant, real and impacted on their psychological well-being. The study created a heuristic base that will provide impetus for this emerging field in research.
