- The study was triggered by the number of process safety incidents that are taking place in South African oil companies. The impact of these process safety incidents has resulted in multiple fatalities, revenue loss, fires and explosions in the past. The problem is so immense that the process safety actions, coming from the process safety studies to prevent and mitigate these incidents have been ignored, not attended to, not implemented or not understood by the oil industries for the past 15-20 years. Hence the process safety incidents are still happening and the oil industry has not learnt from previous incidents.
