- Description: Gastroenteritis, commonly known as diarrhoeal disease, is one of the top killers responsible for substantial human morbidity and mortality especially in third world countries where most people do not have access to potable water and where hygiene levels are low. Many bacterial, viral and protozoal agents are known causes of gastroenteritis and viral gastroenteritis is responsible for over 70% of cases. Rotaviruses are the main causes of viral gastroenteritis and are responsible for most of the cases worldwide. Other viral agents associated with this disease include human noroviruses, Aichi virus 1, enteric human bocavirus, enteric human adenovirus and many other emerging viral agents such as klassivirus, Saffold virus, cosavirus and others. In 2009 the South African government introduced a rotavirus vaccine, RotaRixTM into the expanded programme on immunisation (EPI). More than a 50% decrease in diarrhoea related morbidity and mortality due to rotavirus infections was noted during surveillance studies on the efficacy of the vaccine. However, over 40% of cases of gastroenteritis are of unknown aetiology. The present study aimed to perform a preliminary study to investigate the presence of Aichi virus 1 and enteric human bocaviruses in the Eastern Cape Province by the use of molecular techniques. Furthermore, the study aimed to add to the limited molecular data about enteric adenoviruses in South Africa. Samples used in this study were swab samples collected from Belmont Valley Wastewater Treatment Plant in Grahamstown, South Africa, as well as mussel samples collected from the Swartkops River in Port Elizabeth, South Africa. Both raw sewage and shellfish give a broad idea of what microbes are circulating in the communities. In the present study, twenty swabs and twenty mussel samples were prepared by centrifugation, sonication and filtration. Samples were then subjected to transmission electron microscopy (TEM) analysis, for which the electron micrographs revealed presence of viral particles with diameters ranging from around 20 nm to just over 100 nm. Viral nucleic acids were extracted from 140 μL of the twenty swabs and twenty mussels samples using the QIAamp® Viral RNA Mini Kit, following manufacturer‟s instructions. For detection of Aichi virus 1 from the swab and mussel samples three reverse transcriptase- polymerase chain reaction (RT-PCR) assays using the Verso 1-Step RT-PCR Hot-Start Kit were developed. The first RT-PCR assay targeted amplification of the highly conserved 5′ UTR using published primers. However, despite many amplification attempts no positive results were obtained from both swab and mussel samples. It was only after the addition of DMSO (to a final concentration of 10%) that one swab sample was positive for this assay. Although only a few samples were analysed, this study is the first to confirm the presence of Aichi virus 1 and enteric bocaviruses in South Africa and provides a platform for further investigation into prevalence and epidemiology of these viruses in the country.

Coping with HIV and AIDS in marginal communities: a case study of Chivanhu Settlement in Nemanwa, Masvingo, Zimbabwe

- Description: This thesis seeks to understand and analyse HIV and AIDS and rural livelihoods in Zimbabwe with particular reference to an isolated and marginalised informal settlement called Chivanhu in Masvingo Province. The focus is specifically on questions around HIV susceptibility, AIDS vulnerability and household resilience. In this regard, it is important to recognise that HIV and AIDS cannot be lumped together as one medical or social condition. Rather, there is a progression from HIV infection to AIDS-related chronic illnesses to possible death, and livelihood strategies often alter along this HIV and AIDS time-line. Zimbabwe for over a decade now has gone through a series of economic and political crises which have impacted detrimentally on both urban and rural livelihoods, even for those households which are not directly affected by the HIV and AIDS pandemic. With the economy in free-fall, households have had to pursue a range of livelihood strategies in order to sustain themselves. These socio-economic conditions have in many ways facilitated susceptibility to HIV infection and vulnerability to AIDS. Many studies have examined this in relation to wellentrenched and stable communities in rural Zimbabwe. But the livelihood dynamics for such communities are significantly different to more unstable and informal settlements like Chivanhu, as thesis seeks to show. At the same time, the thesis offers a longitudinal study which is able to map the changes to the livelihoods of infected and affected households in Chivanhu. Though recognising the debilitating effects of the pandemic on these households, it also raises questions about the possible resilience of certain households despite great adversity. In doing so, it goes beyond the individual and household levels of analysis to consider the role of clusters (or groups of households) in responding to the impacts of HIV and AIDS. In this regard, particular emphasis is placed on gender and orphanhood. In the end, the thesis offers a nuanced analysis of the everyday complexities and challenges for affected households in a marginalised and informal rural community in Zimbabwe and thereby makes a contribution to re-theorising HIV and AIDS and rural livelihoods more broadly.

- Description: A critical care unit is a dynamic and highly technological environment. Professional nurses who have been working in the critical care unit for a period of time are passionate about the environment in which they work. They find their on duty time challenging and stimulating. The critical care environment is slowly changing. Due to the fact that there are fewer professional nurses with an additional qualification in critical care available to work in the critical care units. The utilisation of an increasing number of agency nurses leads to an increase in sub-standard nursing care as well as dissatisfied doctors and patients. The shortage of critical care staff has resulted in the need to find an alternative human resources framework and still provide cost effective, safe quality patient care. This leads to the design and implementation of a team nursing care framework for critical care. The research objectives for this study were: - To explore and describe the experiences of professional nurses with regard to a team nursing care framework in private critical care units. - Develop guidelines to optimize the team nursing care framework in critical care units in a private hospital group. The research is based on a qualitative, explorative, descriptive and contextual research design. The study is based on a phenomenological approach to inquiry. Eleven in-depth semi structured face-to-face phenomenological interviews were utilized as the main means of collecting data. A purposive, criterion based, sampling method was used. Specific inclusion criteria were met and consent was obtained from the participants and from the management of the private clinic where the research was conducted. Two central themes were identified:- Theme One: The professional nurses experienced the team nursing care framework in the critical care unit as a burden. Six sub-themes were identified. - Theme Two: Professional nurses made recommendations for improvement of the team nursing care framework in the critical care unit. By describing the lived experiences of the professional nurses in the critical care units, based on research interviews, the researcher painted a clear picture of the team nursing care framework in the critical care unit. Guidelines were developed based on the identified themes. The broad guidelines are aimed at ensuring that the nurses are competent to care for critical care patients prior to them commencing work in the critical care unit. The researcher concludes this study by making recommendations for Nursing practice, education and research.

Students' understanding of metaphor as a linguistic category and comprehension of metaphorical texts are analysed in terms of the principle of relevance. Patterns of comprehension in English first language (Ll) and English second language (ESL) students' analyses of metaphorical texts are discussed and through an analysis of similarities and differences in these patterns of comprehension an attempt is made to develop a pedagogy around relevance theory. Relevance theory's particular emphasis on the role played by "context" in cognition is seen to have significance for the teaching of literature in South African universities. Relevance theory's account of cognition generates a range of educational principles which could be specifically applied to the teaching of metaphor. An appraisal of the strengths and difficulties students experience in expressing their understanding of metaphor in an academic context is included. This was done to further develop relevance theory into a pedagogical approach which takes into account the academic context in which writing occurs.
