Experiences of patients on short term drug resistant tuberculosis regimen at Nelson Mandela District TB Specialist Hospital

Patients on XDR-TB treatment face many challenges which include side effects which they find hard to tolerate and they end up stopping the treatment. Socio-economic difficulties of concern include delayed social assistance from the government to support their families, particularly when they are breadwinners. A qualitative, descriptive, explorative contextual design was used in this research study. A non-probability convenience sampling method was employed. The target population for this study consisted of those DR-TB patients who were on short-term regimen and semi-structured individual interviews were conducted. There were twelve participants who voluntary participated in the research study although the first interview was a pilot study. Ethical principles were adhered to throughout the study. A total of seven themes and twenty-seven sub-themes arose during the data analysis and were fully discussed. Confidentiality and anonymity was ensured throughout this research. The research findings showed that the participants shared positive experiences in as far as being able to easily access the health care service. Most participants were moved through a range of emotions which included shock. They felt heartbroken about the devastation and the disruptive nature of this illness to themselves and their family lives and, more seriously, were fearful of the morbidity and mortality thoughts which pervaded their thoughts but their health improved when drug resistant TB treatment was initiated. Many of the participants found it easy to disclose to their friends as they knew they could count on their support at the best and worst of times in their treatment journey. In conclusion the participants provided suggestions regarding their support needs on the journey to recovery from DR-TB. The researcher provided certain recommendations as far as the challenges expressed by the participants and these will contribute to strengthening the DR-TB adherence strategies. , Thesis (MPH) -- Faculty of Health Sciences, 2021

An investigation into the main causes for poor OEE at the Struandale Engine Plant of Ford Motor Company of Southern Africa

- Description: The Struandale Engine Plant (SEP) was set up in 1924 and was the 16th Ford plant outside North America. The plant has since produced a variety of different components ranging from wheel hubs, to engine components such as crank shafts and cylinder blocks, to the assembly of engines. Manufacturing at this plant has predominantly been via the job-shop method, where mass production techniques were used. The RoCam programme preparation started in 1999. In 2001, SEP experienced a very steep ramp-up due to an unexpected surge in volume requirements. This forced the plant’s Human Resources team to embark on an “emergency” recruitment drive to cover the required human resources to support the programme. Due to this action being taken by the Human Resources department, the educational level of the employees was at a very basic level and had an adverse effect on productivity. The SEP operating committee made a decision in 2004 to focus on the implementation and maintenance of the Ford Production System (FPS). This initiative was important because ageing equipment needed to be maintained at predetermined intervals through continuous improvement techniques that form part of the FPS programme. If these techniques are not in place, the Overall Equipment Efficiency (OEE) suffers. This programme comprises a number of critical elements such as FTPM that requires a certain education and skill level in effort to completely comprehend the system. Evidently, education was an issue and an effort had to be made to upgrade the workforce skill’s level, specifically the team leaders. On the 30th January 2008, Ford Motor Company of Southern Africa (FMCSA) had announced plans to invest more than R1.5-billion to expand its operations in South Africa for the production of Ford's next-generation compact pick-up truck iv and Puma diesel engine. In an effort to secure and retain the Puma contract at SEP, the plant had to prove that it could compete globally; hence it had to improve OEE levels to the world-class standard. The Puma engine contract had been contracted to South America, Turkey, Thailand, and South Africa. This was a strategic move in order to retain flexibility within the supply chain i.e. if one supplier / plant cannot deliver, demand can be shifted to one of three alternative suppliers. The problem statement which will be addressed by this research is: What are the main causes for the poor Overall Equipment Efficiency level (OEE) at the Struandale Engine Plant of Ford Motor Company of Southern Africa? The dyes exhibit good photodynamic activity against MCF-7 cells with IC50 values of 14.3 and 8.5 μM, respectively. The hexyl quaternized dye exhibited a 9.69 log reduction value (0.5 μM) towards S. aureus under illumination for 90 min (250 mW cm−2). The results demonstrate that this set of structural modification strategies for photosensitizer dyes merits further in depth study.

Understanding Generation Y : an investigation of how Hilton College can use technology for communication

- Description: Background to the academic research. Hilton College is an all boys private school in the Kwa-Zulu Natal Midlands. Since the beginning of 2012 boys have been permitted to carry their phones during the school day, (Thomson, 2012). Wireless internet access was also extended across the campus to allow boys and staff to bring their own devices to class and have access to the internet (Machlachlan, 2012). In the past, communication between staff and boys was either verbal or through the use of a daily notice, called the Daily Routine Order (DRO). The DRO is placed at the dining room tables each day and boys are expected to check for any notices that may pertain to them. These notices are emailed to a secretary who prints them out and places them in the dining room each day at lunch. 2. Objectives to the research. This research sought to establish whether current communication methods were effective or whether the prevalence of mobile phones and wireless devices had presented the school with a more effective method. It was important therefore, to investigate what devices were on campus, how those devices were used and when they were used. In addition to this, the research sought to establish communication preference among staff and boys. 3. Research Methodology. The research was conducted through the use of a qualitative survey that was conducted among both staff and boys. Staff and boys were surveyed to establish generation, device prevalence, device usage and device preference. This data was collected using an anonymous qualitative survey that was administered through the use of Google docs in the school labs. 187 Boys in 3 grades 8 and 9 participated in the survey along with 18 staff members. The data was then analysed and compared to data from similar research that had been conducted elsewhere in the world. Because of significant age differences between boys and staff it was critical to establish whether device usage among staff and boys was tied to their generation. Of particular interest were the device behaviours associated with the Millennial generation, who were born between 1982-2002 (Strauss 2005:10). This generational ‘benchmark’ allowed the researcher to compare the Hilton College cohort to those whose device behaviours and usage had occurred and been discussed in generational research elsewhere. It also allowed for certain assumptions to be made about staff and boys based on their generational profile. 4. Findings It was found that staff and boys exhibited communication traits that were consistent with their generation and consistent with other research in this field. It was also found that staff and boys preferred mobile and digital forms of communication over verbal announcements and the DRO system. For this reason, it was recommended that the Hilton College establish a system that took advantage of technology to improve on communication. Various recommendations were given by the researcher that took advantage of current technologies. These included the use of SMS, Instant Messaging, Social Networks and email. It was also recommended that the school consider prescribing devices that allow for communication between staff and boys.

- Description: A number of studies by academics, scholars and researchers have been conducted on the subject of remittances. The focus of these studies has been on developing countries where remittance corridors have long been established. These studies have focused more on the effects of remittances at micro level, in other words poverty alleviation as result of the decline of FDI; PI and ODA. Established remittance corridors are found in Asia, the Pacific Rim, the Middle East, South America, the Caribbean, West Africa as well as North Africa. Few studies have been conducted into new emerging corridors that are still being established, such as the South Africa-Zimbabwe, Botswana-Zimbabwe and the Namibian-Zimbabwe corridors where a large number of Zimbabweans have migrated to. Economic growth and development are major challenges facing developing countries due to lack of finance. With the decline of FDI, PI, ODA as well as credit since the global financial crisis in 2008, there is a need for research to determine other alternative sustainable sources of finance to enable economic growth and development. Available literature and empirical evidence on the subject matter suggest remittances as a complementary source not a substitute of FDI, PI, ODA and credit. This means that governments in developing countries should not neglect their duty to collect revenue, should promote FDI and PI and not be dependant solely on remittances. Like FDI, PI and ODA, the flow of remittances is also determined by the socio-economic and political factors. Any negative effect on the socio-economic and political factors may also lead to the decline of remittance flows. Whilst other developing countries in established remittance corridors have put policies and systems in place to harness and ensure maximum benefits of remittances, countries such as Zimbabwe have not done much to realise potential and the impact that remittances can make. This is evidenced from the lack of reliable data which according to Chami et al. (2008:21) places severe constraints on the types of questions that can be asked and conclusions that can be drawn from statistical analyses. Secondly, emigration from Zimbabwe can be regarded to be in its infancy stage compared countries like Nigeria and Egypt where migration to developed countries has reached maturity stage. The evidence of this is seen on the volume of remittances currently being received by these two countries. Therefore, this study will complement existing data and literature available particularly on this corridor. Remittances are channelled either through formal or informal channels. The literature available and empirical evidence suggests that the bulk of remittances are channelled through informal channels as opposed to formal channels (Crush et al.,2012:20). Within the South Africa-Zimbabwe corridor, 85 percent of remittances to Zimbabwe are channelled informally (von Burgsdorff, 2012: 17) and are not captured in the official statistics such as the Balance of Payments. The drawback of this is lack of accurate data which precludes more rigorous statistical analysis in this field (von Burgsdorff, 2010:12). This study will endeavour to capture valuable statistics with regards to remittances. Most developing countries that have relied on aid are also burdened with huge unsustainable external debts (Obasanjo, 2000). The external debts continue to have an adverse effect of slowing down economic growth and development as these countries have an obligation to pay back the debts. Settling of external debts has been one of the biggest challenges facing developing countries. With the rise of remittances, Olubiyi (2013:1) sees them as a replacement to credit to a country that is constrained. Instead of borrowing finance for growth and development, remittances can be used instead. Apart from substituting credit with remittances, countries which are not burdened with external debts are able to use remittances as a leverage to obtain foreign loans (Taylor, 1999:69). The loans acquired can used for supplementing revenue derived from the fiscus and other sources of external finance such as FDI, PI and ODA.

Few studies have been conducted into new emerging corridors that are still being established, such as the South Africa-Zimbabwe, Botswana-Zimbabwe and the Namibian-Zimbabwe corridors where a large number of Zimbabweans have migrated to. Economic growth and development are major challenges facing developing countries due to lack of finance. With the decline of FDI, PI, ODA as well as credit since the global financial crisis in 2008, there is a need for research to determine other alternative sustainable sources of finance to enable economic growth and development. Available literature and empirical evidence on the subject matter suggest remittances as a complementary source not a substitute of FDI, PI, ODA and credit. This means that governments in developing countries should not neglect their duty to collect revenue, should promote FDI and PI and not be dependant solely on remittances. Like FDI, PI and ODA, the flow of remittances is also determined by the socio-economic and political factors. Any negative effect on the socio-economic and political factors may also lead to the decline of remittance flows. Whilst other developing countries in established remittance corridors have put policies and systems in place to harness and ensure maximum benefits of remittances, countries such as Zimbabwe have not done much to realise potential and the impact that remittances can make. This is evidenced from the lack of reliable data which according to Chami et al. (2008:21) places severe constraints on the types of questions that can be asked and conclusions that can be drawn from statistical analyses. Secondly, emigration from Zimbabwe can be regarded to be in its infancy stage compared countries like Nigeria and Egypt where migration to developed countries has reached maturity stage. The evidence of this is seen on the volume of remittances currently being received by these two countries. Therefore, this study will complement existing data and literature available particularly on this corridor. Remittances are channelled either through formal or informal channels. The literature available and empirical evidence suggests that the bulk of remittances are channelled through informal channels as opposed to formal channels (Crush et al.,2012:20). Within the South Africa-Zimbabwe corridor, 85 percent of remittances to Zimbabwe are channelled informally (von Burgsdorff, 2012: 17) and are not captured in the official statistics such as the Balance of Payments. The drawback of this is lack of accurate data which precludes more rigorous statistical analysis in this field (von Burgsdorff, 2010:12). This study will endeavour to capture valuable statistics with regards to remittances. Most developing countries that have relied on aid are also burdened with huge unsustainable external debts (Obasanjo, 2000). The external debts continue to have an adverse effect of slowing down economic growth and development as these countries have an obligation to pay back the debts. Settling of external debts has been one of the biggest challenges facing developing countries. With the rise of remittances, Olubiyi (2013:1) sees them as a replacement to credit to a country that is constrained. Instead of borrowing finance for growth and development, remittances can be used instead. Apart from substituting credit with remittances, countries which are not burdened with external debts are able to use remittances as a leverage to obtain foreign loans (Taylor, 1999:69). The loans acquired can used for supplementing revenue derived from the fiscus and other sources of external finance such as FDI, PI and ODA.

The results of the interviews and the survey showed that performance management was not applied as a continuous and developmental process at TNPA, and that it was often perceived as punitive. The data bases searched included: MEDLINE, CINAHL, PubMed and Google Scholar. Manual searches were done and completed with the assistance of librarians. A total number of n=50 studies were identified as potentially relevant to the study. The number or articles included for critical appraisal were 20. On completion of the critical appraisal n=17 articles were identified for the review. The included studies for the review were n=7 Level 1, systematic reviews and n=10 Level 2, quasi-experimental studies. Three studies were excluded after critical appraisal from two reviewers, appraisal was done independently, and consensuses were reached between the two reviewers. The Joanna Briggs Institutes critical appraisal and data extraction instruments were used for the study. The descriptive data synthesis was done of the included studies as well as a comparison of teaching strategies to determine which one to better than the other one. Although n = 4 of the teaching strategies (concept mapping, internet-based learning, evidence based interactive strategy and cultural competence) significantly increased knowledge, the overall results found that a variety of teaching strategies to be implemented to increase the knowledge outcomes of the nursing students. The different teaching strategies found were: e-learning, concept mapping, internet-based learning, web-based learning, gaming, problem-based learning, and case studies, evidence based learning and cultural competence. However, more research is needed to investigate the best use of the different teaching strategies and compare the impact of a variety of teaching strategies on increasing knowledge of the nursing student.

The conventional understanding of waiting is as an intentional relationship between a waiting subject and an awaited object. This kind of waiting is end-directed, and, in order for it to be worthwhile, the awaited must, at some point, arrive. In the trilogy, however, the awaited never does arrive, and it is my contention that the novels are concerned with an unconventional kind of waiting, which, being without object or end, takes the form of a non-intentional relationship between waiter and awaited. Significantly, through the non-intentional wait, the subject awaits the unawaited. She or he thereby encounters the radically other, or that which cannot be rendered familiar or assimilated in any way – an unthinkable, unspeakable, ungraspable excess that overflows the limits of thought and language. The texts foreground the vexed question of response to such alterity: how can one approach the ungraspable as ungraspable, when it is in the nature of any approach to attempt to grasp? However, the lack of a common understanding about knowledge itself, its characteristics and how it is constructed has led to diverse approaches about how to "manage" it. This study presents a critical overview of traditional and contemporary KM approaches. The main focus of this study was to discover and apply a suitable methodology for assessing an organisation's knowledge processing environment. This includes an analysis of the current practices and behaviours of people within the organisation relating to the creation of new knowledge and integrating such knowledge into day-to-day work. It also includes inferring from the above practices those policies and programmes that affect knowledge outcomes. This research makes extensive use of the Knowledge Life Cycle (KLC) framework and the Policy Synchronisation Method (PSM) developed by advocates of the New Knowledge Management movement. A case study approach was followed using a range of data collection methods, which included personal interviews, a social network survey and focus group discussions. The selected case is the small IT department at the East London campus of Rhodes University. Evidence from the case suggests that the knowledge processing environment within the IT department is unhealthy. The current knowledge processing practices and behaviours are undesirable and not geared towards the creation of new knowledge and the integration of such knowledge within the business processes of the IT department. There is little evidence of individual and organisational learning occurring and the problem solving process itself is severely hampered by dysfunctional knowledge practices. The study concludes that the above state of affairs is a reflection of the quality and appropriateness of policies and programmes in the extended organisation. Equally, the local definition of rules, procedures and the execution thereof at a business unit level is mostly lacking. The study illustrates that a systematic assessment of the knowledge processing environment provides the organisation with a sound baseline from where knowledge-based interventions can be launched.

A mixed-method exploratory-descriptive research design was employed to collect the quantitative data by means of the Curiosity and Exploratory Inventory. The qualitative data were collected using semistructured interviews to explore how psychologists-in-training have dealt with uncertainty. Purposive-availability sampling was used to select the participants at three South African universities. A total number of 50 participants completed the CEI and six participants were interviewed. The data were analysed using mixed-method data analysis. It was found that participants had moderate-to-high levels of curiosity and exploration, with higher levels of exploration than absorption. The majority of participants reported that they actively sought as much information as they could and frequently looked for new opportunities to grow as persons. The strategies used by the selected group of participants to deal with uncertainty included: reliance on clinical supervision; consultation with peers; self-enhancement; reliance on theory; learning from practical experience; using certain cognitive appraisals; and self-care. Certain conclusions and recommendations were made based on the findings of the study.

- Description: This work addresses some of the difficulties encountered while working in psychotherapy with a residual schizophrenic. While there is an abundance of literature on psychotherapy for schizophrenia, both supporting and also refuting its merit, what the literature fails to reveal is that there appears to be a class of schizophrenic who, while apsychotic and able to communicate in the everyday sense of the word, is in a psychic space which speaks of a break with the basic relational elements of the human order. The quality of the patient's psychic life is such that almost nothing of what the literature describes as useful and appropriate in working with schizophrenics seems to help in the psychotherapeutic work with this type of patient. This study describes these issues with relevance to a particular residual schizophrenic. The Illustrative-didactic case-study method was used to discuss the four-and- a-half month psychotherapy with this patient. The patient's early developmental history, premorbid personality functioning, family and interpersonal relationships, mental state, diagnosis and a rationale for psychotherapy were presented and considered in detail. The structure of the psychotherapeutic process was reviewed in depth. The hermeneutic guidelines to understanding the case were drawn from Object-Relations Psychoanalytic theory, particularly Balint, Khan, Karon & VandenBos, Bollas, Romanyshyn, Perry, Symington, Fordham and others. Various psychic and personality features, as unveiled through the psychotherapeutic process, were elaborated and the implications of these for the therapeutic endeavour were considered as follows: Firstly, the psychic space of the patient, which precluded mirroring, symbolization find object-relationship - and which made psychotherapy untenable, was discussed. Secondly, therapeutic ambivalence and other counter-transference issues were reviewed. Thirdly, the shadow sides of both therapeutic optimism and of psychotherapeutic change were considered. Fourthly, the issues of therapeutic failure and of other treatment possibilities for a residual schizophrenic patient were examined. It was concluded that there needs to be an important countertransference shift with regard to the psychotherapeutic goals for those patients whose condition may be chronic, and for whom it appears that psychotherapy is not going to be of any therapeutic benefit - find where an 'empathic accompaniment' might be as much as it is possible to hope for or achieve.

- Description: This work addresses some of the difficulties encountered while working in psychotherapy with a residual schizophrenic. While there is an abundance of literature on psychotherapy for schizophrenia, both supporting and also refuting its merit, what the literature fails to reveal is that there appears to be a class of schizophrenic who, while apsychotic and able to communicate in the everyday sense of the word, is in a psychic space which speaks of a break with the basic relational elements of the human order. The quality of the patient's psychic life is such that almost nothing of what the literature describes as useful and appropriate in working with schizophrenics seems to help in the psychotherapeutic work with this type of patient. This study describes these issues with relevance to a particular residual schizophrenic. The Illustrative-didactic case-study method was used to discuss the four-and- a-half month psychotherapy with this patient. The patient's early developmental history, premorbid personality functioning, family and interpersonal relationships, mental state, diagnosis and a rationale for psychotherapy were presented and considered in detail. The structure of the psychotherapeutic process was reviewed in depth. The hermeneutic guidelines to understanding the case were drawn from Object-Relations Psychoanalytic theory, particularly Balint, Khan, Karon & VandenBos, Bollas, Romanyshyn, Perry, Symington, Fordham and others. Various psychic and personality features, as unveiled through the psychotherapeutic process, were elaborated and the implications of these for the therapeutic endeavour were considered as follows: Firstly, the psychic space of the patient, which precluded mirroring, symbolization find object-relationship - and which made psychotherapy untenable, was discussed. Secondly, therapeutic ambivalence and other counter-transference issues were reviewed. Thirdly, the shadow sides of both therapeutic optimism and of psychotherapeutic change were considered. Fourthly, the issues of therapeutic failure and of other treatment possibilities for a residual schizophrenic patient were examined. It was concluded that there needs to be an important countertransference shift with regard to the psychotherapeutic goals for those patients whose condition may be chronic, and for whom it appears that psychotherapy is not going to be of any therapeutic benefit - find where an 'empathic accompaniment' might be as much as it is possible to hope for or achieve.

At a time when empowerment and its future is under intense discussion in South Africa, it seems wise to move away from quantitative studies which do not facilitate the development of comprehensive theory in industrial psychology. This study provides a qualitative feminist analysis of women's understandings and experiences of empowerment in an organisation. Written protocols, interviews and a workshop were used as data collection tools and seven women from one organisation participated in the study. The research revealed that women understand and experience empowerment in a number of ways. These understandings and experiences are affected by various factors: organisational factors; personal characteristics and abilities; their relationship with others at work and at home; and societal factors such as double standards for men and women and role expectations. The breadth and scope of the results imply that any attempt to empower women should include relational, motivational and feminist perspectives on power and empowerment. In addition, the results indicate that providing a space in which the women could explore the network of disempowering practices in their lives, was empowering for the women. Through the process of the research, the participants' understandings of empowerment evolved from viewing empowerment as something that is predominantly external (for example, influenced by others and organisational factors) to something that is internal (for example, influenced by motivational factors). This study cautions against seeing empowerment as something that is solely internal because by doing so women are placing the responsibility of empowerment upon themselves thus setting themselves up for failure. However, through the process of seeing empowerment as internal, the women were able to move towards a feminist understanding of empowerment in which not only is empowerment external ("out there") or internal ("within") but includes acknowledging one's own responsibility in empowerment as well as external societal factors that serve to hamper women.

The objectives of this study were (i) to evaluate the effect of bioslurry effluent on seedling growth of Swiss chard (Beta vulgaris L.), (ii) to study the effect of bioslurry effluent on growth, development and the nutrient content of Swiss chard and (iii) to evaluate the residual effect of bioslurry on biological yield and nutrient content of Swiss chard. Experiment 1 was conducted in a glasshouse using two cultivars (Fordhook giant and Star 1801) seeds planted in 200 cavity trays using Hygromix. The treatments were: control (no fertiliser), 50%Bioslurry, 100%Bioslurry, 200%Bioslurry and the recommended rate of 2:3:4 (30) NPK fertilizer. The experiment was arranged in a randomized complete block design replicated three times. At four weeks after germination, ten seedlings per treatment were destructively sampled randomly to determine shoot height and root length, seedling mass, fresh shoot and fresh root mass, dry mass and nutrient content of leaves. The results showed that fertilization with 100%Bioslurry produced significantly taller seedlings (16.33cm) and shoot height (8.72cm), while the shortest seedlings were obtained from the recommended inorganic fertilizer. Fertilization with 200%Bioslurry and the control treatment produced significantly higher biological yield. compared to other treatments. The interaction between fertilizer treatments, cultivar and season indicated that summer season performed better compared to winter in seedling growth and development, and Fordhook giant fertilized with 100% and 200%Bioslurry was superior compared to Star 1801. Fertilization with 50%Bioslurry resulted in seedlings with higher sodium, while the seedlings grown in the control treatment showed higher copper content. The highest manganese was obtained from the seedlings fertilized with the inorganic fertilizer. In Experiment 2, 10 plots of 4.5m² were established in a greenhouse and were replicated three times, using the same cultivars and fertilizer treatments as in Experiment 1. One seedling was planted per hill at a spacing of 0.25m between plants and 0.5 m between rows and this accommodated 28plants per plot. At 20-day intervals, five plants were randomly selected for data collection until 60 days after transplanting. The cultivar significantly affected growth and biological yield parameters, with Fordhook giant performing better in all the three sampling intervals compared to Star 1801 during summer growing season. Fertilizer application positively affected growth and biological yield parameters and plants treated with 200%Bioslurry and the recommended rate of inorganic fertiliser produced the best results. Experiment 3 was carried out in the same manner as experiment two, except that no fertilizer was applied and the seedlings were planted in corresponding treatment to evaluate the residual effect of bioslurry and inorganic fertilizer. The inorganic fertilizer had the highest residual effect compared to bioslurry. Developers do not need to have an understanding of Session Initiation Protocol event signaling specifications or of the XML Configuration Access Protocol to use the tools that we put together. The developed tools are later consolidated using two sample applications, the friend-finder and child-tracker services. Developer guidelines are also provided, to aid in using the provided tools.
