"Changing ourselves, changing others" : an analysis of the life stories of participants in a training course for volunteers within a non-governmental organisation in the Eastern Cape Province of South Africa

- Description: Gender-based violence has been recognized as a pressing mental health problem that is prevalent within South African society. Non-governmental organizations play a major role in addressing and highlighting the issue. These organizations make use of volunteers in order to assist in meeting their goals. The modernist perspective has been the dominant investigative mode when research into volunteers has been conducted. However, this study has been conducted with an emphasis on narrative. In its use of this constitutionalist and deconstructive perspective, it examines the identity of the research participants within the dominant social and cultural discourses that story their lives. This presents a major challenge to the modernist framework. In examining the life stories of the participants an emergent nature of identity is noted. Through the process of storying their lives and ascribing meaning to their experiences and understandings, the participants engaged in a process of constructing their identity. This research recognizes that identity is both multi-sited and multi-storied. The emphasis on personal agency enables the participants to restory their lives in the light of challenging prevailing discourses. It is in this process of challenge that they reauthor their lives and are in a position to change their own lives and the lives of others.

A full sample of 30 rugby players at Rhodes University was compared to 27 non-contact sport controls. A reduced sample of 20 rugby players and 9 control participants provided improved control for education and IQ and was compared. Measures included the WAIS-III Vocabulary and Picture Completion Sub-tests to estimate IQ level, the symptom checklist on a widely used computer-based program (ImPACT), and a paper and pencil self-report 31-Item Post-Concussion Symptom Questionnaire. Independent and Dependent T-Test comparisons were conducted on the full and reduced samples. The symptoms reported by the rugby group appeared to be more pronounced on both the ImPACT Symptom Scale and the 31-Item Post-Concussion Symptom Questionnaire when compared to the control group at both the pre-and post-season stages. It was concluded that the rugby players demonstrated evidence to support the hypothesis of having sustained more previous concussions and reporting more symptoms at the pre-season stage when compared to comtrol participants. No prevalent changes for either the rugby or control groups were seen in dependent comparisons from pre-to post-season.

The initial sample of 48 participants was divided into groups; Rugby (n = 30) and Controls (n = 18), Rugby Forwards (n = 14) and Rugby Backs (n = 16). A reduced sample (N = 31) comprised of Rugby (n = 20) and Controls (n = 11), Rugby Forwards (n = 9) and Rugby Backs (n = 11). Comparative subgroups were equivalent for estimated IQ but not for age and educational level in the full sample; in the reduced sample there was equivalence for all three variables of age, education and estimated IQ. All cognitive test measures were subjected to independent t-test analyses between groups at the pre- and post-season, and dependent t-test analyses for Rugby and Controls at pre- versus post-season. - Description: In 2001, participatory research, incorporating various participatory rural appraisal techniques, was conducted in five, rural, primary schools in the Eastern Cape, South Africa. The participatory research was aimed at identifying and addressing schools' needs in terms of lifeskills education in a changing South Africa and educational environment. This whole process has been written up as a case study and further analysed in an attempt to describe the emerging role of the psychologist, working in participation with educators, in this kind of setting. Going beyond description, the researcher reflects upon the function and nature of the role of the psychologist within the developmental participatory process. In doing so, the article addresses the need for participatory research results to be further analysed in generating theory, particularly within Psychology. The article highlights some of the strengths of qualitative research, and the primacy of the personal in that endeavour.

(The practice is distinguished from other intentional and in/direct self-harmful or self-damaging behaviours that cause bodily harm). Dominant psychiatric, psychological or medical approaches construct self-injuring as deviant, socially unacceptable or abnormal behaviour that is indicative of more or less severe psychopathology, and importantly as a stereotypically female practice. This research is conducted within a post-essentialist framework and views self-injuring, and the injured body, as discursively constituted as well as a cultural and political act. It therefore moves away from pathologising discourses in which those who self-injure typically find themselves and their own accounts of their behaviour invalidated and silenced. Instead, the mental health perspective is viewed as one party among many that may contribute to the conceptualisation of ‘self-injuring’ practices as socially meaningful and thus self-injuring is critically interpreted without reliance on a medical model of ‘normalcy’. As part of attempts to challenge medical models and cultural ideals of normalcy, this research presents a critical discursive analysis of a series of narratives provided by 5 female participants in which they record their own experiences, feelings and thoughts related to their practices of selfinjuring. It makes use of critical discourse analytic methodology to identify certain characteristics of these narratives as representations of larger collective meaning systems. It analyses the ways in which self-injuring is constructed in women’s stories of their self-injuring experiences, focusing particularly on the subject positions available in these discourses, as well as their ideological effects. The analysis focuses particularly on constructions of the body and subject positions as they enable or undermines the self-injuring subject’s agency. Finally, it attempts to determine the limitations of certain accounts of self-injuring, pursuing multiple meanings of self-injuring and illuminating new dimensions of talk on self-injuring and novel ways of conceptualising and understanding the practice.

Despite the high incidence of concussion at school level, traditional paper-and-pencil neuropsychological testing has generally been absent from school concussion management programmes, largely due to time and cost constraints. Now, the recent development of computerised neuropsychological testing is providing the opportunity for including neurocognitive assessment in this process. The Immediate Post-Concussion Assessment and Cognitive Testing (ImPACT) is a valid and reliable instrument of this type and normed on 13 - 18 year old North American high school athletes, as well as adult groups. The current recommendation is that athletes are baselined preseason in order to provide an individualised comparative level against which to monitor recovery and provide return-to-play recommendations. This in itself is quite a cumbersome process, thus the present study set out to ascertain whether baseline testing of all athletes is necessary, or whether the use of US or SA normative data alone would provide an appropriate standard against which to interpret the postinjury scores. From a leading South African rugby playing school, the 1st and 2nd rugby teams, (16 - 18 years) were baselined using ImPACT. Three athletes, who were subsequently referred with concussion during the rugby season, were followed up with serial testing on ImPACT. An analysis of the follow up scores was conducted to chart the athletes' recovery process, in relation to the athletes own baseline scores (using US and SA reliable change indices) and age appropriate US and SA normative ranges. The relative utility of individual baselines scores versus these normative ranges was then critically evaluated. It was concluded that a combination of both baseline and normative data provided optimal management of the athlete, with the methods complementing each other in the interpretation of post-injury results. Overall, the SA normative ranges seemed to provide slightly better management guidelines than the US normative ranges when used with this sample of South African high school athletes.

The aim of this research was to explore, clarify and explain certain diagnostic and psychodynamic anomalies to emerge in the course of treatment, using a conceptual framework derived from select psychoanalytic object relations theorists in the area of narcissistic pathology. The author, discovering that there was no diagnostic or explanatory object relations model adequate to the therapeutic data, formulated his own diagnostic category narcissistic neurosis and an eclectic object relations model in order to explain the anomolous research findings. Narcissistic neurosis was defined as a form of psychopathology in which a primarily neurotic character structure presents with a distinctly narcissistic profile. The narcissistic false self-structure serves the functional purpose of protecting the psyche from a repressed negative self-representation derived from a destructive bipolar self-object introject. The primary etiological factor to emerge was that of a narcissistic mother conditional affection and self-object target child necessitated adaptive whose insensitivity, relationship with the premature self-sufficiency and the defensive emergence of a narcissistic surface self-representation. It was proposed that narcissistic neurosis and narcissistic personality disorder are two discrete forms of pathology differing in terms of severity, psychodynamics, defensive structure, mode of object relating, therapeutic accessibility and prognosis. Assessment criteria were proposed in order to differentiate the two areas of narcissistic pathology and assess suitability for psychotherapeutic treatment. Positive treatment results in this case-study suggest that narcissistic neuroses may receive long-term benefit from short-term psychodynamic therapy.

It was shown that correcting cognitive distortions (e.g. selective abstraction, catastrophising, misattribution) and challenging early maladaptive schemas of abandonment, emotional deprivation and emotional inhibition (Young, 1990) assisted in enhancing coping mechanisms and the belief that the pain episodes would be short-lived and could be controlled. There was considerable improvement for the second participant, although he chose to withdraw from the program prior to its completion. The components of the psycho-education, relaxation and stress management and exercise program were beneficial for him. The third participant failed to accept the treatment formulation, and did not engage collaboratively in the treatment program. The case is presented as a point for examining therapeutic ailures.

- Description: This study was conducted to determine the influence of age and culture on the fear of snakes. Five age groups, consisting of 20 White and 20 Xhosa subjects each, were tested. In addition to a behavioural and a GSR measure of fear, questionnaires were applied to determine the extent of the subjects' personal and vicarious aversive experiences of snakes and attitudes towards snakes. The behavioural measure showed a significant decrease in the fear of snakes between the 9 - 11 year and the 14 - 16 year White groups, while, for the Xhosa subjects, the fear of snakes increased significantly between the age groups 14 - 16 years and 18 - 20 years. The GSR measure showed a consistent level in the fear of snakes for White subjects. For the Xhosa subjects the mean GSR score for the 18 - 20 year .group was considerably higher than the means for the other age groups. The intensity of the fear of snakes for White and Xhosa subjects of the same age was remarkably similar. Xhosa subjects had significantly more negative attitudes towards snakes than white subjects. This finding was explained in terms of Whites having had greater opportunities to obtain factual information about snakes. No significant relationships were found between (a) the measures of the extent of the subjects' aversive experiences of snakes; (b) the degree of negative attitudes towards snakes; and the measures of the fear of snakes. On the basis of these measures, the etiology of the fear of snakes cannot be explained in terms of aversive experiences with snakes per se. The striking similarity of responses to a live snake by subjects from two widely different cultures suggests caution in an over-hasty dismissal of the theory of an innate fear of snakes.

Tests were administered to 27 subjects, both male (n=19) and female (n=8), with a mean age of 14.1 years (range = 13.3-15.3). The results show that White English speaking children outperform Black Xhosa speaking children on Verbal, Performance and Full Scale IQs and all subtests, but that these differences tended to disappear when Full Scale IQ and school grade average were controlled for. No subtest score scatter was present for either group although more specific test items appeared to be more difficult for Xhosa speaking subjects and may have contributed to generally lowered scores. Results suggest that caution must be employed in assigning Xhosa speaking South African children to absolute IQ categories. - Description: Research shows that maternal depression has adverse effects on mother-infant attachment and subsequent infant development (Cogill, Caplan, Alexandra, Robson & Kumar, 1986). The mechanisms through which this comes about are unclear. Murray & Cooper (1997) suggest an impaired pattern of mother-infant communication is responsible. Within this, Murray proposes that maternal speech may be a key factor. This study constitutes a preliminary exploration into the mechanisms through which maternal depression effects mother-infant interaction in South Africa. 147 predominantly Xhosa-speaking mother-infant dyads that took part in a broader epidemiological study on post-partum depression in Khayelitsha (Cooper, Tomlinson, Swartz, Woolgar, Murray & Molteno, 1999) made up the subjects. Maternal depression was assessed according to the Structured Clinical Interview for DSMIV (SCID). Maternal speech recorded from standard, five-minute, face-to -face mother-infant interactions was translated and analysed according to a coding system developed by Murray (Murray, Kempton, Woolgar & Hooper, 1993). The speech of depressed mothers to their two month old infants was compared to the speech of non-depressed mothers on dimensions of focus, affect and agency, and the role of infant gender was assessed. Results revealed no significant group differences for depression. Maternal speech to male infants was found to hold significantly less ascription of agency than to female infants. Findings suggest that maternal speech may be too narrow a marker of maternal depression in this context and that broader indices are needed. - Description: Research shows that maternal depression has adverse effects on mother-infant attachment and subsequent infant development (Cogill, Caplan, Alexandra, Robson & Kumar, 1986). The mechanisms through which this comes about are unclear. Murray & Cooper (1997) suggest an impaired pattern of mother-infant communication is responsible. Within this, Murray proposes that maternal speech may be a key factor. This study constitutes a preliminary exploration into the mechanisms through which maternal depression effects mother-infant interaction in South Africa. 147 predominantly Xhosa-speaking mother-infant dyads that took part in a broader epidemiological study on post-partum depression in Khayelitsha (Cooper, Tomlinson, Swartz, Woolgar, Murray & Molteno, 1999) made up the subjects. Maternal depression was assessed according to the Structured Clinical Interview for DSMIV (SCID). Maternal speech recorded from standard, five-minute, face-to -face mother-infant interactions was translated and analysed according to a coding system developed by Murray (Murray, Kempton, Woolgar & Hooper, 1993). The speech of depressed mothers to their two month old infants was compared to the speech of non-depressed mothers on dimensions of focus, affect and agency, and the role of infant gender was assessed. Results revealed no significant group differences for depression. Maternal speech to male infants was found to hold significantly less ascription of agency than to female infants. Findings suggest that maternal speech may be too narrow a marker of maternal depression in this context and that broader indices are needed. - Description: his study builds on South African cross-cultural research which demonstrated the importance of careful stratification of multicultural/multilingual normative samples for quality of education in respect of English and African language (predominantly Xhosa) speaking adults and children tested with the WAIS-III and WISC-IV, respectively. The aim of the present study was to produce an expanded set of preliminary comparative norms on the WISC-IV for white and coloured Afrikaans, white English and black Xhosa speaking Grade 7 children, aged 12 to 13 years, stratified for advantaged versus disadvantaged education. The results of this study replicate the findings of the prior South African cross-cultural studies in respect of quality of education, as groups with advantaged private/former Model C schooling outperformed those with disadvantaged former DET or HOR township schooling. Furthermore, a downward continuum of WISC-IV IQ test performance emerged as follows: 1) white English advantaged (high average), 2) white Afrikaans advantaged and black Xhosa advantaged (average), 3) coloured Afrikaans advantaged (below average), 4) black Xhosa disadvantaged (borderline), and 5) coloured Afrikaans disadvantaged (extremely low). The present study has demonstrated that while language and ethnic variables reveal subtle effects on IQ test performance, quality of education has the most significant effect – impacting significantly on verbal performance with this effect replicated in respect of the FSIQ. Therefore caution should be exercised in interpreting test results of individuals from different language/ethnic groups, and in particular those with disadvantaged schooling, as preliminary data suggest that these individuals achieve scores which are 20 – 35 points lower than the UK standardisation.

- Description: The purpose of this study is to investigate the value of using a Daseinsanalytic approach to dream explication in order to reveal the essential meaningfulness of behaviourial possibilities for the patient. After discussing the Daseinsanalytic view on dream explication, emphasizing Medard Boss's approach as the major exponent of this view, the study explores the usefulness and validity of the study as a method of investigating the dreams from this perspective. The dreams related in this study are examined individually, giving a phenomenological understanding of each and also providing a possible prospective focus for therapy. It will be shown that the phenomenological understanding of the patient's dreams corresponds with his subjective experience of his emotional existential situation and that any positive changes in the content of the dreams over time corresponds with observable and related changes in his behaviour, ideation and mood. However, if the suggested prospective focus of Medard Boss had been more usefully employed when each dream was explicated in the therapy sessions, it is felt that the progress of therapy would have been greatly enhanced. This study therefore concludes that the use of Medard Boss's approach to dream explication has not only a hermeneutic value in the process of therapy, but also aids the dreamer in becoming aware of the possible modes of behaviour yet unrevealed in his waking life.
