Ocean plastics are transported from land by rivers to the sea. However, source contextualization is complex. Many African rivers predominantly run alongside human settlements that host informal waste dumpsites. In this study a simple cost effective, easily deployed, consistent and replicable survey methodology was employed. The study quantified macro plastic in three rivers discharging into Algoa Bay, South Africa. The results indicated that industrial Swartkops and metropolitan Baakens Rivers both illustrate moderate plastic pollution (>3000 plastic particles/day), with the relatively natural Sundays River to showing minimal evidence of river macroplastic (<100 plastic particles/day). The countries that are economically developed have stood out because of human capabilities that have triggered economic development. With innovation being one of the major advantages of human capital, new ideas lead to the creation of vast employment opportunities especially for those that have studied critical skill subjects. The economic status of Zimbabwe shows that there is human capital development happening in the country, but there is a serious shortage of skills in sciences, technology, engineering and mathematics that are necessary for the industrialisation of the country. The government believes that Zimbabwe can industrialise if STEM education is integrated in the country’s educational system to make human capital development wholistic. The study aims to understand the concept of human capital development and how it can be developed at a balance. A discussion on the primary role of universities and higher learning institutions on developing human capital will be done. Universities educate and equip students with knowledge and skills, hence both STEM and non-STEM curricula is needed to develop human capital that is balanced. This is because STEM graduates that are innovative can create jobs for non-STEM graduates and help a country to industrialise and develop its economy. Universities are recommended to increase public awareness of STEM, train teachers specifically for STEM, invest in research and development and develop STEM-related policies and systems to develop more STEM graduates to strike a balance with non-STEM graduates and enhance industrialisation in Zimbabwe. Interviews were conducted and analysed and some of the recommendations made were to encourage the government to deregulate to attract investment, to increase the public’s awareness of STEM and to train STEM teachers accordingly.

Trends indicate that only iota of evidence implies that the concept of risk management is fully integrated and embraced as part of a culture for ensuring the successful project implementation by the Modern South African public sector organizations. Empirical research revealed that the common types of risks that have been affecting the successful implementation of government projects and programmes include: riots, fraud, corruption, changes in demographic figures, outsourcing risks, and floods and changes in weather. Most of the participants stated that risks associated with riots do not only arise from the riots within the government departments, but also in the external environment. In terms of the internal riots, they revealed that when the employees go on strike or riots on their own or in solidarity with the other trade unions, leading to the process for the implementation of government projects and programmes being affected. Yet, in certain cases, they noted that internal riots or strikes most occur as preceded by rumours that the riots by a particular union will take place and may significantly affect the ongoing process of implementation of government programmes. Besides risks associated with the internal riots by the employees and the communities, the interview findings also indicated that the other forms of risks arise from the strikes and riots by the private enterprises.

From those tested models elsewhere, it was evident that South Africa has a great Constitution and good policies regarding Local Government Human Resource Department recruitment and appointment processes not withstanding some challenges cited in the study. This assessment of challenges in the public appointments and recruitment processes in Chris Hani District Municipality assisted in the establishment of the real facts about effective and fair recruitment and appointment processes in the Local Government Human Resource Management Department to deserving communities. It became clear from the study that providing efficient and fair recruitment and appointments processes to the Local Government Human Resource Department in South Africa requires a broader advocacy agenda encompassing the development of Human Resource systems and the generation of numerous skills and expertise to deliver services to the poor people of our country.

- Description: For most rare marine species, abundance trends are poorly estimated. This uncertainty often translates into disagreement on management regulations, impeding conservation efforts when they are most urgently required. The world’s largest sea bream, the red steenbras Petrus rupestris, has undergone a substantial and widely acknowledged decline during the 20th century. Standardised probability of encounter over the last 3 decades was used to track red steenbras abundance trajectories in 2 regions, which together represent the species’ distribution. The results predicted a reduction in abundance of 44% in the East region and 96% in the South-West region, from what was already considered to be a collapsed population at the start of the time series. The species is now largely confined to areas in the centre of its historical abundance. Content analysis of a recreational angling magazine revealed a concurrent 50% decrease in reported size between 1984 and 2012. Shore-based observer data (2009-2010) indicate that the impact of recreational fishers on this species far exceeded that of commercial fishers at the time. Regulations in the form of progressively more stringent catch and effort restrictions appear to have been largely ineffective, too lenient, implemented too late or were unsuitable to control fishing mortality for this species. This study highlights 2 fundamental challenges in management of rare marine fish species: (1) providing decision makers with a credible measure of abundance for species whose rarity limits data availability and (2) implementing effective policy changes before the specific measures become ineffective and obsolete.

- Description: The Fourth Industrial Revolution (4IR) is set to disrupt existing economic and social structures through the use of cyber-physical systems that result from a fusion of the digital, biological, and physical spheres. The fifth and current long wave of innovation is going through such a digital revolution in the ongoing deployment period which is being driven by the generalpurpose technologies of Artificial Intelligence and the Internet of Things, among other cyberphysical systems. The impact of mobile money in the access of financial services has shown how disruptive incremental innovations in mobile and digital technologies can be. The transformational power of mobile money in financial access is due to its use as an accessible financial tool that utilizes mobile devices to send and/or receive money over great distances. With the 4IR looming, this thesis determines the factors that enable and constrain the adoption of a disruptive technology amongst Sub-Saharan African small, micro, and medium enterprises (SMMEs). Therefore, due to its impact on financial inclusion and the formalization of SMMEs, mobile money is used as an indicator for the adoption of 4IR disruptive digital technologies. The adoption of mobile money was evaluated using secondary data from a survey conducted by Research ICT Africa, which surveyed 4408 SMMEs in nine African countries. The Diffusion of Innovations (DOI) model and the Unified Theory of Acceptance and Use of Technology (UTAUT) model were used to identify the factors enabling and constraining the adoption of a disruptive technology, in this case mobile money. Factors included gender, vocational training, business skills training, tertiary education, services, performance expectancy, social media, location, and nine African countries (Kenya, Mozambique, Ghana, Nigeria, Rwanda, South Africa, Tanzania, Uganda, and Senegal). The factors were grouped into owner characteristics, firm attributes and country attributes. SMME owners with business skills (49%) showed the highest level of adoption in terms of owner characteristics, Kenyan SMMEs (21%) had the highest adoption between the countries surveyed, and social media (62%) showed the highest adoption in terms of firm attributes followed by the formal variable (47%). In general, only 29% of SMMEs surveyed adopted mobile money. The study found that women SMME owners were more likely to be affected by business formality when adopting a disruptive technology compared to male owned SMMEs. This is because informality often exacerbates other barriers/challenges women face such as lower access to finance, lower ability to exercise property, business, and labour rights, and lower visibility. The results also demonstrate that vocational training is more important than general tertiary education for the ii adoption of a disruptive technology such as mobile money. Furthermore, when using social media as a tool for business advice SMME owners were more likely to adopt the disruptive technology. The study suggests that to encourage African SMMEs to adequately adopt disruptive technologies of the 4IR, more women owned SMMEs need to enter the formal economy, and vocational training targeted at business skills must be promoted amongst all SMME owners. Eastern African SMMEs were found to be more likely to adopt mobile money compared to other African regions. - Description: Variations in the yield, chemical composition, antibacterial, and antioxidant properties of the essential oils of Rosmarinus officinalis L. cultivated in Alice, Eastern Cape of South Africa over a period of 12 months using the solvent-free microwave extraction and traditional hydrodistillation methods were evaluated. The GC-MS analyses of the essential oils revealed the presence of 33 compounds with 1,8-cineole, a-pinene, camphor, verbenone, bornyl acetate and camphene constituting about 80 percent of the oils throughout the period of investigation, with the solvent-free microwave extraction method generally yielding more of the major components than the hydrodistillation method. Each of the major components of the oils varied in quantity and quality of yield at different periods of the year. The method of extraction and time of harvest are of importance to the quantity and quality of essential oil of Rosmarinus officinalis. Higher amounts of oxygenated monoterpenes such as borneol, camphor, terpene- 4-ol, linalool, a-terpeneol were present in the oil of SFME in comparison with HD. However, HD oil contained more monoterpene hydrocarbons such as a-pinene, camphene, β-pinene, myrcene, a-phellanderene, 1,8-cineole, trans- β-ocimene, γ-teprinene, and cis-sabinene hydrate than SFME extracted oil. Accumulation of monoterpene alcohols and ketones was observed during maturation process of Rosmarinus leaves. Quantitative evaluation of antibacterial activity, minimum inhibitory concentration values were determined using a serial microplate dilution method. The essential oils obtained using both methods of extraction were active against all the bacteria tested at a concentration of 10 mg mL-1. The minimum inhibitory concentrations for the SFME extracted oils ranged between 0.23 and 1.88 mg mL-1, while those of the HD extracted oils varied between 0.94 and 7.5 mg mL-1, thus suggesting that the oil obtained by solvent free microwave extraction was more active against bacteria than the oil obtained through hydrodistillation. The antioxidant and free radical scavenging activity of the obtained oils were tested by means of 1,1-diphenyl-2-picrylhydrazyl radical (DPPH+) assay and β- carotene bleaching test. In the DPPH+ assay, while the free radical scavenging activity of the oil obtained by SFME method showed percentage inhibitions of between 48.8 percent and 67 percent, the HD derived oil showed inhibitions of between 52.2 percent and 65.30 percent at concentrations of 0.33, 0.50 and 1.0 mg mL-1, respectively. In the β-carotene bleaching assay, the percentage inhibition increased with increasing concentration of both oils with a higher antioxidant activity of the oil obtained through the SFME than the HD method. Thin layer chromatography (TLC) was used to analyze the chemical composition of the extracts using three eluent solvent systems of varying polarities i. e. CEF, BEA and EMW and sprayed with vanillin-sulfuric acid. The chemical composition of the different extracts was similar with the exception of methanol and water extracts which had only one or two visible compounds after treating with vanillin-spray reagent. To evaluate the number of antibacterial compounds present in the fractions, bioautography was used against two most important nosocomial microorganisms. S. aureus (Gram positive) and E. coli (Gram negative). Nearly all the crude serial extraction fractions contained compounds that inhibited the growth of E. coli. The hexane extract had the most lines of inhibition followed by ethyl acetate. Bioassay-guided fractionation against E. coli was used to isolate antibacterial compounds. The largest number of antibacterial compounds occurred in the hexane fraction. Furthermore we tried to complete the characterization by extracting and studying other biologically important plant metabolites such as phenolic compounds to evaluate the antioxidant capacity of Rosmarinus extracts.

- Description: In this research the experience of pet loss in relation to the intensity of grief and the repression of that grief due to societal restrictions was studied in a South African sample of 12 pet owners who had experienced the loss of a pet within 24 months prior to the study. A pilot study was conducted in order to refine the semi-structured interview schedule. Participants were asked about their pets, their experience of pet loss in relation to their feelings before and after the loss and whether they felt they could openly grieve around friends and family. The findings indicated that the intensity of grief experienced by the participants was influenced both by the intensity of their Bond with the pet and the repression of their grief due to assumed social sanctions surrounding mourning the loss of a pet. The findings from the study clearly indicate the necessity for pet loss counselling, however it was clear that societal views surrounding pet loss are shifting, thus allowing pet owners to mourn their loss more openly with family and friends. This indicates a decrease in cases of disenfranchised and complicated grief. Furthermore the impact of pet loss necessitates the social legitimation and acknowledgment of the significance of pet loss in contemporary society as these relationships with pets are an increasingly prominent feature both within and as an alternative to familial systems.

- Description: This study deals with the problems of external examining, the inflexible demands of which dominate and dictate to literature teaching in South African schools today. The aims of teaching literature are discussed, and it is suggested that the negative attitudes among pupils resulting from the present examining system defeat many of these aims. The opportunities for the enjoyment of literature are minimised by the process of preparation for external examinations. Creative teaching methods are abandoned in favour of coaching for specific types of questions, which are determined and limited by the practical constraints of a mass external examination. In catering to the demands for admininstrative reliability and efficiency, the educational validity and efficiency of the examinations are sacrificed. In Britain the Newbolt and Bullock Reports, among others, have made forceful recommendations for alternative approaches to external examining. Subsequently, much experimentation with internal examining, course-work and open-book examining has followed, aspects of which are discussed in this study. There has been limited experimentation in these areas in South Africa. The TED conducted a successful internal examining experiment in English literature, the results of which are considered in this thesis. The national English Olympiad open-book examination is a further example of the success of an alternative approach. By contrast, a comparison of examination papers set by the JMB and CED over the last ten years, shows clearly that the stated syllabus aims of teaching literature and the aims of examining the subject were wholly incompatible. Recommendations are made for the adoption of alternative examining strategies in order to address the shortcomings identified

- Description: The South African wine industry is one of the oldest sectors of the country’s economy, beginning in the early years of South African colonialism in the 17th century through the use of slave and slave-like labour. As an industry that has been defined by farm paternalism and dependency, the South African wine industry has had to demonstrate changes from its history of extreme racial and gendered exploitation to an industry that reflects the democratic values of a new South Africa. Ethical certifications are considered one way through which the post-apartheid South African wine industry can demonstrate that it has moved away from its historical practices and is now characterised by practices of decent work and social transformation. An important certificatory label that allows local wine producers entry into global wine production networks is Fairtrade International. While Fairtrade certification is often reserved for small-scale producers, this certification has been extended to large-scale, commercial producers within the South African wine industry despite its history of farm paternalism and dependency. This research asks if this inclusion promotes decent work through social upgrading or if it offers a platform for the continuation of farm dependency under the guise of ‘Fairtrade’. In this study, social upgrading has been defined through a ‘bottom-up’ approach that prioritises workers’ independence as a key means of improvement. Four key pillars are applied as embodying the concept of social upgrading: regular employment with set working hours; legally enforceable worker rights; social protection through collective and individual bargaining power; and non-discriminatory social dialogue that promotes significant socio-economic progression. A key finding of this research is that practices of dependency and paternalism continue on some large-scale commercial farms, despite their Fairtrade certification. Furthermore, the findings suggest that the benefits that farmworkers receive lock farmworkers into their position on the farm with marginally improved conditions and cease if workers depart from the Fairtrade wine farm. This suggests that, at best, this inclusion of commercial farmers within Fairtrade certification appears to create a top-down form of social upgrading that locks farmworkers into their position on the farm with marginally improved conditions and beneath a glass ceiling of development. At worst, this Fairtrade inclusion facilitates a global poverty network through paternalism and dependency under the guise of ‘Fairtrade’. The use of Police force in crowd management

- Description: South Africa has a violent and oppressive past. They are various historical incidents1 of extreme cruelty perpetrated by the previous apartheid regime. Much of the modern South African democratic state was forged by protests. During the 1970s and 80s, the legislator by passing unjust laws was used to assist the government to maintain the oppression of the people of South Africa. From the Soweto uprising in the 1970s to the current service delivery protests of the 21st century, gatherings have always had the potential for deadly violence. The motivation for this research started with the emotions evoked by the iconic picture of the body of Hector Pietersen2 being carried after being shot by the police. Strikingly the images of the killing by the police of Andries Tatane conjured further questions concerning the use of deadly force within crowd management situations. The research undertook an analysis of the use of force by the police during crowd management situations. A brief analysis of South African law relating to the use of force by the police prior to 1996 is provided. There are legislative prescripts for the use of force during the maintenance of public order. It must be noted that the legislation falls short on providing clear, concise authority for the use of deadly force. Normally, the use of force by the police and civilians for the purpose of arrest is regulated by the Criminal Procedure Act3 , whereas the Regulation of Gatherings Act4 providing the authority for the use of force by the police in crowd management situations to preserve public order. At first glance, section 49 of the CPA seems to validate arguments that it violates some constitutionally protected rights, among which are the right to dignity, life, to freedom and security of the person, against cruel, inhuman or degrading treatment or punishment and to a fair trial, which includes the right to be presumed innocent. Section 49 however, withstood Constitutional muster as set out in Re: S v Walters & another. As the right to life is a non derogable right.5 The limitation of this right may lead to constitutional scrutiny. The emphasis will thus be on ensuring that the balance with regards to proportionality in the use of deadly force is maintained. During the research it became apparent that the police, especially during crowd management situations, served political interests.6 This had the unintended consequence that the laws were applied to suit the political narrative and not the rule of law. The use of force in the policing arena is controversial. It is very clear that any misuse of force in crowd management situations will evoke the historical wounds associated with apartheid. However, within crowd management, the use of force and the authority to use deadly force is absolutely necessary. The Marikana massacre was used to highlight the mistakes that police have made during inappropriate use of force and its catastrophic consequences.7 It was observed that the legislative framework concerning the use of force, whether under section 49 of the CPA or section 9 of the RGA, is incoherent and too complex. The research argues for simplicity and accuracy within policy and applicable legislative alignment. The linkages from the applicable legislation to the institutional policies should never be outdated or incorrectly formulated. The violent rhetoric from politicians such as ex-president Jacob Zuma, 8 Minister Fikile Mbalula 9 and Bheki Cele10 fuels the argument that the police are susceptible to misdirected notions and may cause the police act unlawfully. The Constitution requires the police to “enforce the law”11 and as such there is an obligation on the police to do this within the constitutional parameters. The Marikana massacre was used to highlight the mistakes that police have made during inappropriate use of force and its catastrophic consequences.7 It was observed that the legislative framework concerning the use of force, whether under section 49 of the CPA or section 9 of the RGA, is incoherent and too complex. The research argues for simplicity and accuracy within policy and applicable legislative alignment. The linkages from the applicable legislation to the institutional policies should never be outdated or incorrectly formulated. The violent rhetoric from politicians such as ex-president Jacob Zuma, 8 Minister Fikile Mbalula 9 and Bheki Cele10 fuels the argument that the police are susceptible to misdirected notions and may cause the police act unlawfully. The Constitution requires the police to “enforce the law”11 and as such there is an obligation on the police to do this within the constitutional parameters. The study confirms the adaptation claim that as cognitive sense-makers, teachers mutate and enact a curriculum according to their personal subjective interpretations in the context of unique use-setting implementation realities. Although one of the participants’ understanding and practice displayed considerable comprehension of the requirements of the curriculum, the other teachers displayed an understanding of this curriculum in a superficial way, and experienced few positive experiences and several conceptual and operational constraints in its implementation. Drawing on their practical experiences with the implementation of the curriculum, teachers offered suggestions for transforming the implementation proficiency of this curriculum, which formed part of the conceptual strategy I developed for improving practice. Thus the resultant achievement of the study was a conceptual strategy that was constructed from the key findings of the study to provide educational change leaders with nuanced ideas and insights for improving practice.

However, this medium has not been fully exploited by these banks as there are many hurdles the banks must triumph over. In deploying this technology and these systems, there are several factors which banks must take into consideration before fully deploying such a system to their customers, hence the motivation for this study. The absence of suitable and sufficient knowledge on this topic also exposes a “rhetoric versus reality” argument of whether the intention to adopt Internet banking is critical to the strategies and ultimate success of banks in Ghana. For banks to stay ahead of competition as well as to attract and maintain their clientele, it is of paramount importance to gather and link the perspectives of both clients and bank managers in order for banks to ensure that they perform according to the needs and expectations of their clients. In order to achieve the intended results, an empirical study was conducted by taking into consideration the viewpoints of both bank clients and bank managers in determining the factors that customers take into consideration before adopting the Internet banking medium. The primary aim of this study was to quantify significant relationships between the selected variables. Therefore the positivism research paradigm was used, while the phenomenological paradigm was employed for the measuring instruments. Because multiple sources of data were used, from the perspectives of banking clients and managers in Ghana, methodological triangulation was adopted for this study. The results of the empirical investigation showed that both groups (clients and managers) considered the variables of market share, technology acceptance, diffusion of innovation, organisational variables, organisational efficiency, and business strategy to have direct influence on the adoption of Internet banking. However, they differed in opinion concerning the degree of influence of these variables. The bank managers’ responses leaned more towards strong agreement with the importance of these variables than did those of the bank clients. Thus, for bank clients to readily adopt the Internet banking medium for their banking transactions, bank managers must take a closer look at these determinant factors described in the study. The study showed that the population group, educational and income levels exerted an influence on the perceptions clients have regarding Internet banking adoption factors. It was found that the higher the education and income levels of the clients, the easier it was for them to adopt Internet banking. Also, the male group dominated the use of the Internet banking. This is supported by the fact that there is a growing middle class in Ghana that falls within this category of banking clients.

The gain-sharing incentive has been adopted by many organisations today to enhance staff loyalty and productivity. A gain-sharing programme ensures that both the organisation and team members benefit from the expected performances, rather than an individual or group incentive which is given based on a specific finite goal which could be achieved at the cost of other key critical business need. A couple of years ago Autocast Aluminium decided to enhance employee performance by initiating the gain-sharing incentive programme. After the programme was rolled out, the organisation is yet to examine the extent to which it enhances job satisfaction in the organisation. The primary objective or aim of the study is to examine the extent to which gain-sharing at Autocast meets the purpose for which it was created. The sample of the study was 176 of the employees at Autocast Aluminium; however 130 questionnaires were distributed. A total of 88 questionnaires were returned, of which 80 were usable, whilst eight were scrapped due to more half of the questionnaire having missing data. This resulted in a 68 % response rate. A framework has been developed to measure to what extent does the programme enable employees to feel satisfied with their jobs. The empirical results revealed that out of six independent variables tested: fair payment, involvement, personal growth, communication, job security and leadership style, only three that plays significant role on employee job satisfaction. These are fair payment, communication and leadership style.

This study aims to establish whether accounting for structural changes in the conditional variance process, through the use of Markov-switching models, improves estimates and forecasts of stock return volatility over those of the more conventional single-state (G)ARCH models, within and across selected African markets for the period 2002-2012. In the univariate portion of the study, the performances of various Markov-switching models are tested against a single-state benchmark model through the use of in-sample goodness-of-fit and predictive ability measures. In the multivariate context, the single-state and Markov-switching models are comparatively assessed according to their usefulness in constructing optimal stock portfolios. It is found that, even after accounting for structural breaks in the conditional variance process, conventional GARCH effects remain important to capturing the heteroscedasticity evident in the data. However, those univariate models which include a GARCH term are shown to perform comparatively poorly when used for forecasting purposes. Additionally, in the multivariate study, the use of Markov-switching variance-covariance estimates improves risk-adjusted portfolio returns when compared to portfolios that are constructed using the more conventional single-state models. While there is evidence that the use of some Markov-switching models can result in better forecasts and higher risk-adjusted returns than those models which include GARCH effects, the inability of the simpler Markov-switching models to fully capture the heteroscedasticity in the data remains problematic.

The complexity of the shoulder’s anatomy has led to the development of multiple radiographic projections and techniques within plain film imaging with each projection intended to demonstrate specific aspects of the anatomy of the shoulder. However, reproducing the required projections can be difficult especially if radiographers are not familiar with the projections and their evaluation criteria. Literature has revealed the importance of a comprehensive knowledge and understanding of anatomy, patient positioning, beam direction and centring point, and evaluation criteria to ensure a quality projection for accurate diagnoses. The aim of the study was to determine the knowledge and understanding of radiographers with regard to the supraspinatus outlet projection (SOP) for shoulder impingement syndrome (SIS) and its evaluation criteria. The inferences derived from the research findings were used to develop guidelines for a structured in-service training programme for practising radiographers to optimise their knowledge and understanding of the supraspinatus outlet projection in shoulder impingement syndrome. The proposed study followed a quantitative approach. Furthermore, a descriptive, exploratory, contextual design was employed. The research population consisted of practising radiographers working in the public and private hospitals of the Nelson Mandela Bay Municipality. The data were collected by means of a structured self-administered questionnaire. The questionnaire comprised of three sections. The first section requested demographic information from the participants. The second section assessed their knowledge and understanding regarding the scapular ‘Y’ and the supraspinatus outlet projections and shoulder impingement syndrome. The third section assessed their knowledge and understanding of anatomy and image evaluation/critiquing. The reliability and validity of the data collection instrument was ensured by conducting a pilot study and comparing the results with those of the main study. In addition, the expertise and guidance of a radiographer experienced in the clinical training of radiographers, the supervisor (who has twenty years’ experience in the teaching of radiographers) and a statistician was obtained. Descriptive and inferential statistical analyses were performed by means of a statistical programme and with the guidance of a statistician. The researcher ensured that the study was conducted in an ethical manner by adhering to the ethical principles of beneficence, justice and respect for persons.

The aim of the study was to determine the knowledge and understanding of radiographers with regard to the supraspinatus outlet projection (SOP) for shoulder impingement syndrome (SIS) and its evaluation criteria. The inferences derived from the research findings were used to develop guidelines for a structured in-service training programme for practising radiographers to optimise their knowledge and understanding of the supraspinatus outlet projection in shoulder impingement syndrome. The proposed study followed a quantitative approach. Furthermore, a descriptive, exploratory, contextual design was employed. The research population consisted of practising radiographers working in the public and private hospitals of the Nelson Mandela Bay Municipality. The data were collected by means of a structured self-administered questionnaire. The questionnaire comprised of three sections. The first section requested demographic information from the participants. The second section assessed their knowledge and understanding regarding the scapular ‘Y’ and the supraspinatus outlet projections and shoulder impingement syndrome. The third section assessed their knowledge and understanding of anatomy and image evaluation/critiquing. The reliability and validity of the data collection instrument was ensured by conducting a pilot study and comparing the results with those of the main study. In addition, the expertise and guidance of a radiographer experienced in the clinical training of radiographers, the supervisor (who has twenty years’ experience in the teaching of radiographers) and a statistician was obtained. Descriptive and inferential statistical analyses were performed by means of a statistical programme and with the guidance of a statistician. The researcher ensured that the study was conducted in an ethical manner by adhering to the ethical principles of beneficence, justice and respect for persons.

Psychiatric problems in the primary health care context: a study in the Border-Kei area

- Description: A clinic survey was undertaken to investigate the nature of psychiatric problems experienced by the primary health care (PHC) patient population in the Bisho-King William's Town area of the Eastern Cape Region. The study took as its point of departure research findings which attest to the high rate of psychiatric distress amongst this population group in different parts of the world and ohservations regarding the form of presentation in terms of physical complaints. Hypotheses posited relationships between psychiatric problems experienced by patients attending PHC clinics in the study area and four types of variables, namely; somatic complaints, socio-demographic characteristics, patterns of health service utilisation and patient satisfaction with health services. Using a quasi-experimental descriptive approach, a two-stage screening procedure sorted the patient sample into three groups on the basis of the degree of psychiatric symptomatology experienced. The triangulation of the results of between-groups analyses with case materials recorded during psychiatric interviewing provided for an ethnographic account of the cultural experience of distress in the study area. The screening process used standard instruments, the Self-Reporting Questionnaire (SRQ) in the first stage and the Present State Examination (PSE) in the second stage. A pilot study was conducted prior to the fieldwork for the main study. Using the SRQ, thirteen psychiatric paticnts and 31 general PHC patients were sampled for the pilot study and 148 PHC patients were sampled for the main study. Using the PSE, 11 and 57 PSE interviews were conducted in the pilot and main studies respectively. Between-groups analyses used chi-square and F-statistics to investigate possible associations with identified patient correlates (P<0.5). These were socio-demographic, utilisation and satisfaction variables, measured by a separate face-valid self-response instrument compiled for the purposes of this study. Psychiatric symptomatology was found to be statistically significantly related to age, marital status and educational level. Further, patients experiencing more psychiatric symptomatology reported significantly more illnesses requiring treatment, longer consultation periods and a greater number of sick bed days. No statistically significant relationships were found between psychiatric symptomatology and number of children, number of failures at school, amount of treatment utilised, number of consultations, or patient satisfaction with services. Descriptive analyses of symptom and syndrome profiles found certain somatic complaints to be particularly prevalent amongst the patient sample. These include headaches and various tension pains, decreased energy levels and digestive problems. Qualitative analysis of interview data found that many somatic and psychiatric problems experienced constitute culturally defined and meaningful experiences, especially 'umbilini' (or nerves), 'ufufunyana' (a possession state), and accusations of witchcraft. Interpretation of complaints from the local traditional healing perspective, revealed a more complex mode of communication between patients and the health delivery system than may be accounted for in terms of a simple biomedical model. The interpretive analysis in the study showed that some forms of presentation incorporating somatic symptoms, such as 'nerves' may he viewed as help seeking behaviour of the socially unempowered. Implications of the results are discussed in relation to the need for improved identification and management of psychiatric distress at PHC level facilitated by a better developed referral network and closer interaction between biomedical and anthropological perspectives.
