- Description: In 2009, South African police statistics revealed that more than 68 332 women were raped in the country. The evidence from independent researchers has shown that SAPS statistics are highly susceptible to under-reporting and the actual figure is more than double this amount. One pervasive feature of the phenomenology of rape is post-traumatic stress disorder (PTSD). PTSD is a highly debilitating condition with severe individual and societal costs. The condition takes a critical toll on occupational functioning, schooling and personal relationships and is associated with depression, suicide risk, self-harming behaviours and alcohol-abuse problems. The Ehlers and Clark (2000) cognitive model represents the most efficacious treatment for PTSD but the approach is severely under-utilised by South African practitioners working with sexual trauma. The reasons for such under-utilisation relate to a lack of exposure and training surrounding the model and concerns about the transportability of the treatment to a multi-cultural context. One method of addressing these barriers to treatment delivery is through systematic case-based research. Systematic case-based research offers a complementary means of refining theory and developing evidence-based practice in the context of a developing country. The method offers an intensive analysis and description of the particular phenomena under study within its real-life context. It allows the researcher to intensively examine and identify the specific aspects of the therapist’s responses and client’s reactions that contributed to significant change. Unlike efficacy studies, generalisability in case-study research is based on replication on a case-by-case basis and the creation of case law. This research study uses a systematic-case study approach to investigate the applicability of the Ehlers and Clark (2000) model in the treatment of rape-related PTSD in South Africa. The study aims to demonstrate the transportability of the model and develop a needed evidence base for service providers in the country. Seven women participated in the project and lent their treatment process to the research. The participants varied in terms of age, race, culture, socio-economic status and the nature of their sexual trauma. Through synoptic thematic analysis of their therapy process specific client-related personal aspects, client-related contextual factors and state-level factors were found to impede treatment delivery and implementation. The implications of these aspects for clinical practice and social policy are comprehensively discussed.

- Description: This study explores the history of sex communication and education over a period of fifty years (1950 to the present), in a deep rural area of the Eastern Cape. It describes patterns of sex communication between peers, between siblings, between children and parents and between young people and other non-familial agencies within communities. Communication trends are traced from the period before contraception was introduced, through to the introduction of female birth control methods into the HIV/AIDS era where the focus has been on attempting to introduce condoms. Twelve semi-structured interviews and two four-person focus groups were conducted to gather information on how the participants acquired information about sexuality and their responses to the same. The findings of the present study suggest that the widespread use of injectable contraceptives has had a marked effect on the sexual culture of the community under investigation. It has led to the collapse of the regulatory practices which were previously in place. This in turn has significantly affected the sexual communication and negotiation context. Furthermore, it has had a determining influence on male involvement in sexual reproductive health matters and has created a poor context for the adoption of condoms as a prophylactic. There were no major changes in the sexual communication context within families and within communities in that education has always been limited to instructions to avoid pregnancy. Of note was a culture of collusion between adults and children surrounding sexuality, which absolved the parties involved in addressing sexuality. These factors are understood have mediated response to HIV/AIDS prevention efforts, and need to be taken into consideration in the development of sex communication and education programmes.

A social justice approach for counselling psychology in South Africa: lessons from a collaborative action research intervention

The aim of this study is to contribute to the debates about the social responsiveness, scope and focus of the field. The following research question was formulated: What lessons about a social justice approach can a counselling psychologist learn from a youth unemployment community engagement activity? Through engagement with a group of unemployed young people about their status and exploring the role that a counselling psychologist might take, a collaborative action research approach was employed. Data were generated through collaborative action research cycles, semi-structured interviews, social media communications and the researcher’s reflective journal. Template analysis was used to make sense of the data and generate preliminary themes. Process findings illustrated the supportive and constraining role of the academic research process, the time frame and the characteristics of the target group. The content-related findings illustrated the impact of contextual and demographic factors on the participants’ unemployed status. The findings confirmed the negative effect of unemployment on psychological well-being despite support from family, friends and community members. Factors such as education and socioeconomic status influenced the job seeking behaviour of participants and although participants were aware that societal change is needed, there was evidence of internalised self-blame. Recommendations emphasise the inclusion of relevant social justice models for the context of South Africa in the training of counselling psychologists; and a pressing need to engage in micro-, meso- and macro-level advocacy to collaboratively debate the identity and scope of practice of the profession and the review of professional and ethical guidelines.

The literature review showed repeated reference to the "alienation" of the "Coloured" people of South Africa in the political, social and cultural spheres. However, the research in this area has been sparse. The main object of the study, therefore, was to determine if "Coloureds" do feel alienated, as well as the contexts of this alienation, using Whites as a comparative group. possible differences in personality traits between "Coloureds" and Whites have been associated with the differences in behaviour exhibited by these two groups, but past research has failed to find many differences. The second objective of this study, therefore, was to determine if there are differences between the personality traits of "Coloureds" and Whites, and to extend the past research. Researchers have shown that there is a relationship between alienation and personality traits, but have not adequately addressed the nature of this relationship or its implications. Following from this, the third objective of the study was to examine the relationship between alienation and personality traits, as well as their ability to predict alienation. Results on the Alienation Test showed that "Coloureds" do differ significantly from Whites and exhibit far higher levels of alienation. These differences are most marked on those types and contexts of alienation reflecting a distance from the social and political environment. Results on the personality measure showed minimal differences in personality traits between the groups. Where they were evident, they could be attributed to environmental factors. Lastly, there was a relationship between alienation and certain personality traits, but these traits had limited power to predict alienation. Taking all findings of the study into account, it is evident that the social environment, and specifically a discriminatory social environment, affects feelings of alienation to a far greater extent than personality traits. In addition, the large differences in scores of the "Coloured" and White groups on the measure of alienation and the minimal differences on the measure of personality, provides evidence that what is measured by alienation and what is measured by personality traits, are two distinctly different constructs.

Awareness of HIV/AIDS is quite high, yet prevalence rates have not stabilised and are still climbing. Lack of sexual behavioural change among young people is a major factor which explains why prevalence rates are still on the increase among this population. Sexual behaviour change is influenced by perceptions and attitudes, most notably perceived susceptibility. Information on the psychological factors that affect perceived susceptibility can provide an important base for the development of programmes aimed at reducing further transmission among young people. A survey, using a self-administered, anonymous questionnaire with close-ended questions to collect data, was conducted among Grade 11 learners (n = 318) in Rini, Grahamstown, South Africa. One of the constructs (namely Perceived Susceptibility) in the Health Belief Model informed the data collection. The data generated were first analysed descriptively, providing percentages for responses. Secondly cross-tabulations were calculated. The results showed that knowledge about HIV/AIDS is sufficient, young people receive accurate and non-conflictual messages about sex and HIV/AIDS, they are not discriminatory towards People Living With HIV/AIDS, they have adequate access to healthcare and their perceived susceptibility to HIV/AIDS is high. All these are factors which are favourable and conducive for positive sexual behavioural change. However, the study also found that there was little behaviour change among young people especially regarding regular condom use and decreased sexual activity.

It was demonstrated how the pathological organization was employed as a means of evading paranoid- schizoid fears and depressive position anxieties. Two ways in which defences were assembled into the pathological organization could be traced, both leading to resistance in the therapy. The one set of defences would entail a complete withdrawal from contact, where the patient would hold herself aloof from experience through a solitary invulnerability. The other set of defences involved an active attack on the vulnerable, libidinal self. Identified with the bad, anti-libidinal self, the patient would attempt to annihilate the libidinal self in various ways. Fragmentation of her thoughts and memories, self-destructive behaviour and phantasized and actual attacks on the objects in her world which called forth her neediness were all evidenced. By establishing the practice patterns and values that should be central to the profession according to the literature, and ascertaining the extent to which these are reflected among professionals at this time, this study facilitates a re-evaluation of the professional identity of clinical psychologists in South Africa. To this end, an online survey method was used to collect data from a representative sample of 877 participants, i.e. 29,09% of the population of clinical psychologists in South Africa. Statistical analyses were implemented to address a number of key research questions concerning aspects which characterise professional identity, namely: demographic profile; work settings and roles (practices); and theoretical perspective, beliefs and attitudes (values). Findings suggest that, demographically, clinical psychologists do not mirror the clients they serve, and also that there is a large proportion of the population for whom psychological services remain inaccessible and/or inappropriate. Furthermore, the professional identity of clinical psychologists currently does not comprise practices and values which meet the mental health care needs of South Africa. A significant recommendation of the research is its endorsement of the National Health Insurance model given the promise of its role in ensuring psychology remains relevant. It is recommended that future studies expand on these findings qualitatively, and compare clinical psychology with other mental health care fields. Universities are also urged to relook at their training curricula and ensure that trainees are made of aware of issues of policy, and that areas of research and psychological assessment in complex environments are prioritized.

Themes emergent from literature and included studies distinguished effective from non-effective studies included the use of cognitive behavioural therapy, therapist input and reminders to engage with the intervention, and duration of over 6 weeks. Attrition rates did not differ between experimental and control groups. Sample characteristics between the reviewed interventions and South Africa’s population were found to be different and is discussed. Unfortunately, 37 potential studies could not be accessed and thus not included, although several attempts were made. The majority of studies had effect sizes equal to that of face-to-face therapeutic interventions. Implications of application in South Africa’s context with limited technological familiarity, insufficiency of professionally trained therapists, poor literacy, and high costs of internet data are discussed. It is concluded that with appropriate adaptation, online internet interventions for depression and anxiety would be beneficial to South Africa’s people and assist in lowering the country’s burden of mental health disorders.

- Description: This research examines men’s talk around abortion using critical discourse analysis. Current literature indicates a dearth of studies addressing the topic of men and abortion in various domains. An understanding of men’s relationship to abortion, however, is crucial to understanding abortion as a social phenomenon. This study utilises the work of Foucault around discourse and power, as well as Butler’s work on gender to create a theoretical framework to approach data. Data were collected in the form of interview groups made up of men, as well as newspaper articles and on-line forum discussions that featured men as the author. What emerged from theses texts was a ‘Familial Discourse’ which posits the nuclear, heterosexual family as a long term relationship between a mother and father, which forms the ideal site to raise children. Discourses that support the family are a discourse of ‘Equal Partnership’ which establishes the man and the woman as being in a heterosexual relationship where each partner is seen to have equal power, and a discourse of ‘Foetal Personhood’ which constructs the foetus as a child in need of a family. Related to the heterosexual matrix, the formation of a family unit comes to be constructed as ‘natural’. Abortion acts as a disruptor to these discourses. By disrupting the formation of the family unit, abortion negatively affects the individuals involved. A relationship where a formation of a family unit was disrupted cannot survive. If the female partner has an abortion without her partner, it is seen as disrupting the equal partnership between the man and the woman. Men in this case see themselves as ‘powerless’ compared to women. From this point a ‘New Man’ discourse emerges, where men position themselves as loving and responsible in the context of a nuclear, heterosexual family unit. Abortion disrupts ‘Foetal Personhood’ and is constructed as murder. In the case of rape the ‘Familial Discourse’ can be invoked either to justify abortion or resist abortion, based on whether or not a family unit can be formed. These discourses reproduce patriarchy.

More specifically, it shows that father absence is a highly significant factor in the development of serious male psychological and social difficulties. Much of the material in the text concerns the impact of father absence on male 'sex-role' development. It is, as such further concerned with showing that paternal deprivation can lead to conflicts and rigidities in the individual's sex-role adjustment, which, in turn, are frequently related to deficits in emotional, cognitive and interpersonal functioning. Due consideration is given to such family characteristics as the mother's response to the father's absence which generally manifests as pathological and debilitating intrusiveness on the affected son's masculine identity. Other family features considered are the role of older male siblings and father substitutes e.g. stepfathers in the amelioration or exacerbation of these difficulties. An assumption is made that where such substitutes are competent father absence per se can have no more than a limited influence on the son's sex role development and vice versa. Some dreams from the case are presented for the purpose of viewing how the considered difficulties manifest in dream symbolism. Attention is paid also to the question as to whether such dreams can provide any useful clues toward an understanding of the nature, type and psychological situatedness of the affected people. Most studies associate the absence of the biological fathers with psychological challenges which, among others, include negative emotions such as anger, challenges with maintaining romantic relationships and tend to perform poorly at school. Such research on absence of fathers tends to privilege the biological father discourse in its analysis and often mentions in passing the role played by ‘other’ family members as ‘fathers’. Since during data collection participants kept on referring to the role of other family members or father figures, the study then expanded its scope of inquiry to include this phenomenon. Semi-structured face to face interviews were used to collect data from five university students and thematic analysis was used for data analysis. Psychoanalytic theory was used to specifically to understand or conceptualize the psychological implications on participants caused by the absence of the biological father. Black Social organization theory and Structural Functionalism theory were used to conceptualize the role of other family members/families or father figures in participants’ experiences and the influence of the society they grew up in. This study found that the participants accepted the role of the biological father as central in their lives and its absence resulted in psychological and economical difficulties. To cope with such difficulties, the support from other family members or other father figures was found to be significant in participants lives. Also, this study found that father absence motivates the participants to succeed to better their lives and majority of the participants regarded their grandmothers as father figures.

The same spirit which exists in the field of medicine when there are evidences that an epidemic is beginning to take its toll, must come into being amongst those who are concerned with traffic and its problems. The public itself needs to become more aware of road safety. Many associations and institutions have been created to assist with the inculcation of the road safety ideal. The National Road Safety Council, which creates opportunities for investigation into aspects of road safety must continue to expand its sphere of influence. The research project contained on the following pages represents an attempt to contribute to the human factor in traffic safety, particularly by making a closer study of the driver of the motor vehicle. The writer is concerned with the carnage that is caused by road accidents. His previous study of personality and particularly of the projective techniques of assessment has prompted a consideration of the problem.

- Description: Serious efforts must be made to combat the high accident rate on the roads in the Republic of South Africa. The same spirit which exists in the field of medicine when there are evidences that an epidemic is beginning to take its toll, must come into being amongst those who are concerned with traffic and its problems. The public itself needs to become more aware of road safety. Many associations and institutions have been created to assist with the inculcation of the road safety ideal. The National Road Safety Council, which creates opportunities for investigation into aspects of road safety must continue to expand its sphere of influence. The research project contained on the following pages represents an attempt to contribute to the human factor in traffic safety, particularly by making a closer study of the driver of the motor vehicle. The writer is concerned with the carnage that is caused by road accidents. His previous study of personality and particularly of the projective techniques of assessment has prompted a consideration of the problem.

- Description: Summary: The central purpose of this study was to establish whether there is a positive relationship between a high achievement motive (as measured by the Thematic Apperception Test, using the Arnold's scoring system) in Blacks and Good Job Performance (as measured by good merit and supervisory ratings). Factors taken into consideration in the study were: (i)level of urbanization (ii)resistance to change (iii)job satisfaction. An alternative method of scoring the T.A.T. (McClelland) was compared with the Arnold System. A novel T.A.T. was designed with pictures with which the Blacks could easily identify, so that the writing of stories was facilitated. The level of urbanization and resistance to change were measured on the Urban- Rural Scale. Job satisfaction was measured by means of the Job Satisfaction Index. The measures of job performance were obtained by means of 'man specifications' and merit ratings. The Achievement Motivation Score was found to be significantly positively correlated at the ,05 level with job performance ratings, but a predictive study would have to be carried out before it can be accepted as predictive of job performance. Level of urbanization had no significant effect on the achievement, motivation scorer (Arnold System) whereas 'the 'Resistance to Change' factor was found to be significantly negatively correlated at the ,05 level with the achievement motivation scores (Arnold). 'Too little information was available to establish the relationship between high achievement motivation scores (Arnold), low job performance ratings and low job satisfaction scores. No significant correlations was obtained between the McClelland scores on the T.A.T., and the Arnold scores as well as between the McClelland scores on the T.A.T. and job performance ratings. The T.A.T., using the Arnold scoring system, proved to be a reliable test (both test - retest and inter - scorer reliabilities This study showed that the T.A.T. has strong possibilities of playing a large role in the selection of Blacks for higher level jobs in industry.

- Description: Adolescents have become focal points of discussions and debates regarding sexuality and reproductive health matters. However, little research has been done particularly in South Africa to examine their knowledge concerning abortion and emergency contraception. Research indicates that a substantial proportion of adolescent pregnancies are unintended or unwanted. Abortion and emergency contraception are both time-sensitive services. Thus having accurate and comprehensive knowledge about both abortion and emergency contraception is pivotal, in the case of unintended or unwanted pregnancy or when engaging in unprotected sex or experiencing contraceptive failure that could lead to pregnancy. The 1994 International Conference on Population and Development (ICPD) defined reproductive health rights for both men and women as the right to "decide freely and responsibly the number, spacing and timing of their children and to have the information and means to do so" (p. 60). That is, men and women should "have the right to make decisions concerning their reproduction free of discrimination, violence and coercion" (ICPD, 1994, p.60). Based on these definitions, it is rather evident that comprehensive and accurate knowledge are at the core of one's ability to make an informed consent. This is confirmed by Adler's (1992, p. 289) definition of informed consent or choice "a) access to sufficient information b) understanding the information c) competence to evaluate potential consequences d) freedom to make a choice and e) the ability to make and express that choice". It is from this framework that this study emerged. The aim of this study was to examine adolescents' knowledge concerning abortion and emergency contraception. The participants were Grade 11 learners between the ages of 15-24 years from five different schools in the Buffalo City Municipality. A sample of 514 was achieved. Data were analysed using descriptive cross-tabulation, chi-square and qualitative methods where appropriate. The results revealed that most of the participants did not have sufficient accurate knowledge concerning the Choice on Termination of Pregnancy Act, consequences of legal abortion and emergency contraceptive pills to make informed decisions. Furthermore, data also revealed that the participants' schools playa role in their sexual activity, their knowledge about the Choice on Termination of Pregnancy Act and about emergency contraceptive pills. Although this method made it feasible for the researcher to make general assumptions, non-responses were one of the limitations of the study. Similar research in various municipalities/cities in and outside the Eastern Cape is recommended so as to increase further awareness concerning the level of knowledge that adolescents have about contraceptive pills particularly emergency contraceptive pills, the Choice on Termination of Pregnancy Act and abortion in general.

High prevalence of FASD has been observed in certain parts South Africa, but many areas of the country remain unstudied, and factors contributing to drinking among pregnant women are poorly understood. This study surveyed 1099 women attending antenatal care at public primary healthcare facilities in the Buffalo City Metropolitan Area in the Eastern Cape. Data was collected using the Alcohol Use Identification Test, a 10-item self-report measure of drinking behaviour in isiXhosa, English, and Afrikaans. Factors associated with alcohol use, risky drinking, binge drinking, and hazardous/harmful drinking were identified using logistic regression. A minority of the sample reported alcohol use (64%). Among those reporting alcohol use, most reported occasional binge drinking (63%) and met criteria for risky drinking (59%) and hazardous/harmful drinking (52%). Living with a regular drinker was significantly associated with alcohol use (OR 1.98, 95% CI 1.51 – 2.58), risky drinking (OR 2.03, 95% CI 1.49 – 2.76), binge drinking (OR 2.21, 95% CI 1.64 – 2.97), and hazardous/harmful drinking (OR 2.0, 95% CI 1.41 – 2.99). However, women who were married/cohabiting were less likely to report alcohol use (OR .71, 95% CI .53 - .95). Experiencing intimate partner violence during the current pregnancy was associated with alcohol use (OR 2.42, 95% CI 1.38 – 4.27) and hazardous/harmful drinking (OR 2.62, 95% CI 1.32 – 5.22). In this study, women who identified as Coloured were more likely to report alcohol use than Women who identified as African (OR 2.74, 95% CI 1.4 – 5.36). These findings simultaneously provide evidence of problematic drinking among pregnant women in a previously understudied area and show that external factors affect women’s drinking behaviour during pregnancy. - Description: This study aims to explore, and to a certain extent to clarify, what it means psychologically to experience Traumatic spinal cord injury (TSCI) in a South African spinal unit. The target time chosen for analysis is the acute medical period. The study presents a review of the literature on psychological adjustment to TSCI and then proceeds to introduce and extensively articulate the hermeneutic approach and methodology. Subsequently, an interpretive research strategy is presented for the purpose of studying the acute phase of TSCI. The data for the study was obtained by means of three dialogical interviews which were tape recorded, transcribed and analyzed with~n a cyclical framework consisting of three interdependent levels. The interpretive procedure is modeled upon the 'reading guide' developed by Brown, Tappan, Gilligan, Miller and Argyris (1989). The analysis follows a course from the individual psychological descriptions of the experience to the generation of a general, nomothetic narrative account of the acute phase. The findings are then discussed in relation to the existing literature and evaluated on the basis of the goals of the study. The study highlights the value of some of the 'stage' ways of thinking about SCI adjustment, while simultaneously stressing the need for placing adjustment within a more personalized, and individually meaningful context. Significant differences between the psycho-physical experiences of patients in the categories of complete and -incomplete SCI were found, which suggests that a sharper distinction be made in the literature between these two groups, in order to account for the marked variations in their experiences.-- The study also contains a number of shortcomings, such as a lack of understanding about certain historical and contextual factors which may have mediated the experiences of the trauma for the individuals concerned. These shortcomings and some suggestions fro their resolution are then discussed. The study concludes with an evaluation of the research strategy and methodology and also offers some suggestions for future research.
