The design, synthesis and antiplasmodial activity of a series of halogenated fosmidomycin analogues and hybrid drugs

- Description: Malaria continues to be a devastating disease and a major cause of death in sub-Saharan Africa. With resistance against most of the available antimalarial drugs, there is a need for ongoing research and development of antimalarial agents. Fosmidomycin and its acetyl analogue FR900098 have been identified as potent inhibitors of Plasmodium falciparum, the causative agent of the most deadly form of malaria. Clinical trials of these agents have revealed poor absorption due to their high hydrophilicity. In the present studies the effect of halogenation of the acyl chain as well as the biological effect of extending the acyl sidechain was explored. This provided the basis on which fosmidomycin hybrids were designed to investigate the feasibility of hybrid extending into NADPH binding pocket. Synthesis of a series of halogenated FR900098 analogues was carried out in three stages. This included i) The introduction of the phosphonate group by reaction with 1,3dibromopropane in an Arbuzov reaction, ii) The introduction of a hydroxamate group by reaction of the propyl phosphonate by means of a nucleophilic substitution reaction with BocNHOBn and iii) The introduction of a halogenated acyl side chain on a protected fosmidomycin backbone. The synthesis of fosmidomycin-hybrids for which chloroquinefosmidomycin hybrids were used as the prototype, involved convergence of the two separately constructed moieties i.e. fosmidomycin and the quinoline moieties in a covalent linkage. The quinoline moiety was easily synthesized from the reaction of 4,7dichloroquinoline with 1,2-diamino ethane. The aminoquinoline so formed resulted in chloroquine-fosmidomycin hybrids 3.8 and 3.9 when reacted with halogenated FR900098 analogues. Antiplasmodial assays were conducted on the chloroquine-fosmidomycin hybrids and the halogenated fosmidomycin derivatives against the chloroquine resistant Gambian FCR-3 strain of P. falciparum. The most potent iodoacetyl fosmidomycin analogues 2.21 gave an IC50 value of 5.54 µM which is eight times more potent than the known antiplasmodial FR900098 which gave an IC50 value of 41.67 µM. All the halogenated FR900098 analogues showed better antiplasmodial activity than their non-halogenated derivatives. This indicated that the presence of halogens in the FR900098 analogues contributes to their biological Chapter 1 Literature review activity. The acetyl and propyl linked hybrids 3.8 and 3.9 showed potent antiplasmodial activity with IC50 values of 0.18 and 0.82 µM respectively. These were by far the most potent hybrids synthesized and provided leads for a new class of promising antimalarial agents. Preliminary E. coli DXR enzyme inhibition assays were carried out on the halogenated fosmidomycin analogues. The results showed good inhibition of the enzyme by the phosphonic acids of the chloroacetyl and chloropropyl analogues 2.1 and 2.2 respectively. Molecular modelling of the compounds on E. coli (PDB code: 2EGH) and P. falciparum (PDB code: 3AUA) DXR showed strong binding of the halogenated fosmidomycin analogues while the hybrids in the absence of docked NADPH showed minimum binding to the enzymes.

The writer's interest in this phenomenon grew out of her own experience of this situation as a clinical Masters coursework student, an experience which was of important to the writer and meaningful still as a therapist in training. In dialogue with experienced therapists the importance of this situation was again made apparant. It was the opinion of these therapists that although for some the details of this experience had become dulled by time, what remained meaningful to them was that this experience was seen as the beginning of a project which remains important to them - they identified this situation as an important moment in the history of their development as psychotherapists. In the hope that the literature pertaining to psychotherapy would throw some light on this situation, the writer turned to a number of sources in this area to discover that no literature available to her elucidated this situation in a holistic manner. This led the writer to go back to the beginning therapists themselves so that they may speak for themselves of their experience of this situation. A phenomenological method of enquiry is implemented in this study as it renders the subject matter accessible to investigation, and allows it to reveal itself as it essentially is. - Description: The purpose of this study is to ascertain whether the clients who make use of the services of the Psychology Clinic at Rhodes University perceive the masters students being trained at the Clinic as providing adequate psychotherapy. Using a sample of 18 clients, who underwent psychotherapy at the Rhodes Psychology Clinic during 1990, a follow-up study was conducted to ascertain the perceived success of that psychotherapy. Those variables most pertinent to the outcome of psychotherapy are examined. The study uses both qualitative and quantitative methodology and shows that, overall, 89% of the clients who participated in this study perceived the psychotherapy at the Rhodes Psychology Clinic to be satisfactory. Archontic Aporias: the mediums of memory

- Description: Contextualising my research within the Eastern Cape as a descendant of 1820 settlers, I question the modalities of historical recollection by introducing memory as a viable mode of archival production alongside that of the archive. Through interrogating Eastern Cape archival institutes and employing an autoethnographic approach to my familial archives, I show that archival curation affects the gaps, schisms, and interpretations of archives as much as the ‘unreliability’ of memory. I rely on definitions from Jacques Derrida and literature from Achille Mbembe and Verne Harris, as well as reflexive methodologies, to engage the ways of remembering the past and methods of storytelling. With this undertaking, I expose the aporias within archival processes. This written component is part of broader research that encompasses theoretical study and a practice-based Fine Arts research project culminating in an exhibition that shares themes of memory, archive, trauma, and curatorial and personal heritage management. - Description: While depression is experienced worldwide, the World Health Organization indicated that over 80% of this disease burden is found in low- and middle-income countries, with 16% of depressive disorder cases being found in Africa. Specifically, in South Africa, the lifetime prevalence of major depressive disorder is 9.7%. Research conducted on traditional healing estimates that as high as 80% of black individuals in South Africa choose to consult traditional healers. Although there was initial consensus that the use of traditional healers reflected a lack of access to alternative resources, more recent research has however indicated that traditional healers are consulted even when psychiatric facilities and medication are available. Research has illustrated that many patients that chose to consult traditional healers do so as a result of their shared beliefs, and traditional healers' ability to understand the cultural frameworks underlying illness. The current study investigated how traditional healers in the Eastern Cape perceive and treat phenomena currently understood as the symptoms of depression and how these understandings can be incorporated into collaborations between Western medicine and traditional healing practice. A qualitative research design was used to examine the perspectives and treatment methods of isiXhosa traditional healers in the Eastern Cape. Four healers participated in the study. Key findings indicated that there is no single cause or definition of the phenomenon of depression by healers. Causality is greatly attributed to spiritual causes and ancestors play a key role in guiding the diagnosis and multiple treatment interventions employed by healers to treat the experience of depression. Studies that concern marital satisfaction provide us with crucial insights into factors that support or compromise the longevity of long-term, committed relationships. Little work has been done in understanding the role of cultural practices in marital satisfaction particularly in Africa, and South Africa more specifically. Ukuyalwa is a Xhosa ritual that takes place during a Xhosa traditional wedding, whereby the bride is introduced to the (traditional) guiding practices of her new home by the women of the family together with her sisters-in-law. This study informs the practice of African psychology as formulated by Ratele (2017b).Critical African psychology as a framework recognizes the construction of culture alongside political, socioeconomic, and historical matrices. These are acknowledged as an important element in psychological theories, methods, and explanations. This research project aimed to identify the discourses that circulate around ukuyalwa as a Xhosa cultural practice and consider the implications for the marital satisfaction of amaXhosa couples. The study interviewed 3 Xhosa couples of varying ages who had been married for different durations to develop an understanding of their experiences of ukuyalwa and marital satisfaction. Data was analyzed using a Foucauldian Discourse Analysis (FDA) which allowed for the identifications of the discourses that the couples used to construct ukuyalwa and marital satisfaction. The couples discursively constructed marriage as ‘divine unity’, ‘unification of families’ and ‘partnership’. Ukuyalwa was constructed as a ‘gendered process’, ideas of tradition and modernity, and a ‘foundation’ also became apparent. Satisfaction was constructed as ‘open communication’ and ‘work’. The couples’ positions shifted in their conversations, where in some instances they were 'learners’, ‘initiates’, and/or ‘passive’, and in other instances, they were ‘in charge’, ‘knowledgeable’, and ‘experts’. The discourses of culture, religion, and partnership circulated prominently in relation to marriage, ukuyalwa, and marital satisfaction. The findings of the study speak to how the couples’ constructions of marriage and cultural customs such as ukuyalwa are valuable as they often inform how couples behave in their marriage relationships and what they regard as satisfactory to them when it comes to marriage. The novobiocin-induced turnover of fibronectin via low density lipoprotein receptor-related protein 1 alters matrix morphology with physiological consequences on cell growth and migration

- Description: Fibronectin (FN), an extracellular matrix protein, is secreted as a soluble dimer which is assembled into an insoluble extracellular matrix. The dynamics of FN matrix assembly and degradation play a large role in cell migration and invasion thereby contributing to the metastatic potential of cancer cells. Previous studies have shown the direct binding of Heat Shock Protein 90 kDa (Hsp90) and FN in vitro, and that inhibition of Hsp90 with novobiocin (NOV) caused internalisation of the FN matrix. Low density lipoprotein receptor-related protein 1 (LRP1) is a ubiquitous receptor known to bind both Hsp90 and FN. Using an LRP1 expressing Hs578T breast cancer cell line and an isogenic mouse embryonic fibroblast (MEF) model system of differential LRP1 expression we demonstrate that LRP1 is involved in turnover of FN in response to C-terminal Hsp90 inhibition. The first objective of this study was to identify the mechanism of NOV-induced LRP1-mediated FN turnover. Our data show that NOV-mediated FN turnover via LRP1 did not require the activity of matrix metalloproteinases (MMPs), which play an important role in processing and degradation of the extracellular matrix and FN. In addition, the levels of the main FN receptor responsible for its extracellular assembly, β1-integrin, did not change in response to NOV. LRP1 is known to undergo regulated intramembrane proteolysis (RIP) which generates smaller fragments that may translocate to the nucleus and modulate gene transcription. Using inhibitors of LRP1 cleavage and nuclear fractionation we determined that LRP1 processing was not required for the NOV-induced FN response suggesting that a mechanism unrelated to LRP1 RIP is involved. A possible mechanism may be in altered Hsp90-LRP1 cell signalling as we observed disruption of the FN-Hsp90-LRP1 complex at the cell surface in NOV treated cells. How this affects downstream eHsp90-LRP1 signalling is still to be determined but may be related to a significant increase in phospho-AKT and loss of phospho-ERK upon NOV-treatment; two key signalling proteins involved in FN matrix regulation and which are downstream of LRP1 signalling. The second objective of this study was to determine the physiological consequences associated with FN turnover in response to NOV treatment. Using migration assays we demonstrated that levels of insoluble matrix-associated FN and FN concentration are not solely responsible for migratory capacity of cells on decellularized extracellular matrices, but rather that structural composition and integrity of the matrix plays a bigger role. Using confocal and scanning electron microscopy, we identified NOV treated matrices to be flatter, less mature and contain thicker, rope-like FN fibrils to which cells adhered better but were generally less proliferative. Comparatively, cells adhered less to the more mature and 3-dimensional untreated matrices but exhibited increased spreading and cell growth, which may in part be due to the thinner fibrils and web-like matrix. In summary, this study substantiates the role of LRP1 in NOV-mediated FN turnover, and provides new insights into the possible mechanisms of the Hsp90-LRP1 mediated loss of FN matrix. Alcohol use among women attending antenatal care in Buffalo City, Eastern Cape

High prevalence of FASD has been observed in certain parts South Africa, but many areas of the country remain unstudied, and factors contributing to drinking among pregnant women are poorly understood. This study surveyed 1099 women attending antenatal care at public primary healthcare facilities in the Buffalo City Metropolitan Area in the Eastern Cape. Data was collected using the Alcohol Use Identification Test, a 10-item self-report measure of drinking behaviour in isiXhosa, English, and Afrikaans. Factors associated with alcohol use, risky drinking, binge drinking, and hazardous/harmful drinking were identified using logistic regression. A minority of the sample reported alcohol use (64%). Among those reporting alcohol use, most reported occasional binge drinking (63%) and met criteria for risky drinking (59%) and hazardous/harmful drinking (52%). Living with a regular drinker was significantly associated with alcohol use (OR 1.98, 95% CI 1.51 – 2.58), risky drinking (OR 2.03, 95% CI 1.49 – 2.76), binge drinking (OR 2.21, 95% CI 1.64 – 2.97), and hazardous/harmful drinking (OR 2.0, 95% CI 1.41 – 2.99). However, women who were married/cohabiting were less likely to report alcohol use (OR .71, 95% CI .53 - .95). Experiencing intimate partner violence during the current pregnancy was associated with alcohol use (OR 2.42, 95% CI 1.38 – 4.27) and hazardous/harmful drinking (OR 2.62, 95% CI 1.32 – 5.22). In this study, women who identified as Coloured were more likely to report alcohol use than Women who identified as African (OR 2.74, 95% CI 1.4 – 5.36). These findings simultaneously provide evidence of problematic drinking among pregnant women in a previously understudied area and show that external factors affect women’s drinking behaviour during pregnancy. A dashboard (coupled with data visualization) is regarded as a suitable way to present personal health data effectively, to inform self-care and self- management-based decisions. However, there is no homogeneous (i.e. “one-size-fits-all”) approach to designing dashboards; therefore, this research aimed at creating a set of design guidelines that can be used as a foundation for tailoring dashboards that support the presentation of personal health data. The Design Science Research methodology was used to systematically develop and evaluate the set of dashboard design guidelines. Within this methodology, literature was reviewed and focus groups with participants were conducted to extract insights and inform the development of the initial set (proposed) design guidelines. The proposed design guidelines were then used to develop a medium-fidelity personal health dashboard prototype. Usability testing with the dashboard prototype was conducted to evaluate the efficacy and utility of these design guidelines. The evaluation provided insights that strengthened the quality of the design guidelines. The evaluation indicated that the proposed set of guidelines are suitable for designing a dashboard that presents personal health data effectively. The proposed design guidelines were then used to develop a medium-fidelity personal health dashboard prototype. Usability testing with the dashboard prototype was conducted to evaluate the efficacy and utility of these design guidelines. The evaluation provided insights that strengthened the quality of the design guidelines. The evaluation indicated that the proposed set of guidelines are suitable for designing a dashboard that presents personal health data effectively.
