A qualitative approach was utilized following an exploratory, descriptive research design as the researcher sought to understand the perspective of the managers at Nerina regarding causes of re-offending and the effectiveness of the intervention programmes aimed at addressing this phenomenon. The researcher conducted face to face interviews with the mangers at Nerina One Stop Youth Justice Centre. For the purpose of this study the population was the total complement of the management staff at the One Stop Youth Justice Centre, this included the personnel of the departments of Justice, Social Development, and South African Police Service who are all involved in rendering services at Nerina. The method of data collection was one on one semi structured interviews and data analysis undertaken according to the steps for qualitative data analysis as proposed by Tesch in Creswell (1994). Guba’s model (Krefting, 1991) was employed for assessing the trustworthiness of the qualitative data. Some of the important findings included the causes of re-offending which included loss of parents, alcohol and drug abuse, inadequate parent role, role of peers and inadequate facilities. Further more from the study suggestions for improving programmes at the One Stop Youth Justice Center were discussed and these include involvement of community, involve more role players, training staff and increasing staff.

Antibiotic use in two hospitals in West Wollega, Ethiopia

Overuse or inappropriate use of antibiotics has resulted in a major increase in the development of multi-drug resistant pathogens. Antimicrobial resistance is one of the world’s most serious public health problems with great implication in terms of morbidity, mortality, and costs. To date, there has been no formal antibiotic use study conducted in the West Wollega zone of Ethiopia to assess antibiotic utilization. The objective of this study was to determine the pattern of antibiotic use in two hospitals in the West Wollega zone of Ethiopia, namely Gimbie Adventist Hospital and Nedjo Hospitals, using drug utilization metrics and the costs associated. In addition it was to assess the correlation between diagnosed infectious diseases and antibiotic prescriptions. This study was a cross-sectional, retrospective, descriptive review of antibiotic usage in the two hospitals in the year 2007. Prescriptions dispensed in the first month of each quarter of 2007 were reviewed. The number of prescriptions screened, antibiotic courses started, antibiotic days by specific agent and overall, the cost of individual and all antibiotics, the number and type of infectious diseases diagnosed were collected from which core drug use indicators were calculated. The correlation between infectious disease diagnosed and the antibiotic days prescribed were analyzed. A total of 18568 antibiotic and non-antibiotic prescriptions were reviewed retrospectively in the four months of the study period, 47 percent of which contained at least one antibiotic. The average number of antibiotics per prescription was 1.33 and 1.09 whilst the percentage of injectable antibiotics prescribed was 83.2 percent and 3.76 percent to outpatients and inpatients respectively. Antibiotics prescribed from the Essential Drug List (EDL) and List of Drugs for District Hospital (LDDH) were 63.0 percent, 74.8 percent, and 90.8 percent and 76.1 percent for outpatients and inpatients respectively. 98.6 percent of outpatient and 97.0 percent inpatient prescribed antibiotics were actually dispensed. Penicillins and quinolones were the most prescribed antibiotics in both inpatient and outpatient departments constituting 43.46 percent and 24.08 percent respectively. The antibiotic utilization ratio, incidence of outpatient antibiotic use, incidence of inpatient antibiotic use, the number of Defined Daily Doses (DDD)/1000inhabitants/year and DDD/100 Occupied Bed Days (OBD) for the zone was 0.16, 17.25, 23.56, 158.61, and 70 respectively. Antibiotic cost constituted 33.7 percent of all expenditure on drug, cost of antibiotic per patient care day and cost per antibiotic day was 3.84 Ethiopian Birr (ETB) ($0.40) and 6.29 ETB ($0.66) respectively. The correlation between infectious diseases diagnosed and antibiotic prescription shows significant variation. At outpatient departments alone an average number of antibiotic courses started was 2.7 at Gimbie Adventist Hospital and 7.6 for Nedjo Hospital. When overall antibiotic days prescribed and required was compared in both hospitals, there were 2.4 and 5 times more antibiotic days prescribed than were required for Gimbie and Nedjo Hospitals respectively. This suggests that the overuse of antibiotic is worse in the government hospital (Nedjo Hospital) than in the mission hospital (Gimbie Adventist Hospital). This study suggested that there was overuse of antibiotics in the West Wollega hospitals although further investigation is needed to identify its underlying causes and nature. It is recommended that the health personnel, the hospital management, the zonal and regional Health Bureau, the regulatory bodies and Non-Governmental Organizations (NGOs) work hand-in-hand to promote the rational use of antibiotics in this region so that the consequences and financial cost of antimicrobial resistance can be prevented.

- Description: Currently some confusion exists as to how health professionals should best respond to the psychological needs of those affected by disasters. Some have argued that early psychological intervention is essential and others have argued that early formal psychological interventions have no useful role in post trauma response. This study highlights the importance of considering both counselling and non-counselling factors as potentially influencing the psychosocial response of disaster victims. Although posttraumatic stress disorder (PTSD) is not the only mental disorder that can develop as a result of exposure to disasters, it is probably the most frequent and debilitating psychological disorder associated with traumatic stress. In this exploratorydescriptive study the researcher aimed to explore and describe psychologists’ perceived influences of early strategies on the psychosocial response to those affected by disaster. The researcher used non-probability snowball sampling to access participants. The sample consisted of 5 participants. Semi structured interviews were conducted. Content analysis was used to analyse the data obtained from interviews. Results that emerged from the data suggest that there are many factors that influence the psychosocial response to those affected by disasters. These factors include the screening process, needs of survivors, the method of choice for treatment, the timing of intervention, pharmacology, the South African context, training and planning. The study makes a contribution to the growing knowledge of early strategies in response to those affected by disasters.

- Description: In South Africa the pharmacy profession has experienced a number of changes around the turn of the century such as the introduction of the National Drug Policy (NDP), pharmacy ownership and price regulation. With this the role and earnings of the pharmacy profession, as well as to what extent the pharmacist adds value to the profession and society, are being questioned. Community pharmacists are thus faced with the challenge to prove that the value that they add to society is meaningful. Therefore, the aim of the study was to document community pharmacy availability and activities in South Africa and based on this to quantify the perceived value that the community pharmacist adds to society through the delivery of pharmaceutical services and pharmaceutical care. In order to determine the pharmacist’s true value added two surveys were conducted in 2006; an original pharmacist survey and a general public survey. The results obtained were verified by a follow–up pharmacist survey in 2009 to confirm or reject the results obtained in the original survey. The study was representative of both the community pharmacies and the general public in South Africa and was primarily quantitative in design and analysis. More than half of the responding pharmacies (63.16%) were open seven days a week. The average hours of service per day ranged from 10 hours (Monday to Friday) through to 6.45 hours on Saturdays and 3 hours on Sundays. Pharmacists continuously upgraded their professional knowledge. More than three quarters of pharmacies had the necessary equipment available to perform the services investigated in the study. The general public was not aware of all the services provided by pharmacists and as a result, depending on the service, many people did not make use of these services. The general public that made use of services delivered by community pharmacies mainly perceived the services delivered to be of good quality. The main barrier to practicing pharmaceutical care was indicated by pharmacists as not receiving payment for the advice given followed by pharmaceutical care being time consuming, and that there was not enough time to talk to patients. The general public indicated that they found it difficult to ask questions in pharmacies because other patients could hear what was discussed, or other patients had to wait longer if they asked something, and pharmacy staff being too busy. The results of the original pharmacist and the general public survey were confirmed by the results of the follow-up survey with the exception of dispensing prescription medicine (8 minutes 28 seconds), OTC medicine (7 minutes 23 seconds), counselling of prescription medicine (8 minutes 51 seconds) and OTC medicine (8 minutes) which on average took longer to conduct than in the previous analysis. The study highlighted the value added to the wellness and quality of life of the community of South Africa through the delivery of pharmaceutical care and pharmaceutical services by community pharmacy staff, and proved that pharmacists are committed to the provision of pharmaceutical care and pharmaceutical services.

- Description: Children are placed in a children’s home when a Presiding Officer finds them to be in need of care and when there is no viable community placement alternative. The body of literature on children’s homes focuses primarily on the negative effects and outcomes of such statutory placement. The assertion that children who grew up in children’s homes will continue to use the services of welfare organisations in adulthood, is supported by the researcher’s observation as a practicing social worker. This, and the study of available literature, resulted in this qualitative, exploratory-descriptive and contextual research study with the following goal: to enhance the understanding of how intervention programmes at children’s homes can contribute to adult adjustment and independent functioning of those children in their care. A purposive sampling method was used to identify ten participants who had spent at least 2 years in a children’s home and who had been discharged from the children’s home at least 5 years ago (to the date of data collection). The sample was drawn from clients at non governmental social welfare agencies who fitted the sampling criteria. The data was collected via semi-structured interviews using an interview guide, which were recorded, transcribed and then analysed using a thematic content analysis approach. The results of the study may be used to develop and implement meaningful intervention strategies for individuals placed in children’s homes.

Systemic influence on Black South African adolescents' career development : adolescent and parental perspectives

- Description: Currently, no career theories exist that sufficiently explain the career development of South Africa’s diverse population groups. Consequently, South African researchers have been entirely dependent on international, western-informed career theories. While such theories have taken on a more ethnocentric complexion in recent times, they remain essentially decontextualised for South Africa. Furthermore, although the influence of family and the significant roles of parents have been theoretically acknowledged as critical influences in adolescent career development, there is still a considerable lack of research in South Africa on this topic. The present study therefore explored the perceptions of systemic influences on adolescent career development from the perspectives of both Black middle-class South African Grade 11 learners and their parents. The research was conceptualised within the Systems Theory Framework (STF) of career development and used its derivative instrument, the My Systems of Career Influences (MSCI, Adolescent). The present study utilised a qualitative research method, using answers derived from the MSCI (Adolescent) booklets to inform semi-structured interviews. The data analysis procedure involved the use of Interpretive Phenomenological Analysis to qualitatively analyse data obtained from the semistructured interviews. Findings revealed a number of influences within the individual system (personality, values, abilities), social system (adolescents’ parents and teachers) and societal-environmental system (financial support, the opportunity to work overseas, geographical location, job availability, and the location of universities) that were acknowledged as having an influence on the career development of South African Black middle class adolescents. Each of these findings were explored and unpacked under the four xii identified Interpretive Phenomenological Analysis (IPA) superordinate themes of Family Dynamics, Great Expectations, The Ghost of Apartheid and Coconuts Fall Far From the Tree. Lastly, the limitations of the present study, as well as recommendations for future research are discussed.

- Description: Currently, no career theories exist that sufficiently explain the career development of South Africa’s diverse population groups. Consequently, South African researchers have been entirely dependent on international, western-informed career theories. While such theories have taken on a more ethnocentric complexion in recent times, they remain essentially decontextualised for South Africa. Furthermore, although the influence of family and the significant roles of parents have been theoretically acknowledged as critical influences in adolescent career development, there is still a considerable lack of research in South Africa on this topic. The present study therefore explored the perceptions of systemic influences on adolescent career development from the perspectives of both Black middle-class South African Grade 11 learners and their parents. The research was conceptualised within the Systems Theory Framework (STF) of career development and used its derivative instrument, the My Systems of Career Influences (MSCI, Adolescent). The present study utilised a qualitative research method, using answers derived from the MSCI (Adolescent) booklets to inform semi-structured interviews. The data analysis procedure involved the use of Interpretive Phenomenological Analysis to qualitatively analyse data obtained from the semistructured interviews. Findings revealed a number of influences within the individual system (personality, values, abilities), social system (adolescents’ parents and teachers) and societal-environmental system (financial support, the opportunity to work overseas, geographical location, job availability, and the location of universities) that were acknowledged as having an influence on the career development of South African Black middle class adolescents. Each of these findings were explored and unpacked under the four xii identified Interpretive Phenomenological Analysis (IPA) superordinate themes of Family Dynamics, Great Expectations, The Ghost of Apartheid and Coconuts Fall Far From the Tree. Lastly, the limitations of the present study, as well as recommendations for future research are discussed.

Mental health services form an integral part of the integrated primary health care package as implemented in the campus health service. Students attend the Campus Health Service for help related to their health. Health care is provided by professional nurses and counsellors through a comprehensive primary health care service which serves students as well as staff. Some students may state that they experience feelings of being stressed or depressed. Other students may complain of physical symptoms such as headache or upper backache. Upon further investigation emotional problems may be identified as the cause of the psychosomatic symptoms. The professional nurses working in the campus health service verbalised that it is sometimes difficult to identify a mental illness or mental distress as there is no effective assessment tool that they can use. Management of conditions is also problematic as there are no protocols indicating the therapeutic interventions that can be taken. The research question in this study was therefore: What information should be included in a protocol to assess and manage a student experiencing mental distress that can be used by professional nurses working in a Campus Health Service? The aim of this study is to develop a mental health care protocol for campus health service professional nurses to assess and manage university students who are experiencing mental distress. The research design of this study was qualitative, explorative, descriptive, explanatory and contextual. In this study the Delphi research technique was used to create an instrument to standardise mental health care in a campus health service. The Delphi technique is a series of sequential questionnaires or “rounds” interspersed with controlled feedback that seeks to gain the most reliable consensus of opinion of a group of experts. A questionnaire was developed based on an extensive literature review. The research population of this study consisted of two groups: professional nurses with knowledge of student health care needs and expert psychiatric nurses. The study was conducted at the Campus Health Service at a university in the Eastern Cape. The data collection and analysis was done utilising the Delphi technique. Trustworthiness was ensured by using the Lincoln and Guba Model utilising the criteria of credibility, applicability, dependability and conformability. In this study the ethical principles of beneficence, non-maleficence, justice and self determination were applied to ensure that participants are treated with respect and consideration and ensured high ethical standards. Informed consent was obtained from the participants in this study. The findings of this research were utilised to assist the researcher in developing a protocol for mental health care of students in campus health service settings.

- Description: Increasingly, developmental psychology has emphasized that childhood career development should be viewed as holistic and comprise all aspects of a child’s maturation. This would include an emphasis on the career development of children which is considered vital to the complete education of the child (Brown, 2002). Career development refers to the process of developing beliefs, values, skills, aptitudes, interests, personality characteristics and knowledge of work (Zunker, 2006). Research has indicated that early societal factors and personal preferences associated with gender influence the child’s later occupational aspirations (Stockard & McGee, 1990). There are many ways in which individuals learn about gender roles and acquire “gender-appropriate” behaviour during childhood, some of which manifest in the occupational aspirations of children. As children grow up they learn, through reinforcement and modeling, that society has different expectations and standards for the behaviour of males and females. While family and friends are often the most important agents of socialization in young children, television and other popular media have also played a vital role in gender stereotyping, resulting in children forming perceptions regarding which occupations “belong” to which gender (Taylor, Peplau, & Sears, 2006). The present research aims to explore, describe and compare the occupational aspirations and the occupational gender stereotyping of male and female South African and Australian senior primary school learners. The research approach for the study was descriptive and exploratory in nature and was conducted within a quantitative framework. A survey-type questionnaire, the Career Awareness Survey xiii (McMahon & Watson, 2001), was used as the data collection measure as part of a larger international study. The sample comprised of 511 South African and 372 Australian participants from Grades 6 and 7. Responses to the occupational aspirations questions were coded according to Holland’s (1985) interest typology and status level coding for occupations. For descriptive purposes, frequency counts were computed for the coded typology, status level and occupational gender stereotyping data. The z-test and chi-square test for independence were computed in order to test whether gender groups differed in terms of their occupational aspirations and occupational stereotyping. The chi-square test was also used to compare the occupational aspirations and gender stereotyping of South African and Australian senior primary school learners. The results of the present research indicate that male and female South African and Australian female children tend to aspire towards more Investigative and Social type occupations in the high status level category. The Australian male children, however, tend to aspire towards more Social and Realistic type occupations in the high status level category. Across nation and gender, the majority of the children believed that both males and females could perform certain occupations, with senior primary school children tending to limit the range of occupations which they believe to be predominantly suited to either male or female. Cross-national comparative results yielded interesting findings with few significant differences emerging on occupational aspiration typology, status level and the occupational gender stereotyping of occupations. The results of the present research emphasise the need for further cross-national comparative studies on the occupational aspirations and occupational gender stereotyping of senior primary school children.

- Description: Previous studies have indicated an association between childhood sexual abuse (CSA) and an increased risk of engaging in unsafe behaviours during adulthood, including risky sexual practices. This study examined the relationship between CSA and adult HIV sexual risk behaviour among a sample of South African men who have sex with men (MSM). Potential pathological long-term mental health outcomes of CSA, including dissociation, sex-related substance abuse, depression, sexual compulsivity, impaired interpersonal communication, and over-reliance on submissive sexual scripts, were treated as variables mediating the relationship between CSA and sexual risk behaviour. Men frequenting a gay internet dating site were randomly selected to complete an electronic version of the anonymous survey. Results indicate that one-fourth of participants reported a history of CSA. Men with a history of unwanted sexual activity during childhood were more likely to report recreational substance abuse, sex-related substance abuse, sexual compulsivity, and adult revictimisation experiences. Men who were abused were also more likely to engage in unprotected anal intercourse compared to those who were not abused. Mediation analyses revealed that MSM who are survivors of CSA are particularly susceptible to drug abuse, sex-related drug abuse, and sexual compulsivity, and these sequelae in turn predict higher reported numbers of male sexual partners. The current data suggest that CSA is widespread among men at high risk for HIV infection, and that it may have a devastating influence on the quality of life and health risk behaviour of these men. These results also highlight the importance of mental health services and new approaches in HIV prevention for MSM who have been sexually abused as children. Further research is needed into the contextual factors of the childhood abuse experience which account for the variability in longterm negative mental health outcomes of CSA survivors.

RFAs were undertaken to investigate the effect of the introduction and use of exposure charts (ECs) on department and student reject rates of extremity radiographs. Methods: A quantitative comparative pre and post-treatment research design was used. Data was collected from the x-ray departments of two training hospitals in Windhoek, Namibia over a five month period. A retrospective RFA was conducted to determine the department and student reject rates for both departments before intervention. Emphasis was placed on exposure related reject films. ECs were compiled and introduced at Katutura State Hospital (venue B) by the researcher. The students were instructed to use these charts. At Windhoek Central Hospital (venue A) no ECs were used. A prospective RFA was conducted to establish department and student reject rates at both hospitals after the intervention at venue B. Results: During the retrospective phase the department reject rate for venue A was 21 percent while the student reject rate was 23 percent. At venue B 24 percent and 26 percent were scored respectively. Students at venue A produced rejected radiographs due to overexposure (49 percent) and underexposure (23 percent), whilst 37 percent was recorded for both causes at venue B. At venue A, 35 percent of films were rejected due to incorrect mAs selection, at venue B the figure was 42 percent. Undiagnostic radiographs due to inaccurate kV selection comprised 62 percent for venue A and 59 percent for venue B. During the prospective phase the department reject rate for venue A was 20 percent and that of the students was 19 percent. For venue B 12 percent and 11 percent were scored respectively. At venue A radiographs rejected due to over and underexposure were 43 percent and 33 percent respectively while those at venue B were 33 percent and 34 percent. Incorrect mAs selection caused 33 percent of discarded films at venue A and 38 percent at venue B. The figures for inaccurate kV selection were 68 percent and 62 percent for venues A and B. Conclusions: The introduction and use of ECs lowered the student reject rate at venue B in the prospective phase.

Undiagnostic radiographs due to inaccurate kV selection comprised 62 percent for venue A and 59 percent for venue B. During the prospective phase the department reject rate for venue A was 20 percent and that of the students was 19 percent. For venue B 12 percent and 11 percent were scored respectively. At venue A radiographs rejected due to over and underexposure were 43 percent and 33 percent respectively while those at venue B were 33 percent and 34 percent. Incorrect mAs selection caused 33 percent of discarded films at venue A and 38 percent at venue B. The figures for inaccurate kV selection were 68 percent and 62 percent for venues A and B. Conclusions: The introduction and use of ECs lowered the student reject rate at venue B in the prospective phase.

The sample consisted of 53 participants at a coeducational high school in the Nelson Mandela Metropole. The demographic data of the participants was gathered through the administration of a biographical questionnaire. The participants’ exposure to violence was explored by using the Harvard Trauma Questionnaire- South African Adaptation (HTQ-SAA), while their moral development was assessed by means of the Moral Judgment Test (MJT). The data was analysed using descriptive statistics and both the Pearson’s correlation coefficient and Chi square test was computed to draw inferences regarding the relationship between exposure to violence and moral development. The results of the study revealed that all of the participants were exposed to violence directly or indirectly on one or more occasion; however the frequency with which the participants were exposed to violence was lower than expected. The results of the MJT further indicated very low to low moral judgement competence scores for the majority of the sample. No significant correlation or association was found between the two constructs.

- Description: This research aimed to trace the development of psychologists as therapists within a South African context. Two distinct career levels were explored in relation to a competency model, a skills development model, and a developmental phase model of psychotherapists' professional development. Through purposive sampling five intern psychologists and six registered psychologists with at least seven years post-qualification client contact were selected according to the inclusion criteria of the study. Through semi-structured interviews, couched in the social constructionist position, the participants' experiences were thematically analysed. In addition, the researcher's own personal journey of developing as a psychotherapist was reflected upon. The current research study found that a number of aspects fostered the development of psychotherapists. Across the two career levels the life experiences of participants particularly featured events that were personally wounding or placed the participant in the helper role. The theoretical orientation(s) of participants were voiced as influential in their development, yet the ability to adjust their theoretical orientation to clients' needs was of overriding importance. Participants emphasised forming a strong therapeutic relationship with clients as the centrepiece of psychotherapy and depicted the therapeutic process using diverse metaphors. No key differences in use of metaphors emerged between intern and established psychologists. Catalysts for professional growth included personal therapy, "unforgettable cases", group supervision and, in the case of established psychologists, continuing professional development workshops. xiii Shifts in competency were recognised by intern psychologists as they gained more experience, namely, dissolution of naivety, a decrease in anxiety regarding their clinical work, a greater flexibility in the therapeutic process, a decreased sense of inappropriate responsibility for clients' progress, and an ongoing process of negotiating the interface of their personal and professional lives. Established psychologists spoke of having gained more confidence through their years of practice and yet experienced moments of anxiety which they found fostered humility. Limitations of the study and potential future research directions were outlined.

- Description: In 2002 there were an estimated 1.5 million people admitted into casualty emergency units within the Republic of South Africa. Many of these admissions dealt with events that were traumatic for both families and practitioners and these traumatic events have effects on the biological, psychological and social wellbeing of these individuals. Coupled with this is an ever increasing demand for mental health services and a worldwide shortage of qualified individuals to provide these services. Registered nurses in casualty units deal with the majority of these issues and the effect of these traumatic events. The crisis intervention model may provide these registered nurses with the necessary skills to deal with these problems not only for patients but possibly for themselves. Furthermore the biopsychosocial model of health allows these registered nurses to assess the impact of these events on the individuals. Yet, some registered nurses feel that they lack the necessary skills to deal with and assess these problems and intervene in these crisis situations. This study was exploratory descriptive in nature and aimed to examine whether there was a perceived need for registered casualty unit nurses to engage in supplementary counselling skills training. These perceptions were obtained through purposively sampled interviews and analysed qualitatively, using Tesch’s model of content analysis. Findings indicated that the registered nurses do perceive a need for supplementary counselling training, both for use with the patients and for themselves, and are willing to engage in this training although there are problems that inhibit this willingness. Recommendations regarding the implementation of a supplementary counselling skills training course as well as future research in the field were made.

A non-probability purposive sampling method was used to select eight participants from Komani Hospital in Queenstown. Data was collected through semi-structured interviews. The services of an Independent Interviewer were used to avoid any bias as interviews took place where the researcher is employed. All interviews were transcribed verbatim and the data collected was analyzed according to Tesch’s eight steps of data analysis as described in Cresswell (1994:155). The researcher utilized services of an Independent Coder who verified the identified major themes. Four major themes emerged from the analysis of the interview: Mental health care practitioner’s perceptions of mental illness, perception of the causes of mental illness within the isiXhosa cultural context, mental health care practitioners’ views in the management and treatment of mental illness and suggestions put forward to improve the services to mental health care users. The major findings of this study were the lack of knowledge of culture of mental health care users. It is recommended that it would be proper if there could be co-operation between mental health care practitioners and traditional healers by working together as a team.

- Description: While career development is viewed as a lifelong process, there are numerous limitations regarding existing career theories and research pertaining to the adolescent population. Further, insufficient longitudinal research represents one of the major obstacles for a more holistic understanding of career development across the lifespan. Thus, the present longitudinal project of which the current study forms a part was initiated to address the lack of research and theory concerning adolescent career development. The larger longitudinal project intended to make information available regarding the occupational aspirations and perceptions of a sample of South African children and adolescents, from early childhood into their adolescent years. The current study investigates the occupational aspiration development of 15 to 16 year old South African adolescents. It is argued that the South African knowledge base on career development behaviour still requires extensive development, especially with regards to adolescent career development. The current longitudinal study aimed to explore and describe possible developments and changes over a two year period in the interest typology, occupational status level and occupational gender stereotypes of the 43 adolescents. One of the objectives was to empower rural communities for purposes of improving their daily living standards and also to bring about social change.In determining the sample size, forty (40) respondents were selected from Mqanduli Administrative Area. Out of the forty (40) respondents, thirty (30) respondents were selected from the Mqanduli community members, five (5) were O.R.Tambo District Municipality stakeholders and the other five (5) were Social workers from Mqanduli Administrative Area. The instrument utilized to collect data was a questionnaire. Data was analysed using the quantitative and qualitative measure. The outcome of the study indicates that the majority of rural communities in the Mqanduli Administrative Area were in dire need of empowerment which will enable these disadvantaged communities towards development and social change.

- Description: Bacterial resistance to antibiotics has been a great problem for many years. The degree of resistance and the speed with which resistance develops varies with different organisms and different drugs. Enzymes called β-lactamases are produced by bacteria and are one mechanism in which bacteria develop antimicrobial resistance. Gram-negative bacteria producing enzymes called ESBLs because of their wide substrate range are of a particular concern in nosocomial infections. In many countries people still use traditional medicine derived from plants as an alternative to the Western medicine due to increased cost of Western medicine and microbial resistance of antibiotic treatments. Biologically active compounds isolated from plants species are used in herbal medicine. Because of the high prevalence of the ESBLs and their increasing resistance to the antibiotics, this research study was done to test the antimicrobial activities of selected medicinal plants of the Eastern Cape; G. incanum, D. angustifolia and E. autumnalis which were traditionally used to treat various infections. The in vitro antimicrobial activity of three different extracts (acetone, methanol & distilled water) and the traditional preparations of the three plants were tested against the selected strains of ESBL-producing bacteria, non β-lactamase producers and the different fungal species. The extracts were screened against 26 Gram-positive bacterial strains, 53 Gram-negative bacterial strains and 15 fungal strains. The Gram-positive bacteria included strains from S. aureus, B. cereus and E. faecalis. The Gram-negative bacteria included strains from E. ii coli, E. cloacae, K. pneumoniae, P. aeruginosa and Acinetobacter spp. The fungal strains included 9 strains of Candida albicans and a single strain of each of the following opportunistic fungi, Mucor sp, Geotrichium sp, Penicillium sp, Fusarium sp and Rhizopus sp. The agar dilution assay was used for the antimicrobial screening of the plants extracts and for the determination of the MICs. The Ames test was performed for the determination of probable carcinogenicity of the extracts of G. incanum and D. angustifolia. The distilled water extracts followed by acetone extracts of the plants revealed the highest antimicrobial activity against the different microbial strains. The extracts of G. incanum followed by the extracts of D. angustifolia inhibited the highest number of microbial strains. The extracts of E. autumnalis did not show any antimicrobial activity against all the pathogens in this study. More of the Gram-positive bacteria were inhibited by the plant extracts. The lowest MIC was obtained with Gram-positive bacteria. The bacterial strains of E. faecalis and P. aeruginosa were not inhibited by any of the plants extracts in the agar dilution assay yet Acinetobacter species which are MDR were inhibited by the distilled water and methanol extracts of G. incanum. A single strain of Mucor sp was the only spore forming fungi that was inhibited by the distilled water extracts of G. incanum. None of the plants extracts showed any mutagenic effects on the TA100 S. typhimurium strains incorporated on the Ames test. Apart from revealing of new antimicrobial agents that may be used against resistant organisms, the proper use of antimicrobial agents should be recommended. The study has highlighted a need for further investigations on the properties of the medicinal plants used in this study.

- Description: South Africa, with an estimated 5.7 million people living with HIV, continues to have one of the largest epidemics in the world. The introduction of HAART resulted in prolonged and improved quality of life of many infected patients. However, adverse effects caused by these drugs have become a major concern as they affect the adherence of patients and in some cases even result in the death of patients. Although much research has been and is still being conducted in the area of understanding, preventing and management of ARV adverse effects, there is still a need for patients to be actively involved in self-monitoring for adverse effects. This will assist health care professionals in early identification of serious or potentially serious ARV effects. This study aimed at evaluating the usefulness of strategies developed and employed in the identification, recording and monitoring of adverse effects. The study was conducted with patients receiving HAART from a private HIV and AIDS clinic in Uitenhage, Eastern Cape, South Africa. The research project was approved by the Nelson Mandela Metropolitan University Research and Ethics Committee and the research site. This was an experimental, randomized controlled study carried out over a period of three months (August to October 2009), with a sample size of 160 patients divided into four study groups of 40 patients each. Two monitoring strategies, namely an ARV adverse effect monitoring tool and a patient self-monitoring diary were developed and used for the identification and recording of adverse effects. The four study groups included a Control group, a Tool group, a Diary group and a Tool-Diary group. Willing patients, after signing an informed consent form, were randomly assigned to one of the four groups by participating health care workers at the study site. Data was retrieved from the patient files by the researcher. Descriptive statistical analysis of the findings of the study was conducted using SPSS®. One hundred and forty nine patients were included in the final data analysis. Of the 80 diaries handed out to patients, only 33 were returned and due to errors only 31 were suitable for analysis. Monitoring tools were completed and analysed for 36 patients. The tool was found to be more effective in identifying adverse effects of a physical nature (such as peripheral neuropathy and lipodystrophy) than the usual methods of monitoring employed by the clinic, whilst the diary, used alone, was found to be less effective. Use of the tool and diary combined resulted in the most significant identification and recording of central nervous system related adverse effects and physical adverse effects. However due to the low return rate of the diaries and the majority of the monitoring tool not being completed in many instances the results of this study may not be generalisable. The study results did however suggest that combining the tool and the diary methods of adverse effect identification, yielded the most favourable results when compared to each method alone. This may be attributed to the fact that the tool is useful in identifying objective symptoms and the diaries subjective symptoms, particularly in instances where the patients forget to report their symptoms to healthcare professional whilst at the clinic. The diaries were also reported to improve adherence for more than 90 percentage (n=31) of the patients. More research would be needed in order to verify the exact significance of the tool and the diary in identifying and recording adverse effects and symptoms of adverse effects.
