A journey with an abused child : a non-directive play therapy perspective

- Description: The extensive amount of research conducted in the field of family violence internationally indicates that child abuse has a detrimental effect on both the physical and emotional development of children as well as having a profound effect on an individual’s psychological development and functioning in adult life. The aim of this research study was to describe the therapeutic process that unfolded with a seven year old allegedly abused female client within the framework of non-directive play therapy. The case was further contextualised utilising Erik Erikson’s theory of psychosocial development. This study served to broaden the body of psychotherapeutic knowledge by means of meaningful qualitative enquiry. The client was referred for therapy because of severe emotional and behavioural problems following the alleged sexual abuse. The client was seen over a period of eight months and this included 11 sessions of non-directive play therapy, three parent interviews and psychometric assessments conducted by a colleague. The case study method was utilised in this study. To achieve the aim of the research, the methodology of choice was the descriptive dialogic case study. A purposive sampling technique was used in the selection of the research subject for this study. The data collection and analysis were conducted according to Yin’s (1994) analytical generalisation, which consists of two main strategies: (a) using a theoretical framework as a guide to determine what data is relevant; and (b) developing a matrix as a descriptive framework for organising and integrating the data. Furthermore, the process of data analysis was aided by the use of guidelines proposed by Irving Alexander (1988) with Axline’s non-directive play therapy and Erikson’s theory of psychosocial development as the theoretical frameworks. The findings of this study suggest that plotting the play therapy sessions according to the framework of the four stages of play therapy was a particularly useful tool to monitor Michelle’s progression through the therapeutic process. This can be seen as a valuable application of a tool which can be used within the non-directive play therapy approach. Contextualising Michelle’s development according to the stages of Erikson’s theory of psychosocial development was also found to be a valuable endeavour. According to her chronological age, Michelle should have been in the fourth stage of industry versus inferiority, but in reality Michelle was still struggling to strike a healthy balance between the terms of conflict of trust versus mistrust issues of the first stage. From a therapeutic point, this was an important exercise as it helped to inform the therapist and consequently, the therapeutic process. This research undertaking can be recognised as a positive demonstration of the value of non-directive play therapy (Virginia Axline) and Erik Erikson’s theory of psychosocial development in the therapeutic process. In addition, this study has served to facilitate a more holistic understanding of the case study approach to research. Recommendations regarding future research undertakings that utilise the case study approach and methodology have been made. Design Expert 8 was used for the purpose of finding the combination of independent variables that would yield an optimal formulation for each microbe and the three microbes in combination. The model selected by the software managed to reasonably correlate the predicted models to the experimental data. Encapsulation of carboxyfluorescien (CF) into a liposome formulation enhanced its uptake by Staphylococcus aureus and Escherichia coli as well as Candida albicans. This was evident in the increase in CF uptake when the uptake rate of free CF was compared with that of liposomal CF.

- Description: The shoulder is a complex anatomical structure and imaging plays an important role in the diagnosis and management of shoulder conditions. The complexity of the shoulder’s anatomy has led to the development of multiple radiographic projections and techniques within plain film imaging with each projection intended to demonstrate specific aspects of the anatomy of the shoulder. However, reproducing the required projections can be difficult especially if radiographers are not familiar with the projections and their evaluation criteria. Literature has revealed the importance of a comprehensive knowledge and understanding of anatomy, patient positioning, beam direction and centring point, and evaluation criteria to ensure a quality projection for accurate diagnoses. The aim of the study was to determine the knowledge and understanding of radiographers with regard to the supraspinatus outlet projection (SOP) for shoulder impingement syndrome (SIS) and its evaluation criteria. The inferences derived from the research findings were used to develop guidelines for a structured in-service training programme for practising radiographers to optimise their knowledge and understanding of the supraspinatus outlet projection in shoulder impingement syndrome. The proposed study followed a quantitative approach. Furthermore, a descriptive, exploratory, contextual design was employed. The research population consisted of practising radiographers working in the public and private hospitals of the Nelson Mandela Bay Municipality. The data were collected by means of a structured self-administered questionnaire. The questionnaire comprised of three sections. The first section requested demographic information from the participants. The second section assessed their knowledge and understanding regarding the scapular ‘Y’ and the supraspinatus outlet projections and shoulder impingement syndrome. The third section assessed their knowledge and understanding of anatomy and image evaluation/critiquing. The reliability and validity of the data collection instrument was ensured by conducting a pilot study and comparing the results with those of the main study. In addition, the expertise and guidance of a radiographer experienced in the clinical training of radiographers, the supervisor (who has twenty years’ experience in the teaching of radiographers) and a statistician was obtained. Descriptive and inferential statistical analyses were performed by means of a statistical programme and with the guidance of a statistician. The researcher ensured that the study was conducted in an ethical manner by adhering to the ethical principles of beneficence, justice and respect for persons.

HIV (Human Immunodeficiency Virus) is the virus that is believed to cause AIDS. AIDS (Acquired Immune Deficiency Syndrome) is the collection of illnesses or symptoms that ultimately results in death. Antiretroviral (ARVs) drugs or Highly Active Antiretroviral Therapy (HAART) is the treatment that has been applied to combat the HI virus in a bid to slow down the progression of AIDS and ultimately prolong the life of the infected individual. The study aimed to explore and describe the factors contributing to adherence and non-adherence to ARVs in individuals on treatment. A sample of 81 individuals who have been on ARV and HAART treatment for six months or more was used. The methodology used was exploratory-descriptive and the data obtained was quantitative in nature. A biographical questionnaire and questionnaire with questions aimed at ascertaining the possible factors that contribute to individuals either adhering to or defaulting on their treatment, such as side effects and cost of treatment, was administered. The data obtained was analysed by means of descriptive statistics and frequency counts. The results of the study showed that the sample had a fairly high level of adherence. The factors that could undermine adherence were identified as lack of support, as familial and health provider support acts as a motivator to adhere; substance abuse as it can lead to forgetting to take treatment; unemployment and poverty, as these can lead to an inability to return for follow up clinic visits or failure to have food to take with the pills; and the type of treatment regimen whereby the more complex the treatment is the more likely it is that adherence will be difficult to maintain. Suggestions were made as to future research involving antiretroviral therapy (ART). Finally the limitations as well as the value of the research were outlined.

In South Africa, spirit possession experiences are not uncommon; in some instances, culturally, they can be expressed as ‘amafufunyana’, while religiously, the term demonic/spirit possession holds true. The symptoms of a spirit possession state, should be of interest to psychology, given the similarity of its symptoms with those that are often typical of a psychological diagnosis, such as dissociation, hallucinations and delusions. While much attention has been given to patient experiences of spirit possession, little attention has been focused on the cultural competence required in working with patients with diverse worldviews. This study explores psychologists perceived cultural competence in working with patients who present with the experience of being possessed by a spirit. This exploration becomes more interesting, given that little to no attention in the literature has focused on the role and influence of psychologists religious and cultural belief systems and how these may influence (if at all), the understanding, diagnosis and treatment of a patient who believes that they are possessed. The researcher employed a qualitative method and design. Participants were recruited through use of a purposive sampling technique. Twelve psychologists agreed to participate in the study and thematic analysis was used to analyze the data, by following a data analysis method by Braun and Clarke (2006). Four themes were identified namely, 1) Psychologists’ understanding and treatment of dissociative disorders/states, 2) Psychologists’ understanding of spirit possession; 3) Spirit possession experiences versus psychological disturbances; and 4) Cultural acknowledgement versus cultural considerations-implications for cultural competence. An intervention for high-risk type 2 diabetic clients during preconception and internatal in re-engineering of primary health care

- Description: The South African disease profile has changed significantly and has increasing prevalence of overweight and obesity in relation to body mass index. Type 2 diabetes mellitus is greatly associated with increased body mass index and poses a great health concern for women of childbearing age. Complications of type 2 diabetes mellitus in pregnancy has life-threatening for the mother and baby. South African women have a tendency of avoiding and neglecting routine screening before pregnancy, that is part of preconception care or internatal care. The aim of the study sought to identify women of childbearing age with predisposing factors of type 2 diabetes, in order to intervene, using a lifestyle modification to delay and prevent the occurrence of type 2 diabetes. A quantitative intervention study using parallel group randomized control trials was used. One hundred and forty-six women of childbearing age (18-45 years) were recruited and assigned to either control (n=73 + 9) or experimental group (n=73 +9). using a computer software randomizer application to avoid bias in selection of participants by the researcher. Both groups received health education from the researcher and the dietician from hospital. The control group had no intervention done, whereas the experimental group attended physical activity sessions which were administered by a hired lay coach. The findings showed that both groups are at risk of developing type 2 diabetes although they both groups lose weight. The weight loss of the control group was owed to education during recruitment. Vegetable consumption was minimal to the younger age group than the older age groups. Waist circumference above 105cm consumed less vegetables, a higher waist circumference is associated with insulin resistance. There was also minimal exercises and knowledge about the preconception care. Diabetes prevention is vital for the women of childbearing age as it affects both mother and child. The study recommends lifestyle modification (exercise and healthy diet) to prevent and delay of type 2 diabetes and gestational diabetes mellitus. The study also recommends Preconception care and Re- engineering of Primary health care uses community health care workers for screening high risk women during home visits.

- Description: The South African disease profile has changed significantly and has increasing prevalence of overweight and obesity in relation to body mass index. Type 2 diabetes mellitus is greatly associated with increased body mass index and poses a great health concern for women of childbearing age. Complications of type 2 diabetes mellitus in pregnancy has life-threatening for the mother and baby. South African women have a tendency of avoiding and neglecting routine screening before pregnancy, that is part of preconception care or internatal care. The aim of the study sought to identify women of childbearing age with predisposing factors of type 2 diabetes, in order to intervene, using a lifestyle modification to delay and prevent the occurrence of type 2 diabetes. A quantitative intervention study using parallel group randomized control trials was used. One hundred and forty-six women of childbearing age (18-45 years) were recruited and assigned to either control (n=73 + 9) or experimental group (n=73 +9). using a computer software randomizer application to avoid bias in selection of participants by the researcher. Both groups received health education from the researcher and the dietician from hospital. The control group had no intervention done, whereas the experimental group attended physical activity sessions which were administered by a hired lay coach. The findings showed that both groups are at risk of developing type 2 diabetes although they both groups lose weight. The weight loss of the control group was owed to education during recruitment. Vegetable consumption was minimal to the younger age group than the older age groups. Waist circumference above 105cm consumed less vegetables, a higher waist circumference is associated with insulin resistance. There was also minimal exercises and knowledge about the preconception care. Diabetes prevention is vital for the women of childbearing age as it affects both mother and child. The study recommends lifestyle modification (exercise and healthy diet) to prevent and delay of type 2 diabetes and gestational diabetes mellitus. The study also recommends Preconception care and Re- engineering of Primary health care uses community health care workers for screening high risk women during home visits.

The absence of haemoglucotest (HGT machines at clinics was another contributing factor to diabetes distress. Most of the patients in the study felt that they would manage their diabetes better if the government supplied these machines. The results informed various recommendations that might help the management of OR Tambo Health District to make changes and thus minimise the diabetes distress reported by its patients. The objectives of the study were to: · Explore and describe postnatal women’s experiences of the PMTCT Programme offered in the East London Hospital Complex. · Propose recommendations into the existing guidelines for midwives who implement the PMTCT Programme in the health services to ensure optimal implementation of this programme. The research population included postnatal women who participated in the PMTCT of HIV Programme at the East London Hospital Complex (Frere site). Permission to conduct the research was obtained from the Eastern Cape Department of Health Ethics Committee, Chief Executive Officer of the hospital complex and the Advanced Degrees Committee and Human Ethics Committee at the Nelson Mandela Metropolitan University. The research study was qualitative, exploratory, descriptive and contextual in design. Data collection was carried out by face-to-face semi-structured interviews with postnatal women at the East London Hospital Complex (Frere site). The tape-recorded interviews were transcribed verbatim with the aid of field notes. Data analysis was done according to Tesch’s method of data analysis (in De Vos et al, 2000:343). Themes were identified from the transcriptions and finalised after consensus discussions with an independent coder who was experienced in qualitative research. Literature control, guided by the themes identified in the interviews, was carried out to compare and verify the findings of the study. Three major themes with sub-themes were identified during data analysis. The major themes were identified as follows: · The participants expressed feelings of being devastated by the results that confirmed their HIV positive status. · The participants expressed a thirst for knowledge on how to live with the diagnosis and on how to continue with the PMTCT Programme. · The physical environment where counselling and testing were done, as well as the practical arrangements, were not conducive to the full implementation of the PMTCT Programme at the antenatal clinic. Conclusions were drawn and recommendations were made in the form of additional guidelines for midwives implementing the PMTCT Programme in the antenatal clinic health services. Guidelines for further midwifery-related research were formulated.

- Description: Bullying appears to be a worldwide phenomenon. It occurs within schools, homes, and in the community too. Bullying is a form of aggressive behaviour. Female bullying is not so easily noticed, as girls hardly ever use physical forms of aggression. Bullying can have a detrimental effect on the mental health of the victim. The victims of bullying experience symptoms of mental discomfort, which include low self esteem, feelings of helplessness, feelings of worthlessness and inferiority, lack of confidence, isolation, self-conscientiousness; and lastly, this can lead to suicide. Some form of intervention is necessary to provide adolescents with skills to combat bullying and help them to become mentally healthy again. The overall goal of the study was to develop, implement and evaluate a psycho-educational programme for female adolescents in a secondary school setting, in order to assist them in coping with bullying. The objectives of the research study were to: Conduct asituational analysis to identify the mental health needs of adolescent girls, as victimsof bullying. Develop a psycho-educational programme to facilitate the promotion of mental health of those adolescent girls who are victims of bullying. Implement the psycho-educational programme to facilitate coping by adolescent girls who are victims of bullying. Assess whether the adolescent girls have benefitted from attending the psycho-educational programme. The researcher used a qualitative, explorative, descriptive and contextual design. The research methodology consisted of four phases. In phase one of the research, a situational analysis was done; and the characteristics of mental discomfort exhibited by the victim were identified. The needs of the adolescent girl who has been bullied have already been described. Phase two involved the development of a psycho-educational programme. The information from the situational analysis was used to develop the conceptual framework. The six concepts in the survey list of Dickoff et al. (1968:422) were described as follows: the recipient is the adolescent girl who has been bullied, the agent is an advanced psychiatric nurse, the context is the community and the secondary school where bullying takes place, the dynamics of the intervention constitute the mental discomfort experienced by the adolescent girl. This is what motivates her to participate in the programme. The procedure was identified as the psycho-educational programme, while the terminus or outcome of the intervention for the adolescent girl would be for her to experience mental health after being exposed to the psycho-educational programme. The relationship between the concepts was used to form the mind map of the conceptual framework. This guided the development of the psycho-educational programme. The content of the programme has already been described. In phase three the psycho-educational programme is implemented and in phase four the programme is evaluated. The psycho-educational programme taught the teenage girls skills and it provided them with knowledge to cope better with the bullying. Attending the programme made the teenagers aware that they needed to change to experience mental health and happiness. The empirical study took place in phase four. The data-gathering method in phase four included the conducting of semi-structured interviews with the adolescent girls who participated in the programme, as well as the teachers of these adolescent girls and their parents. Naïve sketches, reflective journals, observations made and field notes formed part of this database. The data was analyzed by means of Tesch‟s descriptive analysis (in Creswell, 2003:192). - Description: The primary purpose of CTG is to detect early signs of intrapartum hypoxia and improve foetal outcomes. Intrapartum hypoxia remains the major cause of perinatal deaths mainly in monitored labours. This is attributed to the midwives’ lack of skills in the foetal implementation and interpretation of CTG. Midwives interpret foetal heart rate patterns erroneously hence they fail to detect early signs of severe foetal compromise. Accurate interpretation of CTG is the cornerstone for clinical decision during intra-partum foetal surveillance. Therefore, it is mandatory that midwives are competent in CTG. The purpose of the study was to explore and describe the perceptions of midwives in KwaZulu-Natal public hospitals regarding their level of competence related to the CTG. The purpose of the study was to explore and describe the perceptions of midwives in KwaZulu-Natal public hospitals regarding their level of competence related to the CTG. The objectives of this study were: -to explore and describe the perceptions of the midwives regarding their level of competence relating to cardiotocography. -to establish the levels of competence of midwives in relation to CTG. -to develop clinical guidelines that could be utilised by midwives in the implementation and interpretation of the cardiotocograph. The study utilised a quantitative non-experimental comparative descriptive research design. A self-developed and self-administered questionnaire was developed under the supervision of a statistician and was to collect data from a purposively selected sample of midwives who work in labour wards in Kwa-Zulu Natal public hospitals. A sample of 226 practicing midwives working in labour wards of the nine KZN public hospitals was selected using a non-probability convenience sampling method; only 125 responded. Data were collected on an excel spread sheet and analysed under the supervision of the statistician and research supervisor. The researcher assured of validity and reliability of the data collection method and data as well as of the data collection instrument. The findings revealed that the midwives in KZN public hospitals perceived themselves as being competent regarding CTG tasks; however they lack knowledge and skills in the implementation and interpretation of CTG. It was also evident that the midwives’ years of midwifery clinical experience, labour ward experience, possession of additional post basic midwifery qualification and the perceived level of competence related to CTG tasks number of years of the midwives did not influence their level of competence in the implementation and interpretation of CTG. Literature control supported these findings and there was consistency with that literature. Recommendations were made and clinical guidelines were developed to assist midwives with the assessment of foetal heart rate and the interpretation of the cardiotocograph results.

It was also evident that the midwives’ years of midwifery clinical experience, labour ward experience, possession of additional post basic midwifery qualification and the perceived level of competence related to CTG tasks number of years of the midwives did not influence their level of competence in the implementation and interpretation of CTG. Literature control supported these findings and there was consistency with that literature. Recommendations were made and clinical guidelines were developed to assist midwives with the assessment of foetal heart rate and the interpretation of the cardiotocograph results.

A conceptual framework that is the Adlerian Matrix was included in the study as guide in presenting the research discussion and findings in an illuminating and integrative manner. The research discussion and findings suggested that Adler’s theoretical framework considers the biopsychosocial, cultural and historic influences in Martin Luther King junior’s personality development. In taking the context into consideration, Adler’s individual psychology identified King junior with a social useful lifestyle because of his high social interest which influenced his striving for superiority. This lifestyle guided his fictional goal which was selfless and aimed at contributing to the wellbeing of others. - Description: The fallibility of human judgment in the making of decisions requires the use of tests to enhance decision-making processes. Although testing is surrounded with issues of bias and fairness, it remains the best means of facilitating decisions over more subjective alternatives. As a country in transition, all facets of South African society are being transformed. The changes taking place within the tertiary education system to redress the legacy of Apartheid, coincide with an international trend of transforming higher education. One important area that is being transformed relates to university entrance requirements and admissions procedures. In South Africa, these were traditionally based on matriculation performance, which has been found to be a more variable predictor of academic success for historically disadvantaged students. Alternative or revised admissions procedures have been implemented at universities throughout the country, in conjunction with academic development programmes. However, it is argued in this dissertation that a paradigm shift is necessary to conceptualise admissions and placement assessment in a developmentally oriented way. Furthermore, it is motivated that it is important to keep abreast of advances in theory, such as item response theory (IRT) and technology, such as computerized adaptive testing (CAT), in test development to enhance the effectiveness of selecting and placing learners in tertiary programmes. This study focuses on investigating the use of the Accuplacer Computerized Placement Tests (CPTs), an adaptive test battery that was developed in the USA, to facilitate unbiased and fair admissions, placement and development decisions in the transforming South African context. The battery has been implemented at a university in the Eastern Cape and its usefulness was investigated for 193 participants, divided into two groups of degree programmes, depending on whether or not admission to the degree required mathematics as a matriculation subject. Mathematics based degree programme learners (n = 125) wrote three and non-mathematics based degree programme learners (n = 68) wrote two tests of the Accuplacer test battery. Correlations were computed between the Accuplacer scores and matriculation performance, and between the Accuplacer scores, matriculation performance and academic results. All yielded significant positive relationships excepting for the one subtest of the Accuplacer with academic performance for the non-mathematics based degree group. Multiple correlations for both groups indicated that the Accuplacer scores and matriculation results contribute unique information about academic performance. Cluster analysis for both groups yielded three underlying patterns of performance in the data sets. An attempt was made to validate the cluster groups internally through a MANOVA and single-factor ANOVAs. It was found that Accuplacer subtests and matriculation results do discriminate to an extent among clusters of learners in both groups of degree programmes investigated. Clusters were described in terms of demographic information and it was determined that the factors of culture and home language and how they relate to cluster group membership need further investigation. The main suggestion flowing from these findings is that an attempt be made to confirm the results with a larger sample size and for different cultural and language groups.

Sport represents an optimal means of enabling children and adolescents to meet their daily requirements for physical activity to maintain health. Conversely, despite all of the widely acknowledged benefits, participation in sport among children and adolescents continues to decline throughout the world. Although a great deal of research has been conducted to identify barriers to participation in sport, most of it has concerned poor, rural, disadvantaged, or other marginalised groups. As a consequence, a large proportion of the research has attributed low rates of participation in sport to practical barriers, which usually take the forms of either logistical problems or a lack of resources. The aim of this study was to identify the barriers to participation in sport among adolescents in an affluent, urban setting, at an international school in Cape Town, South Africa. The Barriers to Sport Participation Questionnaire was used to identify the social, personal, and practical barriers that affected 107 adolescents who were enrolled in the school. The data was analysed by means of the SPSS (IBM, Version 27) software package and seven significant barriers were identified. Fear of academic failure, environmental constraints, and lack of interest were identified as the primary barriers, thereby emphasising the significant influence of personal barriers, rather than practical ones. It was also found that the influence of particular barriers was greater on females than their male counterparts and that the respondents who either did not participate in sport themselves or were from households in which other members did not do so were more adversely affected by some barriers than the others. Significant correlations were found between some grades and certain barriers, the most notable being those between respondents in Grades 7, 9, and 10 and the fear of academic failure barrier. In light of these findings, it could be concluded that the global decline in participation in sport among the youth is a complex problem and that particular barriers, which might not have been identified in previous research, could be particularly influential in affluent environments. As it is evident that as barriers can be specific to particular groups and settings, the optimal means of overcoming them is unlikely to be found through the adoption of a one-size-fitsall approach. The newly qualified professional nurses may face difficulties in the hospital facilities when it comes to the execution of duties. The newly qualified professional nurses are expected to display knowledge, skills and to have a positive effect on patients, the relatives of patients and their colleagues to alleviate diseases. This challenge may create emotional distress if newly qualified professional nurses receive inadequate guidance from the senior staff members of the hospital facility. The objectives of this study were to explore and describe the clinical experiences of newly qualified professional nurses, to explore how the newly qualified professional nurses were affected emotionally by clinical experiences and to explore the support system to enhance the performance of the newly qualified professional nurses. A qualitative, explorative, descriptive and contextual design was used to explore and describe the experiences of the newly qualified professional nurses in hospital facilities of the Alfred Nzo District Municipality in the Eastern Cape. A purposive sampling of newly qualified professional nurses, with less than two years of experience after completing the community service in the clinical practice, was used. The data were collected through face-to-face and semi-structured interviews. The interviews were conducted until the data were saturated. A pilot study was done before the main study to prove the adequacy and suitability of the methods to conduct the main study. The participants‟ interviews and results of the pilot study were incorporated into the interviews and results of the main study. Tesch‟s method of thematic analysis and an independent coder were used to analyse the data and to draw meaning from the content. The study used Guba and Lincoln‟s criteria of measures to ensure trustworthiness, which included credibility, confirmability, dependability and transferability. Ethical standards were maintained throughout the study as the researcher complied with ethical principles, namely, respect for persons, beneficence, justice and autonomy. The key considerations to curb the spread of the Covid-19 pandemic, as stipulated by the World Health Organisation, were followed during the data collection. On data interpretation, three themes were developed with eight subthemes on the first theme, two subthemes on the second theme and three subthemes on the third theme. The research findings revealed that the participants experienced negative attitudes from colleagues and multidisciplinary team members. The shortage of staff and resources made participants feel ineffective in their work environment. Clinical exposure to different units and procedures in various units added value to the development of the participants. The recommendations were made with regards to three divisions. They were the hospital facilities‟ personnel, future nursing research personnel and nursing schools personnel. In hospital facilities, the recommendations included that all novice professional nurses should be made aware of the emotional impact at work. An annual plan for workshops and in-service training should be implemented. Professional counselling is required for the novice professionals who were exposed to Covid-19 isolation units. A need to increase staff coverage in the clinical units was identified as the workload is too big. Management is encouraged to be more sensitive about the newly employed nurses‟ challenges in their facilities and the most experienced nurses are encouraged to mentor and coach the novice nurses. In the future nursing research, recommendations included that more studies need to be conducted in South Africa and the Eastern Cape province to verify the findings of this research. A specific research study that will focus on the supportive needs of nurses in clinical facilities should be done institutionally to provide evidence-based practice. The research culture on the nurses‟ experiences in clinical facilities needs to become a norm. - Description: The study aimed to explore and describe the clinical experiences of newly qualified professional nurses in hospital facilities of the Alfred Nzo District Municipality in the Eastern Cape. The newly qualified professional nurses may face difficulties in the hospital facilities when it comes to the execution of duties. The newly qualified professional nurses are expected to display knowledge, skills and to have a positive effect on patients, the relatives of patients and their colleagues to alleviate diseases. This challenge may create emotional distress if newly qualified professional nurses receive inadequate guidance from the senior staff members of the hospital facility. The objectives of this study were to explore and describe the clinical experiences of newly qualified professional nurses, to explore how the newly qualified professional nurses were affected emotionally by clinical experiences and to explore the support system to enhance the performance of the newly qualified professional nurses. A qualitative, explorative, descriptive and contextual design was used to explore and describe the experiences of the newly qualified professional nurses in hospital facilities of the Alfred Nzo District Municipality in the Eastern Cape. A purposive sampling of newly qualified professional nurses, with less than two years of experience after completing the community service in the clinical practice, was used. The data were collected through face-to-face and semi-structured interviews. The interviews were conducted until the data were saturated. A pilot study was done before the main study to prove the adequacy and suitability of the methods to conduct the main study. The participants‟ interviews and results of the pilot study were incorporated into the interviews and results of the main study. Tesch‟s method of thematic analysis and an independent coder were used to analyse the data and to draw meaning from the content. The study used Guba and Lincoln‟s criteria of measures to ensure trustworthiness, which included credibility, confirmability, dependability and transferability. Ethical standards were maintained throughout the study as the researcher complied with ethical principles, namely, respect for persons, beneficence, justice and autonomy. The key considerations to curb the spread of the Covid-19 pandemic, as stipulated by the World Health Organisation, were followed during the data collection. On data interpretation, three themes were developed with eight subthemes on the first theme, two subthemes on the second theme and three subthemes on the third theme. The research findings revealed that the participants experienced negative attitudes from colleagues and multidisciplinary team members. The shortage of staff and resources made participants feel ineffective in their work environment. Clinical exposure to different units and procedures in various units added value to the development of the participants. The recommendations were made with regards to three divisions. They were the hospital facilities‟ personnel, future nursing research personnel and nursing schools personnel. In hospital facilities, the recommendations included that all novice professional nurses should be made aware of the emotional impact at work. An annual plan for workshops and in-service training should be implemented. Professional counselling is required for the novice professionals who were exposed to Covid-19 isolation units. A need to increase staff coverage in the clinical units was identified as the workload is too big. Management is encouraged to be more sensitive about the newly employed nurses‟ challenges in their facilities and the most experienced nurses are encouraged to mentor and coach the novice nurses. In the future nursing research, recommendations included that more studies need to be conducted in South Africa and the Eastern Cape province to verify the findings of this research. A specific research study that will focus on the supportive needs of nurses in clinical facilities should be done institutionally to provide evidence-based practice. The research culture on the nurses‟ experiences in clinical facilities needs to become a norm. This means that about one out of every ten babies born alive weighed less than 2500g. Furthermore, six of South Africa’s nine provinces, including the Eastern Cape, reported low birth weight rates equal or higher than the national average. These figures raise serious concerns about the health status of infants, their chances of survival and their quality of life, particularly in provinces with a high incidence of low birth weight. Literature has linked the maternal health status to adverse birth outcomes. Statistics from the district office of the Nelson Mandela Bay Health District (NMBHD) indicates that for the fourth quarter of 2015, between 16.65 and 20.9% low birth weight infants were born. However, limited information is available regarding the causes and maternal health status of the mothers of the infants born with adverse birth outcomes in the Nelson Mandela Bay Health District (NMBHD). The objective of the research study is to investigate the associations between maternal health status and birth outcomes in order to identify the major drivers of adverse birth outcomes in NMBHD. The study used a quantitative research approach. In order to enhance the design, the researcher used an explorative, descriptive, cross-sectional, contextual and survey research design. The study was conducted at the regional hospital in Nelson Mandela Bay Health District (NMBHD) and Midwifery Obstetric Units (MOU). The participants were selected using a convenient and purposive sampling technique. A structured, self-administered questionnaire was used as the data collection tool. A statistician assisted with the data analysis. Descriptive and inferential statistics were used. The researcher ensured that ethical considerations were maintained throughout the study to protect the participants. Reliability and validity were also ensured throughout the study. The total sample of the study was 207 and the mean age of the participants was 26,9 years. Hypertension and HIV were the conditions most diagnosed prior to, and during, pregnancy. Most of the delivered infants were females. The findings of the study revealed a significant association between maternal diabetes, maternal hypertension and the infants’ birth weight. Additional findings iv revealed that independent of gestational age, mothers with hypertension are likely to deliver low birth weight (LBW) infants. Antenatal care is of the utmost importance during pregnancy and special attention should be given to the management of hypertension. The researcher developed recommendations for primary health care (PHC) nurses in antenatal clinics (ANC) to address the management of the major maternal drivers of LBW infants in order to decrease and prevent adverse birth outcomes.

Lind (2002:18) explains that global trends continue to show that the most severe work-related health problems that exist amongst computer users are musculoskeletal disorders. As technology has lead to increases in automation, so it has lead to increases in work-related illnesses. Although studies have explored the effects of ergonomics (Thibodeau, 1995:322) in static working positions there has been little evidence supporting a solution in overcoming poor occupational postures. Many health practitioners argue that occupationally caused, or aggravated, musculoskeletal disorders are steadily increasing. Thus, even though computers have improved productivity and made work easier for the population in general, they have adverse effects as well. Designing the proper tools or a setup of the work place is of prime importance for the elimination of chronic diseases attributed to sedentary lifestyles. Regular physical activity had long been regarded as an important component of a healthy lifestyle. This notion has recently been reinforced by scientific evidence linking regular physical activity with a wide array of physical and mental health benefits, synonymous with an improvement in wellness (Pratt, Macera, and Wang, 2000:63). According to Pratt et al. (2000:63) higher direct medical costs associates with physical inactivity. Further cross-sectional epidemiologic studies and controlled experimental investigations conducted by Okura, Nakata and Tanaka (2003:1131) had demonstrated that physically active adults, in contrast to their sedentary counterparts, tend to develop and maintain higher levels of physical fitness. These studies had not only demonstrated the positive results of physical activity, such as an improvement in blood lipid profile, body composition, glucose tolerance and insulin sensitivity, but had also shown that participation in such activity decreased the risk of developing several chronic hypokinetic diseases, including coronary heart disease (CHD), hypertension, non-insulin dependant diabetes mellitus (type II), osteoporosis, colon cancer, anxiety and depression. In addition, low levels of habitual physical activity and the subsequent low levels of physical fitness were associated with a marked increase in all-cause mortality rates. Okura et al. (2003:1131) confirm that effects of exercise intensity on physical fitness and risk factors for coronary Herat disease.

Teenage pregnancy had negative impacts on various aspects of socio-economic well being of school girls. Aim: To determine the proportion of teenage pregnancy among high school girls and to identify the possible factors that influence teenage pregnancy in Mthatha region during the year 2009. Materials and Methods: This is an observational cross sectional, analytic study of teenage pregnancy conducted among high school teenage girls in the Mthatha region of South Africa. Result: A total of 1150 teenage girls from 15 to 19 years of ages responded from seven high schools in the Mthatha region of South Africa. In total 113 (10%) of teenage high school girls were currently mothers or had previously been pregnant. The proportions of those who had ever been pregnant increased significantly with age. The risk of falling pregnant among public high school teenage girls had two times higher compared to private high school teenage girls. Condoms were the most common method of contraception whereas the oral pills were the least common in practice. About half of teenage girls who had ever been pregnant had an abortion and of this one-third had had a backstreet/illegal abortion. The frequency of substance use was significantly higher among teenage girls who had ever been pregnant. A significant correlation was found between low socio-economic status, public schools and teenage pregnancy in this study. Conclusion: Teenage pregnancy is common among high school girls from 15 to 19 years of age in the Mthatha Region, South Africa. Poor socio-economic family conditions, lack of contraceptive use, early sexual maturation, risky behaviour, lack of knowledge about sexuality and reproductive health, multiple sexual partners and substance use were the common contributing factors of teenage pregnancy.
