"But what story?": a narrative-discursive analysis of "white" Afrikaners' accounts of male involvement in parenthood decision-making

In particular, the parenthood decision-making of “White”, heterosexual men from the middle class has been understudied, as indicated in the existing literature. In South Africa, this oversight has been exacerbated by the tendency for researchers to concentrate on “problematic” men, to the exclusion of the “boring, normal case”. I argue that this silence in the literature is a result of the taken for granted nature of parenthood in the “normal” heterosexual life course. In this study, I have turned the spotlight onto the norm of “Whiteness” and heterosexuality by studying those who have previously been overlooked by researchers. I focus on “White” Afrikaans men’s involvement in parenthood decision-making. My aim was to explore how constructions of gender inform male involvement in decision-making, especially within the South African context where social transformation has challenged traditional conceptions of male selfhood giving rise to new and contested masculine identities and new discourses of manhood and fatherhood. In an effort to ensure that women’s voices are not marginalised in the research, as is often the case in studies of men and masculinity, I conducted in-depth, semi-structured interviews about male involvement in decision-making with both “White” Afrikaans women and men. There were 23 participants in total, who all identified as heterosexual and middle-class. The participants were divided into two age cohorts (21 – 30 years and >40 years), which were then differentiated according to gender, reproductive status, and relationship status. Treating the interviews as jointly produced narratives, I analysed them by means of a performativity/performance lens. This dual analytic lens focuses on how particular narrative performances are simultaneously shaped by the interview setting and the broader discursive context. The lens was fashioned by synthesising Butler’s theory of performativity with Taylor’s narrative-discursive method. This synthesis (1) allows for Butler’s notion of “performativity” to be supplemented with that of “performance”; (2) provides a concrete analytical strategy in the form of positioning analysis; and (3) draws attention to both the micro politics of the interview conversation and the operation of power on the macro level, including the possibility of making “gender trouble”. The findings of the study suggest that the participants experienced difficulty narrating about male involvement in parenthood decision-making, owing to the taken for granted nature of parenthood for heterosexual adults. This was evident in participants’ sidelining of issues of “deciding” and “planning” and their alternate construal of childbearing as a non-choice, which, significantly served to bolster hetero-patriarchal norms. A central rhetorical tool for accomplishing these purposes was found in the construction of the “sacralised” child. In discursively manoeuvring around the central problematic, the participants ultimately produced a “silence” in the data that repeats the one in the research literature.

The sample was divided by sports affiliation (Contact n = 115; Non-Contact n = 74) and concussion history (2+ Concussion n = 43; 0 Concussion n = 108). Comparative subgroups were statistically equivalent for age, education and estimated IQ (P > 0.05), with the Contact sport groups having markedly higher incidences of concussion than controls (p < 0.000). Fatigue and aggression were the symptoms most pervasively high for the Contact and 2+ Concussion groups. Dependent t-test analyses at pre- versus post-season, revealed significant practice effects for the Contact group, not in evidence for controls on ImPACT Visual Motor Speed and Digit Symbol Incidental Recall-Delayed. Overall the results imply the possible presence of lingering neurocognitive and symptomatic concussion sequelae amongst South African final year high school participants of a contact sport. The indications gain potency when understood against the background of (i) Brain Reserve Capacity threshold theory, and (ii) the known risk of Type II error in group MTBI research, that might result in under-emphasis of subtle effects and miscalculation of cost-benefit risks. Clinical implications, and the need for prospective case-based research to ratify the results of this predominantly cross-sectional study, are discussed.

"I felt that I deserved it" : an Investigation into HIV-related PTSD, traumatic life events, and the personal experiences of living with HIV : a mixed-method study

- Description: There appears to be a growing body of literature focusing on PTSD and HIV-related PTSD (the diagnosis of HIV being the significant traumatic event) amongst HIV-positive samples, but only a few African studies that attempt to estimate the prevalence of PTSD amongst HIV-positive people, and even fewer that attempt to estimate the prevalence of HIV-related PTSD. The systemic review presented in this study is currently fully inclusive and is the most up-to-date available. Estimates of the prevalence of PTSD and HIV-related PTSD in South Africa range from 0.7 to 54.1% and, 4.2 to 40% respectively. The current cross-sectional study made use of a mixed-method approach to investigate traumatic life events, PTSD and HIV-related PTSD within a primary health-care centre in KwaZulu-Natal. The quantitative sample consisted of 159 adults (18-50 years) who were compliant on ARV medication. Using the CIDI-PTSD module, the adapted CIDI-PTSD module for HIV, and IES-R, findings indicated that 62% had reported some kind of traumatic event in their lifetime, with 29.6% of participants meeting the criteria for lifetime PTSD, and 40.9% meeting the criteria for lifetime HIV-related PTSD. Altogether, 57.9% of individuals met the criteria for some form of PTSD (either regular PTSD or HIV-related PTSD), and 12.6% met the criteria for both PTSD and HIV-related PTSD. Of the different categories of traumatic events, interpersonal violence has the highest rate of PTSD, followed by a diagnosis of and living with HIV, and then disaster. Furthermore, the IES-R was compared for its usefulness as a screening measure for PTSD against both the CIDI, but results suggest that it is an inferior screening measure to the PDS. The qualitative study consisted of six participants who were examined using IPA methodology informed by the Ehlers and Clark (2000) Model of trauma. Their experiences revealed experiences of stigma, a number of negative appraisals, negative emotions and coping behaviours. Some of the latter might serve as compensatory mechanisms to avoid negative judgements. Hypervigilance seems to be a feature of ARV-compliance that might confer added vulnerability to PTSD and other anxiety disorders.

The study draws on Butler’s notion of performative resistance and adopts a Foucauldian genealogy to examine the conditions that make visibility possible and through which particular representations of homosexuality are articulated and read, or remain unread or misread. Information about dressing-up, cross-dressing and drag performance was obtained in interviews, from documentary evidence, and from audio-visual recordings of drag shows and gay and lesbian beauty pageant competitions. Semiotics and a Foucauldian approach to analysing discourse were used to interpret the written, spoken, and visual texts. In this study I argue that the state prohibition of homosexuality during apartheid meant that people could not admit to knowing about it, and this ‘not knowing’ provided a cover for homosexual behaviour in public. At this time, the threat of being identified was associated with police raids on private parties. In the 1990s, homosexual visibility was more viable than it had been in the past. However, the strategies that were adopted to negotiate public visibility at this time were tailored to appease normative sentiments rather than challenge them. I argue that, historically, race and gender have played a role in diminishing and exacerbating homosexual visibility and its politics. Addressing the potential for harm that is associated with homosexual visibility in the 21st century post-apartheid context, this study considers the circumstances in which invisibility is desirable.

5-hydroxytryptamine and sexual behaviour in rhesus monkeys

PCPA in the doses used reduces the levels of 5-hydroxyindoleacetic acid (5HIAA) in the cerebrospinal fluid to 40 per cent of the normal oestradioltreated condition. Both the increased sexual receptivity and the lowered 5HIAA levels "in the CSF are in turn reversed by 5-hydroxytryptophal (5HTP), the irrmediate precursor of 5HT and the substance whose synthesis is inhibited by PCPA. 5HTP on its own reduces sexual receptivity and increases 5HIAA levels in the CSF of ovariectomised, oestradiol-treated (but otherwise intact) female rhesus monkeys. A substance other than an adrenal androgen has therefore been shown to restore sexual receptivity in adrenalectomised female monkeys . Testosterone propionate and oestradiol benzoate both lower the turnover rates of 5HT in the brains of ovariectomised female monkeys, as measured by the 2 hour probenecid test. Taken together, these findings suggest that adrenal androgens could act on specific sites in the female monkey brain via 5HT-containing neural systems, to control (or at least influence) sexual receptivity. All the results of administering oestradiol to ovariectomised monkeys in these experiments are consistent with the established roles of this hormone in female sexual attractiveness and in the gonadotrophin- controlling systems of the hypothalamo-hypophyseal axis. In contrast to these findings on 5HT and sexual receptivity in female monkeys, no clear role for 5HT- containing neural systems could be demonstrated in the grooming, aggressive or social behaviours of female monkeys. No clear role for 5HT could be demonstrated in the refractory period following ejaculation in male monkeys , or when testosterone replacement is given to castrated male monkeys.

A cross-cultural study of interpersonal distance and orientation schemata

- Description: Expectations about interpersonal distance during social encounters (distance schemata) and body orientation (orientation schemata) were investigated among White English-speakers and Xhosa groups which included illiterate traditionalists (Reds), poorly educated urban dwellers, and highly literate students and nurses. In a series of six experiments a doll placement task was used in which subjects represented dyadic encounters by placing pairs of standing dolls. For each situation represented the responses of each group of subjects were summarised in the form of a profile which showed the mean of the distance and three angle measures (IPOS profile). Independent variables included culture of subject, type of situation represented (friendly encounter, quarrel, accusation and denial, request) and the sex, age or relationship of the persons represented. In the culminating experiment (Experiment 6), females from three Xhosa groups (Reds or XR, poorly educated urban or XU, and urban nurses or XN) made twenty-three placements. In some respects the schemata of the four groups were very similar, while in others both distance and orientation schemata were a function of cultural group. The experiments allowed an assessment of the validity of the doll placement method to be made, and results were discussed in terms of the effects on interpersonal distance and body orientation of cultural norms concerning the showing of respect and the nature and strength of the emotions present in the various types of situation. It was concluded that with cultural movement away from the traditionalist pattern the schemata of the urban Xhosa showed a transition towards those found among the Whites in some respects. - Description: Expectations about interpersonal distance during social encounters (distance schemata) and body orientation (orientation schemata) were investigated among White English-speakers and Xhosa groups which included illiterate traditionalists (Reds), poorly educated urban dwellers, and highly literate students and nurses. In a series of six experiments a doll placement task was used in which subjects represented dyadic encounters by placing pairs of standing dolls. For each situation represented the responses of each group of subjects were summarised in the form of a profile which showed the mean of the distance and three angle measures (IPOS profile). Independent variables included culture of subject, type of situation represented (friendly encounter, quarrel, accusation and denial, request) and the sex, age or relationship of the persons represented. In the culminating experiment (Experiment 6), females from three Xhosa groups (Reds or XR, poorly educated urban or XU, and urban nurses or XN) made twenty-three placements. In some respects the schemata of the four groups were very similar, while in others both distance and orientation schemata were a function of cultural group. The experiments allowed an assessment of the validity of the doll placement method to be made, and results were discussed in terms of the effects on interpersonal distance and body orientation of cultural norms concerning the showing of respect and the nature and strength of the emotions present in the various types of situation. Using a Foucauldian postcolonial feminist approach and narrative-discursive analysis, this study explores abortion decision-making narratives in a Zimbabwean context where abortion laws are restrictive. In this study I elicited the narratives of women who had undergone an abortion about how they came to make the decision and proceeded to terminate the pregnancy. I highlight the discourses employed in constructing these narratives and how women position themselves in these narratives and discourses. These are then compared to the subject positions enabled in health service providers’ narratives on the same topic. These narratives are then linked to the social discourses and power relations that work to enable or constrain reproductive justice. The data were collected from three sites in Harare, Zimbabwe. The three sites were Harare Hospital, Epworth and Mufakose. An adapted version of Wengraf’s (2001) narrative interview was used to elicit narratives from 18 women who had terminated pregnancies (six at each site). Semi-structured interviews were conducted with six service providers (two nurses at Harare Hospital, two village health workers in Epworth and two nurses in Mufakose). All the service providers interviewed have experience working with women who have terminated pregnancies. In narrating their stories about their abortions, the women employed discursive resources around shame, stigma, religion, health and culture. These discursive resources were drawn upon in the construction of the women’s micro-narratives. The women spoke in a socially sanctioned manner where stories were enabled and constrained by particular religious, cultural and gendered discursive resources. In these stories, cultural constructions, gendered understandings of motherhood and femininity constrained reproductive justice for women who have terminated pregnancies. Comparisons of the way women positioned themselves and how they were positioned by health service providers point to the existence of social discourses and power relations that work to constrain reproductive justice. While the women saw themselves as having ‘unsupportable pregnancies’, the service providers positioned them as being evil, selfish and irresponsible. The negative positions deployed by the service providers point to the vilification and blaming of women who have undergone a termination of pregnancy. In these positions, the woman is at fault and there is silence on the role of men in abortion decision-making. In the women’s narratives and the health service providers positioning of the women a ‘reproductive rights’ discourse was absent. This was significant as much of the activism around abortion has centred on the woman’s rights to her body. Where rights were mentioned, it was in reference to foetal rights (using cultural, moralistic religious understandings of abortion as killing). The missing ‘reproductive rights’ discourse points to a need to move from a reproductive rights framework to a reproductive justice framework that can be applied through local understandings of hunhu/ubuntu. By doing this, abortion is not seen as a ‘choice’ that a woman makes but rather as involving broader social and environmental circumstances that make a pregnancy ‘unsupportable’.

- Description: Psychologists investigating dreams in non-Western cultures have generally not considered the meanings of dreams within the unique meaning-structure of the person in his or her societal context. The majority of dream studies in African societies are no exception. Researchers approaching dreams within rural Xhosa and Zulu speaking societies have either adopted an anthropological or a psychodynamic orientation. The latter approach particularly imposes a Western perspective in the interpretation of dream material. There have been no comparable studies of dream interpretation among urban blacks participating in the African Independent Church Movement. The present study focuses on the rural Xhosa speaking people and the urban black population who speak one of the Nguni languages and identify with the African Independent Church Movement. The study is concerned with understanding the meanings of dreams within the cultural context in which they occur. The specific aims of the study are: 1. To explicate the indigenous system of dream interpretation as revealed by acknowledged dream experts. 2. To examine the commonalities and the differences between the interpretation of dreams in two groups, drawn from a rural and urban setting respectively. 3. To elaborate upon the life-world of the participants by the interpretations gained from the above investigation. One hundred dreams and interpretations are collected from two categories of participants referred to as the Rural Group and the Urban Group. The Rural Group is made up of amagqira [traditional healers] and their clients, while the Urban Group consists of prophets and members of the African Independent Churches. Each group includes acknowledged dream experts. A phenomenological methodology is adopted in explicating the data. The methodological precedure involves a number of rigorous stages of explication whereby the original data is reduced to Constituent Profiles leading to the construction of a Thematic Index File. By searching and reflecting upon the data, interpretative themes are identified. These themes are explicated to provide a rigorous description of the interpretative-reality of each group. Themes explicated within the Rural Group are: the physiognomy of the dreamer's life- world as revealed by ithongo, the interpretation of ithongo as revealed through action, the dream relationship as an anticipatory mode-of-existence, iphupha as disclosing a vulnerable mode-of-being, human bodiliness as revealed in dream interpretations and the legitimation of the interpretative-reality within the life-world. Themes explicated within the Urban Group are : the physiognomy of the dreamer's life-world revealed in their dream-existence, the interpretative-reality revealed through the enaction of dreams, tension between the newer Christian-based cosomology and the traditional cultural-based cosmology, a moral imperative, prophetic perception and human bodiliness, as revealed in dream interpretations and the legitimation of the interpretative-reality within the life-world. The essence of the interpretative-reality of both groups is very similar and is expressed in the notion of relatedness to a cosmic mode-of-being. The cosmic mode-of-being includes a numinous dimension which is expressed through divine presence in the form of ancestors, Holy Spirit or God. These notions cannot be apprehended by theoretical constructs alone but may be grasped and given form in meaning-disclosing intuitions which are expressed in the lifeworld in terms of bodiliness, revelatory knowledge, action and healing. Some differences between the two groups are evident and reveal some conflict between the monotheistic Christian cosmology and the traditional cosmology. Unique aspects of the interpetative-reality of the Urban Group are expressed in terms of difficulties in the urban social environment and the notion of a moral imperative. It is observed that cultural self-expression based upon traditional ideas continues to play a significant role in the urban environment. The apparent conflict revealed between the respective cosmologies underlies an integration of traditional meanings with Christian concepts. This finding is consistent with the literature suggesting that the African Independent Church is a syncretic movement. The life-world is based upon the immediate and vivid experience of the numinous as revealed in the dream phenomenon. The participants' approach to dreams is not based upon an explicit theory, but upon an immediate and pathic understanding of the dream phenomenon. The understanding is based upon the interpreter's concrete understanding of the life-world, which includes the possibility of cosmic integration and continuity between the personal and transpersonal realms of being. The approach is characterized as an expression of man's primordial attunement with the cosmos. The approach of the participants to dreams may not be consistent with a Western rational orientation, but nevertheless, it is a valid approach. The validity is based upon the immediate life-world of experience which is intelligible, coherent, and above all, it is meaning-giving in revealing life-possibility within the context of human existence.

Through examination of the literature a question was formulated that would disclose the analyst's experience of the technique of using the couch. Four experienced psychoanalytic practitioners who could be operationally defined as 'analysts' were interviewed. Using a phenomenological method the protocols were comprehensively analyzed to produce descriptions of the general structure of the experience. These were then texturally enhanced using interleaved direct citations from the interviews. The structural and textural 'findings ' so produced were then hermeneutically dialogued with contemporary psychoanalytic notions of critical discourse and intersubjectivity. The phenomenological ' findings ' of the study disclosed the meaning of the couch as context-based, paradoxical and ambiguous. The couch was found to be a symbol of the analyst as analyst and the process as authentic analysis. Furthermore, at its best, the couch was found to mediate a mode of being that is containing and intimate and in which psychological life may be evoked, tracked and interpreted. The most significant contributor to this mode of being was found to be privacy, which, in particular, helps the analyst maintain an analytic attitude. The couch was also found to be significantly implicated in the generation of an intersubjective analytic third and to support reverie. These 'findings' were hermeneutically dialogued with literature on the couch as well as contemporary psychoanalytic theoretical notions. The dialogue fell into three foci. The first focus entailed deconstructing the meaning of the couch as context-based and ambiguous and not essential. The second pursued critiques of the role that the couch plays in domination, of its function as a symbol/evocative object and of the way in which it shapes being-together, bodily attunement, privacy, the intersubjective analytic third and reverie. Finally the 'findings' were critically examined in terms of both Lacan's notion 'analytic discourse ' and its role in revealing/concealing the analysand as subject. The study concludes with an examination of its own limitations and suggestions for further research.

The most significant contributor to this mode of being was found to be privacy, which, in particular, helps the analyst maintain an analytic attitude. The couch was also found to be significantly implicated in the generation of an intersubjective analytic third and to support reverie. These 'findings' were hermeneutically dialogued with literature on the couch as well as contemporary psychoanalytic theoretical notions. The dialogue fell into three foci. The first focus entailed deconstructing the meaning of the couch as context-based and ambiguous and not essential. The second pursued critiques of the role that the couch plays in domination, of its function as a symbol/evocative object and of the way in which it shapes being-together, bodily attunement, privacy, the intersubjective analytic third and reverie. Finally the 'findings' were critically examined in terms of both Lacan's notion 'analytic discourse ' and its role in revealing/concealing the analysand as subject. The study concludes with an examination of its own limitations and suggestions for further research.

Contemporary Kleinian and neo-Kleinian theoretical perspectives provided the interpretative perspective on the complexities of inaccessible personalities and subtle forms of internal destructiveness. Four research questions were formulated to interrogate the individual and collective experiences of three male patients whose internal worlds seem to be governed by rigid intrapychic structures organized around the dictatorship of a constellation of seemingly impenetrable defensive strategies. These questions were as follows: 1. What emotional states, actions and experiences of self and others characterize the clinical phenomenon designed as destructive narcissism and distinguish it from other forms of character pathology? 2. What early developmental experiences and relationships may have pre-disposed individuals to the development of this type of character pathology? 3. How are the psychodynamic processes of destructive narcissism structured and configured in the psychotherapeutic process and progress? 4. What are the transference/countertransference psychotherapeutic manifestations of the psychodynamics of destructive narcissism? The illustrative-didactic case study method was utilized to discuss pertinent aspects of each patient. This included their early developmental histories, inter- and intrapersonal relationships, their current mental state, defensive strategies and their stated reasons for commencing psychotherapy. In addition, the structure of the psychotherapeutic process with these patients was reviewed in depth. Various psychic and personality features, as unveiled through this process were discussed, as well as the implications of these for the therapeutic endeavor. The features chosen for discussion were: Firstly, the constellation of the internal object world, the capacity for symbolic thought and defensive organizations. Secondly, therapeutic ambivalence, which made psychotherapy untenable, was explored in conjunction with transference/countertransference issues. Thirdly, the shadow sides of psychotherapeutic change with these patients were considered and the issues of therapeutic failure and other treatment possibilities were examined. It was concluded that there need be an important shift with regard to the psychotherapeutic goals for those patients whose condition may be chronic, and for whom it appears that psychotherapy is of little benefit. In essence, the intent of psychotherapy with these patients is to reach the healthy sane patient of the patient within the pathological organization. Attempts to unravel the perverse gratification and protection derived from the domination of the narcissistic structure may not be enough, and the patient's collusion with the internal destructive gang should also be exposed. If this can be achieved, the patient may come to accept the existence of a part of himself as truly destructive. This, in turn, cannot be disowned, therefore the patient has to live with it. As an individual with autism Grandin faced many challenges growing up. Despite a difficult and absent beginning, Grandin developed into a stable and scientifically creative adult who contributes to society. She excels as an animal scientist and designer of humane livestock handling facilities and has an international reputation for her contribution to the livestock industry and animal welfare. The primary aim of this study is to describe and interpret the life of Temple Grandin through Erikson’s (1950/1973) theory of psychosocial development. A mixed method approach (Yin, 2006) was employed for the conduction of this study. The overarching data processing and analysis guidelines for this study were provided by Miles and Huberman (1994, 2002a, 2002b). The conduction of the processing and analysis of data was aided by Alexander’s (1988, 1990) method of asking the data questions as well as an integration of Yin’s (2014) time series analysis with Erikson’s (1950/1973) triple bookkeeping approach. This study contributes to the development of psychobiographical research in South Africa as well as to personality and developmental theory.

The evidence from independent researchers has shown that SAPS statistics are highly susceptible to under-reporting and the actual figure is more than double this amount. One pervasive feature of the phenomenology of rape is post-traumatic stress disorder (PTSD). PTSD is a highly debilitating condition with severe individual and societal costs. The condition takes a critical toll on occupational functioning, schooling and personal relationships and is associated with depression, suicide risk, self-harming behaviours and alcohol-abuse problems. The Ehlers and Clark (2000) cognitive model represents the most efficacious treatment for PTSD but the approach is severely under-utilised by South African practitioners working with sexual trauma. The reasons for such under-utilisation relate to a lack of exposure and training surrounding the model and concerns about the transportability of the treatment to a multi-cultural context. One method of addressing these barriers to treatment delivery is through systematic case-based research. Systematic case-based research offers a complementary means of refining theory and developing evidence-based practice in the context of a developing country. The method offers an intensive analysis and description of the particular phenomena under study within its real-life context. It allows the researcher to intensively examine and identify the specific aspects of the therapist’s responses and client’s reactions that contributed to significant change. Unlike efficacy studies, generalisability in case-study research is based on replication on a case-by-case basis and the creation of case law. This research study uses a systematic-case study approach to investigate the applicability of the Ehlers and Clark (2000) model in the treatment of rape-related PTSD in South Africa. The study aims to demonstrate the transportability of the model and develop a needed evidence base for service providers in the country. Seven women participated in the project and lent their treatment process to the research. The public itself needs to become more aware of road safety. Many associations and institutions have been created to assist with the inculcation of the road safety ideal. The National Road Safety Council, which creates opportunities for investigation into aspects of road safety must continue to expand its sphere of influence. The research project contained on the following pages represents an attempt to contribute to the human factor in traffic safety, particularly by making a closer study of the driver of the motor vehicle. The writer is concerned with the carnage that is caused by road accidents. His previous study of personality and particularly of the projective techniques of assessment has prompted a consideration of the problem.

- Description: A qualitative empirical phenomenological study was undertaken to determine the self-experience of procrastinating behaviour. Five students each gave an account of an occasion when they procrastinated. The resultant protocols were analysed and the Situated Structure of each individual’s experience was reported. From these, the General Structure of procrastinating behaviour was determined. A further, novel step was added to the standard methodology, whereby ‘themes’ were extracted from participant protocols and a ‘Composite Reality’ of everyday-life procrastination was rendered. Participants’ accounts suggest they are concerned the results of intellectual tasks they undertake will be seen as equivalent to their quality of being-as-an-individual: poor work results will be interpreted by important-others as evidence of participants’ poor quality of self – which is to be avoided. This study suggests that procrastination is a ploy used by individuals to avoid criticism, by deflecting assessment of their capacity to complete a task well, to instead, what they are capable of when only a limited time is available. Conclusions drawn by the important-others of participants’ true ability are thereby confounded. The results achieved in the phenomenological study were compared with others originating from various quantitative studies, and considerable overlap was found. The experiential richness of the phenomenological results point to a worthwhile methodological strategy for future procrastination research.

- Description: This study examines the workings of the ground rules which make up the framework of psychotherapy, in an institutional context, by analysing transcripts of twelve audio taped sessions of therapy conducted in a psychiatric hospital. The breaks in the ground rules of the sessions are noted and the patients' responses to these breaks are analysed using Langs's (1982, 1988) method for decoding patients' material, suitably modified for use as a hermeneutic research method. Although all of the ground rules are broken in the institutional context, only one of the ten ground rules appears to be essentially affected by this particular institutional context. Other ground rules are broken out of choice of technique or through error. The institutional context has a structural impact only on the ground rule requiring a one to one relationship with privacy and confidentiality and this ground rule is transgressed in a number of ways in all twelve sessions examined in this study. However the patients' responses to this breach only occur in ways predicted by communicative theory when the break in the ground rule involves actual entry into the therapy space by another person. Other contraventions to this ground rule that do not involve such an entry do not elicit the predicted patient responses. The many other ground rule breaks occurring in the institutional context evoke the predicted responses in the patients' material. In the study, no therapist interventions are found to comply with the communicative therapy requirements for sound interventions; concomitantly it was found that no therapist interventions receive the required derivative validation. The results indicate that it is possible to conduct therapy of a substantially secure frame variety in this institutional context with minimum effort on the part of therapists and given proper training and supervision of therapists in the techniques of communicative psychotherapy. Furthermore the results lend weight to the importance of the communicative methodology for listening to patients' material in psychotherapy in an institutional context. However, further rigorous study of competently performed therapy, executed within the context of a secure frame within an institutional context, is needed in order to demonstrate the benefits of the communicative psychotherapy interventions and interpretations in this context.
