Its methodology, predominantly qualitative, employed anthropological and psychological procedures. The fieldwork (1988-1989)encompassed a three month stay in the village of Msobomvu. Patients continued to be tracked informally until June, 1995. The empirical research has three parts. In part one, the person with a mental illness was contextualized within Xhosa cosmology and social attitudes. The cognitive and social ecologies were tapped through the narratives of high school and university students at different stages of a Western-biased education. Social attitudes regarding mental illness, and confidence in treatment by traditional healers and the hospital, were also evaluated. Traditional attitudes and supernatural beliefs of illness causation persisted in spite of Eurocentric education, with a concurrent increase in the acceptance of Western-type causal explanations commensurate with continued education. Part two considered the the patients in relation to (i) the biomedical framework (the mental and local hospitals), and (ii) their readjustment to the community after hospitalization. Data came from patient charts, interviews with medical staff, and follow-up visits in the villages. Socio-political and economic issues were salient. Part three case-studied people identified by the village residents as having a mental illness. Resources for treatment - traditional healers, mobile clinic, and village health workers - were the focus. The traditional healing system, and biomedicine, were compared for effectiveness, through the course of illness events. While biomedicine was more effective in containing acute psychotic episodes than treatment by the traditional healer, lack of appropriate resources within the biomedical setting had disastrous results for patient compliance and long-term management of the illness, particularly in people with obvious symptoms of bipolar disorder. The mental hospital emerged as an agent of control. While Xhosa culture provided a more tolerant setting for people with a mental illness, the course of severe mental illness was by no means benign, despite research suggesting a more positive outcome for such conditions in the developing world.

The therapist moves from a position of trying to understand the experience in relation to his own world, to the realisation that it is co-determined by the patient. From a position of reflective distance he re-appropriates aspects of his world that were closed to him while under the influence of the patient, in addition to appropriating previously unowned aspects. The therapist dialogues these appropriations with the invoked feelings, allowing him to differentiate those aspects of his feeling-state which are authentically his from those which are unowned aspects of the patient's aspects of his world that were closed to him while under the influence of the patient, in addition to appropriating previously unowned aspects. As 45-jarige man het De Wet in die Anglo-Boereoorlog geveg en vinnig opgang as 'n militêre leier gemaak. Sy vindingryke ontsnappingspogings het die Britse magte hoofbrekens besorg. Wat De Wet verder uniek maak, is dat hy met net drie maande skoolopleiding in die parlement gedien het, as president verkies is, die hoogste gesag in die Verdedigingsmag gehad het en 'n boek oor sy oorlogsherinneringe geskryf het. Alhoewel daar akademiese werke oor De Wet bestaan, is daar tot op hede nog geen psigobiografiese studie oor hom gedoen nie. Die doel van die studie was om die mens agter die legende na vore te bring. Daar is ook gepoog om moontlike antwoorde en verklarings op onbeantwoorde vrae te gee. Die studie het van beide kwalitatiewe en kwantitatiewe data gebruik gemaak en kan as 'n psigobiografiese gevalstudie beskryf word. Die Vyf-faktor model van Costa en McCrae (1992) is op die biografiese data van De Wet toegepas om sodoende sy persoonlikheidseienskappe te beskryf. Die bevindinge het interessante persoonlikheidseienskappe van De Wet deur middel van die Vyf-faktor model aan die lig gebring. Volgens die kwalitatiewe navorsingsbevindinge was De Wet 'n vyandige, aksie-gedrewe, bevoegde, prestasie-strewende, selfgedissiplineerde, warm, openhartige, saggeaarde en beskeie persoon. Die kwalitatiewe en kwantitatiewe data-ontledings het uiteenlopende persoonlikheidskenmerke by De Wet geïdentifiseer. Latent murderousness: an exploration of the nature and quality of object relations in rage-type murderers

- Description: In this dissertation I investigate the intrapsychic make-up of rage-type offenders and explore the psychodynamics of the act of murder itself The dissertation begins with a discussion on the defining features of the act of rage-type murder. I then consider the role of personality characteristics and psychopathology in individuals who have committed such offences. With the basic features of the offender and act itself outlined, the following section reviews key areas of debate regarding the psychodynamics of violence and the intrapsychic make-up of the rage-type murderer. I first explore the nature of aggression as debated in psychoanalysis and conclude that the views expressed are often unn,ecessarily polarized regarding the origins of aggression and suggest that the specifics of particular types of aggression require consideration in order to assess their intrapsychic nature. The specifics of rage and violence are discussed with this in mind. In the second chapter of this section I develop a number of intrapsychic dimensions to be used in understanding how different types of violence are constituted. Psychodynamic contributions towards understanding rage-type murder, as a specific form of violence, are then discussed. Following this review, a number of directive ~uestions are formulated regarding (1) the intrapsychic dimension of rage-type murder; (2) the pJ;esence of the borderline personality in such offender~ and its intrapsychic nature; and (3) the_ specific psychodynamics that lie behind what is argued to be a defensive act of murder. A multiple case study approach, using nine imprisoned rage-type offenders, is used to further explore the above issues. Court summary reports, the Thematic Apperception Test and the Psychoanalytic Research Interview comprised the research material, with particular emphasis placed on the interview material. The interview is approached from a psychoanalytic perspective and I develop some theoretical, technical and analytical guidelines to try to broaden Jhe use of psychoanalysis in the research domain. Findings of the research reveal a specific kind of defensive organization that is characterized by a constellation of object relations that I term the 'narcissistic exoskeleton'. I suggest that these findings best fit the description of a particular kind of borderline personality organization typified by apparent 'normality'. Other prominent aspects of the dimensions of violence observed in these cases include: (1) a poor representational capacity; (2) an interactional style characterized by uncontainable projective exchanges between victim and offender; (3) a collusive primary object relationship combined with the absence of an internalized 'third object'; (4) a 'two-faced' superego structure; (5) the internalization of traumatic experience that has become associated with a bad object system; (6) phantasies of restoring ideal good in external objects alongside conscious fantasies of annihilation. Within the context of these factors the intrapsychic events that lead to the act itself are discussed. It is found that a collapse of the 'narcissistic exoskeleton', the intrusion of the bad object system and the unbearable shame that this evokes in the offender are prominent features of what culminates in an act of explosive rage and projective identification. Some of the implications of my research are briefly discussed in the concluding chapter.

The general approach involved four major phases: (1) the clustering of various variables and identification of underlying key factors purported to be related to psychological health based on the writer's clinical experience and review of the mental health literature, (2) the formulation of an ~ priori concept of psychological well-being based on the operational definition of those factors, (3) the construction of an inventory designed to examine the proposed concept, and (4) the interpretation of the results and their implications for the development of the overall concept based on the examination of the factorial structure, validity and reliability of the inventory. The"a posteriori concept" evolved out of this process. The results of the present study indicate that the most valid and reliable factorial components of psychological well-being are self regard,interpersonal relationship, independence, problem-solving, assertiveness,reality testing, stress tolerance, self-actualization and happiness; social responsibility and flexibility emerged as questionable components of the a posteriori concept. The inventory which was designed to study the concept was successful in significantly differentiating various levels of psychological health. This way of conceptualizing and assessing psychological well-being has potential applicability for mental health practitioners and researchers.

The results of the present study indicate that the most valid and reliable factorial components of psychological well-being are self regard,interpersonal relationship, independence, problem-solving, assertiveness,reality testing, stress tolerance, self-actualization and happiness; social responsibility and flexibility emerged as questionable components of the a posteriori concept. The inventory which was designed to study the concept was successful in significantly differentiating various levels of psychological health. This way of conceptualizing and assessing psychological well-being has potential applicability for mental health practitioners and researchers.

In a combined analysis drawing on discursive theory and psychoanalysis, more specifically contemporary attachment theory and intersubjectivity theory, I illustrate how both these mothers and I – as emotional, social and political subjects – co-construct maternal subjectivity. Based on the findings, recommendations for parent-infant interventions are discussed. Arguing that a purely psychoanalytic reading of investment perpetuates notions of individual blame and pathology, I advocate for a psychosocial reading that does not neglect failing social systems but rather pursues an open and reflective, yet critical, mindfulness when listening to talk.

The study utilised an interpretative phenomenological analysis (IPA) approach, which informed all aspects of the data-collection and data-analysis processes. Using purposive sampling, 12 women were recruited and interviewed on three occasions over a period of six months. The interviews were conducted in Chichewa and audio recorded and later transcribed into English. Fourteen superordinate themes emerged from the analysis representing the women’s prominent life experiences, how they received their diagnosis, and how they contained the trauma of their diagnosis. The analysis also developed themes regarding how they coped with the challenges of living with the human immunodeficiency virus (HIV) and made sense of their experiences. The results showed that the women struggled with multiple challenges experienced through their various identities as WLHIV. The findings also pointed to interlocking disadvantages that put the women at risk of infection, and which were present from childhood. This has implications for more structural and multidisciplinary interventions for WLHIV.

- Description: This study examined the ethical decision-making process as it transpired in the everyday context of the therapeutic space. In-depth interviews explored the subjective experiences of six South African psychologists, practicing as psychoanalytic psychotherapists, and their efforts to resolve real-life ethical dilemmas. The theoretical framework used to interpret the data subsumed professional literature in psychology on principle-based ethical decision-making as well as contemporary psychoanalytic debates on the phenomenon of countertransference enactments. A review of ethics codes, survey research and seminal decision-making frameworks suggests that ethical dilemmas have traditionally been resolved by recourse to an objective and impartial “principle ethics” perspective. Empirical evidence shows, however, that logical thinking and the rational application of codes, principles and standards are often insufficient to secure ethical action. The establishment of reflective space and the core theoretical notion of “ethical decision-making enactments” were proposed in order to address the subjective, irrational and unconscious dimension of professional decision-making. This study used a broadly hermeneutic research method which transformed participants‟ descriptions of engagement with real-life dilemmas into a psychoanalytically informed interpretive account of ethical decision-making. Twelve aspirational ethical principles were found to guide participants‟ daily analytic work. Beneficence was the principle most strongly identified with and nonmaleficence was the most neglected ethical principle. Unprocessed countertransference responses were shown to drive earlier prereflective phases of the ethical decision-making process. Mature ethical judgment was predicated upon the retrospective analysis of enactment phenomena. Dissatisfaction was expressed by all participants with regard to the role of professional resources in aiding the resolution of stressful ethical dilemmas. Risk factors for compromised professional decision-making included the paucity and perceived irrelevance of postgraduate ethics training, supervisory failure to confront the ethical and countertransference dimensions of common dilemmas and professional isolation. Rather than eliciting the hope of emotional support and greater insight, professional resources on the contrary mostly appeared to induce anxiety, mistrust and fearfulness. Based on the data and the literature, a pragmatic psychoanalytically informed ethical decision-making model was finally generated. The model, which considers both principle ethics as well as countertransference phenomena, offers a preliminary contribution to professional dialogue on the development and evaluation of empirically based decision-making frameworks. Practical recommendations are made for both the revision of the current South African ethics code and for improving the postqualifying ethics education of psychoanalytic practitioners and supervisors. The limitations of the data are discussed and directions for future research initiatives are proposed.

In a review of the cognitive aging literature, the implications of a differential perspective were drawn out in order to establish a theoretical and methodological basis for an investigation into variability. A number of regularly occurring patterns, identified on the basis of available reports in the literature, were used to develop a model of variability (the shuttle model). The empirically-based model was located broadly within a neuropsychological framework, and derived explanatory power from the tenets of brain reserve capacity (BRC) theory. It served to describe the bulge in interindividual variability due to aging (the shuttle bulge), and the shifting occurrence of the bulge in relation to the age axis due to cohort and task-related influences (the shuttle shift). A two phase research study was conducted in order to test hypotheses derived from the model. Phase 1 comprised between-groups analyses of normative data covering a broad range of neuropsychological tests in the domains of attention, memory, language, visual and hand motor skills, in order to examine the progression of variability effects across the adult age range. Phase 2 constituted between and within-groups analyses of normative data from a more limited number of neuropsychological tests. It included the examination of raw score distributions and the characteristics of outliers, and was undertaken to explore more closely the nature of the variability phenomena detected in the first phase of the analysis. Taken together, the results of both phases of the investigation revealed statistically significant variability effects in support of the shuttle model. There was a consistent pattern of increased variability in association with older age regardless of functional modality; frequently, in association with later old age, there was also a subsequent decrease in variability (the shuttle bulge). The age of onset of the initial increase in variability occurred earlier or later (the shuttle shift) as a function of four factors: education, gender, task challenge and age-sensitivity of task. The finding of an earlier onset of variability effects for low education, male gender, high task challenge and high age-sensitivity of task was interpreted in terms of BRC threshold theory. The clinical and social implications of the outcome were discussed with special emphasis on the need for a differential perspective on aging, as a complement to the prevailing normative tradition. It was concluded that the shuttle model has considerable heuristic value. It presents an integrative framework for understanding existing variability data and provides clear indications for future research.

- Description: In 2009, South African police statistics revealed that more than 68 332 women were raped in the country. The evidence from independent researchers has shown that SAPS statistics are highly susceptible to under-reporting and the actual figure is more than double this amount. One pervasive feature of the phenomenology of rape is post-traumatic stress disorder (PTSD). PTSD is a highly debilitating condition with severe individual and societal costs. The condition takes a critical toll on occupational functioning, schooling and personal relationships and is associated with depression, suicide risk, self-harming behaviours and alcohol-abuse problems. The Ehlers and Clark (2000) cognitive model represents the most efficacious treatment for PTSD but the approach is severely under-utilised by South African practitioners working with sexual trauma. The reasons for such under-utilisation relate to a lack of exposure and training surrounding the model and concerns about the transportability of the treatment to a multi-cultural context. One method of addressing these barriers to treatment delivery is through systematic case-based research. Systematic case-based research offers a complementary means of refining theory and developing evidence-based practice in the context of a developing country. The method offers an intensive analysis and description of the particular phenomena under study within its real-life context. It allows the researcher to intensively examine and identify the specific aspects of the therapist’s responses and client’s reactions that contributed to significant change. Unlike efficacy studies, generalisability in case-study research is based on replication on a case-by-case basis and the creation of case law. This research study uses a systematic-case study approach to investigate the applicability of the Ehlers and Clark (2000) model in the treatment of rape-related PTSD in South Africa. The study aims to demonstrate the transportability of the model and develop a needed evidence base for service providers in the country. Seven women participated in the project and lent their treatment process to the research. The participants varied in terms of age, race, culture, socio-economic status and the nature of their sexual trauma. Through synoptic thematic analysis of their therapy process specific client-related personal aspects, client-related contextual factors and state-level factors were found to impede treatment delivery and implementation. The implications of these aspects for clinical practice and social policy are comprehensively discussed.

- Description: In 2009, South African police statistics revealed that more than 68 332 women were raped in the country. The evidence from independent researchers has shown that SAPS statistics are highly susceptible to under-reporting and the actual figure is more than double this amount. One pervasive feature of the phenomenology of rape is post-traumatic stress disorder (PTSD). PTSD is a highly debilitating condition with severe individual and societal costs. The condition takes a critical toll on occupational functioning, schooling and personal relationships and is associated with depression, suicide risk, self-harming behaviours and alcohol-abuse problems. The Ehlers and Clark (2000) cognitive model represents the most efficacious treatment for PTSD but the approach is severely under-utilised by South African practitioners working with sexual trauma. The reasons for such under-utilisation relate to a lack of exposure and training surrounding the model and concerns about the transportability of the treatment to a multi-cultural context. One method of addressing these barriers to treatment delivery is through systematic case-based research. Systematic case-based research offers a complementary means of refining theory and developing evidence-based practice in the context of a developing country. The method offers an intensive analysis and description of the particular phenomena under study within its real-life context. It allows the researcher to intensively examine and identify the specific aspects of the therapist’s responses and client’s reactions that contributed to significant change. Unlike efficacy studies, generalisability in case-study research is based on replication on a case-by-case basis and the creation of case law. This research study uses a systematic-case study approach to investigate the applicability of the Ehlers and Clark (2000) model in the treatment of rape-related PTSD in South Africa. The study aims to demonstrate the transportability of the model and develop a needed evidence base for service providers in the country. Seven women participated in the project and lent their treatment process to the research. The participants varied in terms of age, race, culture, socio-economic status and the nature of their sexual trauma. Through synoptic thematic analysis of their therapy process specific client-related personal aspects, client-related contextual factors and state-level factors were found to impede treatment delivery and implementation. The implications of these aspects for clinical practice and social policy are comprehensively discussed.

These findings lend support to new phenomenological research which suggests that artmaking intersects with disrupted abilities for perceptual engagement underlying manifestations of schizophrenia symptomology. This research supports assertions that artmaking has the potential to support the minimal sense of self and expand possibilities for renewed embodied and more reflective meaning-making. Explanations of findings centred around the distinct artistic style of each participant, as well as their artworks which revealed individual lifeworlds including a variety of self-positions. The inherent properties of the completed artworks also provided possibilities for renewed experiences of sociality. Additional support for the findings is drawn from recent research in the fields of early neurodevelopmental trauma, and trauma research findings within the art therapy field.

The literature review consisted of two parts. Firstly, the philosophical literature on the theory of interpretation was reviewed. A set of central philosophical issues was identified, which pertain to the psychotherapeutic project of interpreting the meaning of a person's experience in the context of a dialogue with that person. Secondly, the psychotherapeutic literature was reviewed. Previous attempts to conceptualise and prescribe processes of interpretation were described. The issues which appeared to be in need of further clarification were identified. A clinical study was conducted to further explore the questions raised in the literature reviews. A methodology was developed which gave access to the direct experience of both clients and therapists during the events of psychotherapeutic interpretation. The methodology yielded a description of the interpretative structure of the psychotherapeutic dialogue for each therapist-client pair. These were then consolidated into a description of general structural features of the psychotherapeutic dialogue. The results consisted of a description of processes and structural features which are intrinsic to the psychotherapeutic interpretation of the meaning of a person's experience in the context of a dialogue. The results were elaborated in an extensive discussion from which the following findings emerged: (l)It is important to distinguish between communicative and interpretative forms of dialogue. (2)Thematisation activity is mediated by a number of dialectically related operations which are intrinsic to the interpretative project of psychotherapy. (3)Insight-oriented psychotherapy relies on the presence of the therapist as a dialogical partner and the therapist is not merely a facilitator of introspection on the part of the client. (4)The character of interpretation in psychotherapy may be understood in certain respects to be an elaboration of functions of the imagination. (5)The process of interpretation can be understood in relational terms and the variations of interpretative experience may be understood as variations of 'an inter-subjective interpretative ideal. (6)Understanding of certain forms of psychopathology is deepened when they are considered as variations of an ideal capacity to engage in interpretative dialogue. (7)It is possible to describe certain ideal conditions which are facilitative of interpretative dialogue and hence of the psychotherapeutic development of self-insight. In conclusion suggestions for further research were made. It was suggested that the perspective of hermeneutic phenomenology provides an appropriate philosophical and methodological foundation for understanding the unique dialogical interpretative situation which is psychotherapy. The study emphasized, both in its content and in the manner of its execution, the need for interpretative efforts to be accompanied by methodological reflection and especially an awareness of how interpretative strategies partially constitute the realities they set out to describe.

- Description: This study examines the workings of the ground rules which make up the framework of psychotherapy, in an institutional context, by analysing transcripts of twelve audio taped sessions of therapy conducted in a psychiatric hospital. The breaks in the ground rules of the sessions are noted and the patients' responses to these breaks are analysed using Langs's (1982, 1988) method for decoding patients' material, suitably modified for use as a hermeneutic research method. Although all of the ground rules are broken in the institutional context, only one of the ten ground rules appears to be essentially affected by this particular institutional context. Other ground rules are broken out of choice of technique or through error. The institutional context has a structural impact only on the ground rule requiring a one to one relationship with privacy and confidentiality and this ground rule is transgressed in a number of ways in all twelve sessions examined in this study. However the patients' responses to this breach only occur in ways predicted by communicative theory when the break in the ground rule involves actual entry into the therapy space by another person. Other contraventions to this ground rule that do not involve such an entry do not elicit the predicted patient responses. The many other ground rule breaks occurring in the institutional context evoke the predicted responses in the patients' material. In the study, no therapist interventions are found to comply with the communicative therapy requirements for sound interventions; concomitantly it was found that no therapist interventions receive the required derivative validation. The results indicate that it is possible to conduct therapy of a substantially secure frame variety in this institutional context with minimum effort on the part of therapists and given proper training and supervision of therapists in the techniques of communicative psychotherapy. Furthermore the results lend weight to the importance of the communicative methodology for listening to patients' material in psychotherapy in an institutional context. However, further rigorous study of competently performed therapy, executed within the context of a secure frame within an institutional context, is needed in order to demonstrate the benefits of the communicative psychotherapy interventions and interpretations in this context.

The ontological and epistemological assumptions underpinning the empirical study were derived from a scientific realist and a hermeneutical paradigm. Data for the inquiry into the local knowledge of the participants was collected through individual interviews. The data was interpreted, using the grounded theory techniques of constant comparison, coding and compiling theoretical diagrams. Data for the inquiry into the participants' group problem solving practice consisted of video-taped group problem solving processes. This data was analysed, using a multi layered process of progressively deeper interpretation, employing a reading guide technique. Analysis of the research data revealed that the participants perceived a problem as an impediment to satisfactory participation in society. Problem solving was considered as an emotive, cognitive and inter-active process, involving particular role players. This process had a certain structure, involved attitudes and actions and relied on particular resources. Successful problem solving was perceived to result in restoration of social equilibrium. The group problem solving procedure used by the participants consisted of a process of developing a common understanding and group consensus. The strategies employed in the process, the roles played by the participants, the rules adhered to by the participants and the structure underlying the process were all congruent with these aims. There was a mutually reinforcing interrelation between knowledge and practice with reference to the participants’ problem solving.

Linkage in extra-sensory perception

- Description: The aims of this investigation were firstly to find whether 371 subjects, mainly students at Rhodes University, would be able to reproduce target drawings made 470 miles away by an agent in Cape Town, by means of General Extra-Sensory Perception; secondly, to find whether any association existed between any such hit scoring ability shown and personality ratings derived from a battery of personality test material; and thirdly, to find whether providing the subjects with material designed to link them more closely with the agent would increase their scoring rate. The 17,440 drawings returned by the subjects were randomised, and scored against a randomised set of 100 drawings consisting of 50 which had actually been used as targets, intermixed with 50 that were equal in difficulty but which had not been used as targets, and which were inserted merely as controls. Three independent judges assessed the subjects' drawings, and awarded hits in terms of title, shape and association. In the crucial title hits highly significant deviations in favour of the experimental target drawings were found, the control series of drawings showing no such effects. It was found that the subjects' hits were distributed evenly throughout the whole 25 day period of the experiment for each target, and showed no tendency to occur more frequently in the week a particular drawing was being used as a target than in the other weeks when it was not. When the subjects were divided into a high scoring and low-scoring group in terms of their E.S. P. abiility, the high- scoring group proved to be significantly more extraverted (as measured by the Bernreuter B3I scale) than the low- scoring group, confirming a relationship previously reported by Humphrey. In addition the data showed several other relationships, predicted by the work of previous experimenters, although these did not reach the .01 level of statistical significance. To assess the effect of the Linkage Material the subjects were divided into an experimental and a control group. The Experimental Group was supplied with correct Linkage Material, the Control Group with incorrect Linkage Material, which they were led to believe was correct . The two sets of Linkage Material were equated in all other respects. The Experimental Group showed significant improvements in scoring rate when supplied with a photograph and description of the agent, and a probably significant improvement when supplied with squares of handkerchief which had previously belonged to him. When supplied with squares of colour, also appearing on the agent's target sheets, no significant increase in scoring rate took place on the target drawings, but a significant decrease occurred in hits scored on the control targets. By contrast no consistent significant fluctuations were shown by Control Group Subjects, leading to the conclusion that the correctness of the Linkage Material was a factor contributing to its effectiveness. A qualitative examination of the hits scored by the subjects revealed that they tended to reproduce the concepts depicted by the target drawings, rather than the actual shapes drawn by the agent, suggesting that the hits were being produced by a telepathic process rather than a clairvoyant one. The results of the experiment suggest that the subjects were assisted by the Linkage Material to gain access to the agent's mind as a whole, and not to particular items in it. This throws doubt on the correctness of Whately Carington's Association Theory of Telepathy, which the experiment was designed to test.

A qualitative examination of the hits scored by the subjects revealed that they tended to reproduce the concepts depicted by the target drawings, rather than the actual shapes drawn by the agent, suggesting that the hits were being produced by a telepathic process rather than a clairvoyant one. The results of the experiment suggest that the subjects were assisted by the Linkage Material to gain access to the agent's mind as a whole, and not to particular items in it. This throws doubt on the correctness of Whately Carington's Association Theory of Telepathy, which the experiment was designed to test.

- Description: Introduction There is a dearth of research on mental health issues in women offenders in South Africa, especially regarding their socio-demographic backgrounds, offence characteristics, and forensic mental health profiles. Objectives This study examined the psychosocial and forensic mental health profile of women offenders referred by eastern Cape courts for forensic evaluation. A range of socio-demographic, criminological, clinical and forensic mental health variables were systematically explored. Methods A bi-phasic, mixed methods study design was adopted. The clinical and forensic records of all women referred for forensic evaluation to Fort England forensic psychiatric hospital in the Eastern Cape, South Africa were retrospectively reviewed, comprising 173 individual cases in the study period of 1993-2017. Inferential statistical analyses (chi-squared and multivariate logistic regression) were applied to explore relationships between variables and offending outcomes of nterest. Detailed semi-structured interviews were subsequently conducted with a sub-sample of 8 women with mental disorder and violent offending ackgrounds. Interview transcripts thematically analysed. Results Most women came from impoverished and disadvantaged backgrounds. Whilst the majority were first offenders, a high proportion had violent index offences, with murder, attempted murder and assault with intent to do grievous bodily harm accounting for over half of cases. The majority of victims of violence were well known to the perpetrator, especially as biological children, intimate male partners or close family members. Biological children in their first year of life were particularly vulnerable to being victims of homicidal violence. Disproportionately high rates of pre-offence mental illness, alcohol misuse, HIV infection and prior abuse of the offender (especially by intimate male partners) were present. High rates of severe mental disorders (especially psychiatric comorbidity and psychotic-spectrum disorders), and relatively low rates of personality disorders and substance disorders were diagnosed. The majority of women were declared to lack trial competence and criminal capacity, respectively, following forensic evaluation. Women who had backgrounds of prior abuse themselves had over three mes the odds of subsequent violent offending in general, and almost six times the odds of homicidal offending in particular. Homicidal offences were significantly more commonly committed by women with no prior psychiatric history and no psychiatric comorbidity. Women who committed homicide had over eleven times of killing children as opposed to adults. Women over the age of 30 years, and those without psychiatric comorbidity, were significantly less likely to have killed children. Thematic analysis of interviews emphasized the important roles played by gender, self-image, and mental health in violent offending pathways. Conclusions A complex array of socio-demographic, criminological, clinical and forensic variables interact in women offenders of the Eastern Cape referred by courts for forensic evaluation. Exploration of these factors improves understanding of the broader psychosocial context of female offending, and of the personal experiences of the women themselves. This in turn provides an enhanced gender-focus to guide the progressive changes required in policy, legislative, clinical and research endeavours in this field.

Measures included the ImPACT cognitive and symptom composites, and the WMS-III Visual Reproduction and Verbal Paired Associates subtests. The independent and dependent comparative analyses in respect of both nonconcussed and concussed groups, provided cross-validation of poorer acute and/or chronic neuropsychological outcomes for the rugby groups on the ImPACT Reaction Time, Visual Motor Speed, Impulse Control and Symptom composites, and the WMS-III Verbal Paired Associates. The finding of significantly poorer scores on Verbal Paired Associates up to 24 days post concussion for the rugby players versus controls, was longer than the 7 – 10 day recovery period frequently cited in the literature. The overall implication of the study is that even in a group with high cognitive reserve such as these provincial level athletes, there may be prolonged acute recovery, as well as permanent deleterious neuropsychological consequences of cumulative concussive injury in association with a sport such as rugby. Accordingly, the move towards careful individualised postconcussion monitoring of neurocognitive functioning is endorsed, including early identification of any significant permanent reductions in cognitive reserve. Sensitivity of the ImPACT test might be enhanced via inclusion of a verbal associate learning task.
