A best practice guideline for patient-centred care in public hospitals in Nelson Mandela Bay

- Description: In South Africa, the quality of health care is directly related to the concept of patient-centred care and the enactment of the Batho Pele Principles and the Patients’ Rights Charter. The quality of health care delivery has dropped drastically, and reports in the media indicate that public hospitals in the Eastern Cape Province are on the brink of collapse, with thousands of patients being treated in condemned hospitals. Receiving and rendering health care in the face of such challenges, the question arose: “Are patients receiving patient-centred care in public hospitals?” The answer to this rhetorical inquiry appeared to be obvious, but this research study explored and described professional nurses’ perceptions of patient-centred care in public hospitals and their understanding of evidence-based practice and best practice guidelines. The proposed study followed a qualitative, exploratory, descriptive and contextual design. The research population included professional nurses who are employed in public hospitals in Nelson Mandela Bay, and consisted of nurse managers and nurses who work in the wards. The research study consisted of three phases. In Phase 1, semi-structured interviews and focus groups were conducted with nurse managers and professional nurses working in the wards in order to collect data about their perceptions of patient-centred care and their understanding of evidence-based practice and best practice guidelines. The interviews were transcribed and Tesch’s eight steps of data analysis were followed to create meaning from the data collected. Themes were identified and grouped together to form new categories. The researcher ensured the validity of the study by conforming to Lincoln and Guba’s model of trustworthiness, which consists of the following four constructs: credibility, transferability, dependability and confirmability. An independent coder assisted with the coding process. In Phase 2, an integrative literature review was conducted in order to identify previous guidelines regarding best practice for patient-centred care. Relevant guidelines were selected, critically appraised, data was extracted and synthesised for the development of a best practice guideline for patient-centred care. An independent appraiser critically appraised the guidelines, thereby ensuring trustworthiness. In Phase 3, the data in Phase 1 and Phase 2 were integrated to formulate a draft best practice guideline for patient-centred care. The guideline was submitted to an expert panel for review and was modified according to the recommendations of the panel, whereby the best practice guideline for patient-centred care in public hospitals in Nelson Mandela Bay was finalized.

A business model for medical subspecialty training in South Africa

- Description: The shortage of healthcare workers and doctors in the developing world compared to the developed world is a problem, and will continue to be so, due to the continual migration of qualified professionals and the inability of the state to remedy these shortfalls. A shortage of healthcare workers and specialist doctors will seriously hamper the Government’s National Health Insurance (NHI) plan, as well as the sustainability of the private health care sector. In addition, the duration of medical training in South Africa is exceptionally long. The three major hospital groups and other private corporates have over the last number of years taken limited initiative to fund education projects in conjunction with the academic institutions. However, these projects have been poorly focused and have been managed in an unstructured and detached manner. There is a desire from the private sector to get involved in these projects on a much larger scale through more formalised structures. Given this background, the primary objective of this research is to develop a business model for medical subspecialty training to complement the current academic subspecialty training in South Africa. A trial model for training subspecialists in reproductive medicine was developed as a first attempt to address the threatening shortages and training duration. This trial programme is the basis of this research. A two-phased process was used in collecting data. In Phase 1 data was collected from stakeholder groups. The results of this survey assisted in generating variables to include in the measuring instrument for the survey in Phase 2. In Phase 2 the perceptions and expectations of sub-specialists (reproductive subspecialists and cardiologists) regarding sub-specialty training was collected. The results of the demographic variables confirm the aging profile of subspecialist and the need to ensure succession. The results further showed that cardiologists and reproductive subspecialist expectations of the training of subspecialists are very similar except for their expectations on the training duration. Reproductive subspecialist respondents agreed more than cardiology subspecialist respondents that the training duration is too long. The biggest gap between perceptions and expectations is also with the factor training. The results showed that the expectations of subspecialists are not met for training. From these results a business model for the training of medical subspecialists is proposed. This proposed business model can play a complementary role to the existing state controlled system and form the bases of Public Private Partnerships (PPP) in medical training. This proposed business model will fit a developing country were the focus is on primary healthcare, with financial and capacity constraints. The proposed model would require role players to bring about change to accommodate a larger scale Public Private Partnership (PPP) to ensure the implementation of the model. The proposed decentralised business model for training subspecialists would allow the trainee subspecialist to practise as a specialist thus maintaining an acceptable income, and enable an expedited completion and lead to a wider dissemination of medical expertise that can be delivered in a wider national foot print. It further will provide for a structured Public Private Partnership.

- Description: Post-apartheid South Africa is witnessing an education crisis of significant proportions. The new outcomes-based education system has failed to deliver and universities are suffering the consequences of under-preparation of learners for tertiary studies especially in mathematics. The educator corps is lacking and it has become common practice for universities to deploy augmented programmes in mathematics for secondary school learners in the surrounding areas. This thesis describes a particular approach of blended learning, devised for the Incubator School Project (ISP), an initiative of the Nelson Mandela Metropolitan University (NMMU) in the Eastern Cape of South Africa. The defining feature of this blended approach is that it incorporates DVD technology, which offers an affordable and accessible option for the particular group of learners and the schools they attend. The thesis poses the research question: How did the use of the DVD approach within a blended learning environment support the learning of mathematics? This case study explores the particular blended approach and reports six fold on the approach – qualitatively based firstly on a questionnaire completed by learners and secondly on interviews of learners, thirdly on the facilitators reports, fourthly quantitatively on learner performance before and after the intervention. Fifthly six schools are used as a case study where the mathematics performance of the learners who participated in the ISP is compared to those who did not participate in the ISP. Finally the scope of blending of this model is evaluated by means of a radar chart, adapted from an existing radar measure. This research revealed that using the DVD approach within a blended learning environment did lead to an improvement in learners perceptions about mathematics, an improvement in the manner in which they learned mathematics, an extension in their mathematics knowledge and provided learners with a supportive environment in which to learn mathematics. The elements which supported learning in this approach are presented. The findings of the study suggest that this approach impacted favourably on the mathematics learning and enhanced the mathematics learning and performance of these learners. Recommendations are offered for practice, teachers and schools and for further research possibilities.

A chemo-enzymatic process for the production of beta-thymidine, a key intermediate in antiretrovirol manufacture

- Description: The socio-economic impact of HIV/AIDS on South Africa has resulted in lower gross domestic product, loss of skills in key sectors such as education, and increased health-care costs in providing access to treatment. Currently active pharmaceutical ingredients (API’s) such as stavudine (d4T) and azidothymidine (AZT) are imported from India and China, while formulation is conducted locally. A strategy was initiated between CSIR Biosciences and LIFElab under the auspices of Arvir Technologies to investigate the feasibility of local antiretroviral manufacture (d4T and AZT) or the manufacture of a key intermediate such as β- thymidine (dT). Several advantages associated with successful implementation of this strategy include ensuring a local supply of API’s, thus reducing reliance on procurement from foreign sources and reducing the effect of foreign exchange rate fluctuations on providing cost effective access to treatment. A local supply source would also reduce the imports and thus aid the balance of payments deficit, and in addition to this, provide stimulus in the local pharmaceutical manufacturing industry (which has been in decline for several decades), resulting in increased skills and employment opportunities. This thesis describes the development of a superior chemo-enzymatic process for the production of β-thymidine (72 percent yield, prior to isolation), a key intermediate in the preparation of anti-retrovirals. Alternative processes based purely on chemical or bioprocess transformations to prepare either 5-methyluridine (5-MU) or dT suffer from several disadvantages: lengthy transformations due to protection/deprotection strategies, low selectivties and product yields (30 percent in the chemical process) and isolation of the product from dilute process streams requiring the use of large uneconomical reactors (bioprocesss). This contributes significantly to the cost of d4T and AZT manufacture. Our novel chemoenzymatic process comprises of a biocatalytic reaction for the production of 5-MU, with subsequent chemical transformation into dT (3 steps) negating and circumventing the limitations of the chemical or bioprocess routes. During the course of this project development, the β-thymidine selling price declined from 175 $/kg (2005) to 100 $/kg (2008). However, the process described in this work is still competitive based on the current β- thymidine selling price of 100 $/kg. The process economics show that with further optimization and increasing the isolated dT yield from 70 percent to 90 percent, the variable cost decreases from 136 $/kg to 110 $/kg. The increase in isolated yield is highly probable, based on solubility data of β-thymidine. The decrease in β-thymidine selling price and technological improvement in dT manufacture should translate into lower API manufacture costs and more cost effective access to treatment. Our novel biocatalytic process producing 5-MU uses a coupled enzyme system employing PNP, Purine Nucleoside Phosphorylase and PyNP, Pyrimidine Nucleoside Phosphorylase. The overall transglycosylation reaction may be decoupled into the phosphorolysis reaction (PNP) and synthesis reaction (PyNP). During the phosphorolysis reaction, guanosine is converted into guanine and ribose-1-phosphate (R-1-P) in the presence of PNP enzyme. The reaction intermediate R-1-P is then coupled to thymine in the presence of PyNP enzyme during the synthesis reaction, producing 5-MU. The process was scaled up from lab-scale to bench-scale (10 - 20 L) and demonstrated to be robust and reproducible. This is evident from the average guanosine conversion (94.7 percent ± 2.03) and 5-MU yield (88.2 percent ± 6.21) and mole balance (104 percent ± 7.61) which were obtained at bench-scale (3 replicates, 10 L). The reaction was carried out at reactor productivities of between 7 – 11 g.L-1.h-1. - Description: The human immunodeficiency virus / acquired immune deficiency syndrome pandemic (HIV/AIDS) continues to increase in prevalence worldwide, particularly in South Africa. There is a concurrent and distinct increase in the prevalence of HIV/AIDS and HIV-related diseases in the paediatric population in South Africa, particularly those using public sector health services, with a corresponding increase in morbidity and mortality rates (Abdool Karim & Abdool Karim, 2010:363), which impacts greatly on paediatric healthcare services. Adding to this, the provision of paediatric antiretroviral care has numerous stumbling blocks, not least of which is lack of decentralisation of facilities to provide treatment. There is the additional shortage of staff, which includes staff that are comfortable dealing with children, lack of training programmes on the provision of antiretroviral therapy to children, and minimal on-site mentorship of staff regarding HIV/AIDS disease in children. This lack of capacity in the healthcare system means that not all of those who require treatment will be able to access it, and this is particularly pertinent to paediatric patients (Meyers et al., 2007:198). Therefore the purpose of this research was to develop a nursing model that would assist healthcare professionals, in particular professional nurses, to optimise the comprehensive treatment, care and support for HIV-positive children who require antiretroviral therapy at PHC clinics. To achieve the purpose of this study, a theory-generating design based on a qualitative, explorative, descriptive and contextual approach was implemented by the researcher to gain an understanding of how the healthcare professionals and parents/caregivers of HIV-positive children experienced the comprehensive treatment, care and support provided at primary healthcare clinics. The information obtained was used to develop a chronic care coordination model for the optimisation of comprehensive treatment, care and support for HIV-positive children requiring antiretroviral therapy. The study design comprised the following four steps: Step One of the research design focused on the identification, classification and definition of the major concepts of the study. This involved describing and selecting the research population and the sampling process prior to conducting the field work which comprised in-depth interviews with two groups of participants, namely healthcare professionals and parents/caregivers who accompany their HIV-positive children to PHC clinics in order to receive antiretroviral therapy. Step Two of the research design focused on the development of relationship statements in order to bring clarity and direction to the understanding of the phenomenon of interest. Step Three of the design concentrated on the development and description of the chronic care coordination model for optimising comprehensive treatment, care and support for HIV-positive children who require antiretroviral therapy in order to ensure a well-managed child on ART. A visual representation of the structure of the model for chronic care coordination was given and described as well as a detailed description of the process of the model. Step Four was the last step of the research design and its focus was the development of guidelines for the operationalisation of the model for chronic care coordination for the optimisation of comprehensive treatment, care and support for HIV-positive children requiring antiretroviral therapy at PHC clinics. - Description: The human immunodeficiency virus / acquired immune deficiency syndrome pandemic (HIV/AIDS) continues to increase in prevalence worldwide, particularly in South Africa. There is a concurrent and distinct increase in the prevalence of HIV/AIDS and HIV-related diseases in the paediatric population in South Africa, particularly those using public sector health services, with a corresponding increase in morbidity and mortality rates (Abdool Karim & Abdool Karim, 2010:363), which impacts greatly on paediatric healthcare services. Adding to this, the provision of paediatric antiretroviral care has numerous stumbling blocks, not least of which is lack of decentralisation of facilities to provide treatment. There is the additional shortage of staff, which includes staff that are comfortable dealing with children, lack of training programmes on the provision of antiretroviral therapy to children, and minimal on-site mentorship of staff regarding HIV/AIDS disease in children. This lack of capacity in the healthcare system means that not all of those who require treatment will be able to access it, and this is particularly pertinent to paediatric patients (Meyers et al., 2007:198). Therefore the purpose of this research was to develop a nursing model that would assist healthcare professionals, in particular professional nurses, to optimise the comprehensive treatment, care and support for HIV-positive children who require antiretroviral therapy at PHC clinics. A collaboratively constructed process model for understanding and supporting the work of the community volunteer in a community school

- Description: This thesis sought to explore how community volunteers could be recruited, supported and sustained to assist a community school operating in difficult socioeconomic conditions in achieving basic school functionality. Through a collaborative process, the participants in the study attempted to address a significant gap in the literature, namely how this could be achieved in a way that would be beneficial both to the community volunteers and to the school itself. Based on existing literature, the vast majority of South African schools are struggling to reach the basic functionality levels required in terms of legislation. My interest in this topic was piqued while serving as principal of one such school; hence the focus in this thesis on whether schools would benefit in terms of achieving functionality if they partnered with the communities in which they are located. However, communities are seldom actively involved in the schools and school activities on an ongoing daily basis. In this thesis, I argue for an opportunity for schools and the community to collaborate in a way that would be mutually beneficial. In this, I was guided by the School-Based Complementary Learning Framework (SBCLF) in gaining a greater understanding of how multiple stakeholders could support a school to obtain basic functionality. A key stakeholder is the community in which a school is located, and the multidimensional framework provided a framework to understand why the community would want to get involved in the school. Following a Participatory Action Learning and Action Research (PALAR) design, I recruited 15 community volunteers from the existing pool at my then school; some of whom had been volunteering for over twelve years. We formed an action learning set where we collaboratively sought to understand the processes and conditions needed to recruit, support and sustain community volunteers and their involvement in the school. From this action learning set emerged a key advisors’ set, comprising five members of the action learning set, who were entrusted with the responsibility of planning, preparing and analysing the action learning set meetings. Transcripts and visual artefacts from the action learning set meetings and a focus group meeting of the school management team were analysed to generate data, complemented by secondary sources, such as documents. This participatory approach to data generation allowed the voice of every participant to be heard; agency was increased through active participation; and the sense of affiliation to the group was deepened. The iterative design of the research process further ensured that the participants also engaged in a critical discourse analysis of the emerging data, of which the trustworthiness was enhanced through the use of dialogic and process, catalytic, rhetoric, democratic and outcome validity. The emergence of the data through this collaborative engagement was underpinned by the ethical values of mutual respect; equality and inclusion; democratic participation; active learning; making a difference; collective action; and personal integrity. The findings revealed that community volunteers did add immense value to the school by supporting teaching and learning processes. However, the community volunteers also harboured expectations of material support and opportunities to develop skills. In addition, the study revealed that the hierarchical culture and structures present in most South African schools need to become more democratic and collaborative, with those working to make the school more functional, including community volunteers, being valued, acknowledged and supported. The participants also constructed their understanding of what a community school should be and do and how it should serve the interests of the children from the community. A process model was constructed from these findings regarding on ways to recruit, sustain and support community volunteers involved in community schools, specially designed so that schools could adapt it to suit individual contexts. This study is unique; I am not aware of any similar study ever having been conducted in a community school in South Africa. Furthermore, the collaborative approach used in the study helped ensure that the methodology used could be of value to principals and other school stakeholders in addressing the various complex challenges that confront schools in these contexts. Also, the findings will add to the theoretical body of knowledge around volunteerism, especially in difficult socioeconomic conditions.

A combination of platinum anticancer drugs and mangiferin causes increased efficacy in cancer cell lines

- Description: This thesis mainly deals with some biochemical aspects regarding the efficacy of novel platinum anticancer compounds alone and in combination with mangiferin, as part of a broader study in which both chemistry and biochemistry are involved. Various novel diamine and N-S donor chelate compounds of platinum II and IV have been developed in which factors such as stereochemistry, ligand exchange rate and biocompatibility were considered as additional parameters. In the first order testing, each of these compounds was tested with reference to their “killing” potential by comparing their rate of killing, over a period of 48 hours with those of cisplatin and oxaliplatin. Numerous novel compounds were tested in this way, using the MTT cell viability assay and the three cancer cell lines MCF7, HT29 and HeLa. Although only a few could be regarded as equal to or even better than cisplatin, CPA7 and oxaliplatin, the testing of these compounds on cancer cells provided useful knowledge for the further development of novel compounds. Three of the better compounds, namely Yol 25, Yol 29.1 and Mar 4.1.4 were selected for further studies, together with oxaliplatin and CPA7 as positive controls, to obtain more detailed knowledge of their anticancer action, both alone and when applied in combination with mangiferin. In addition to the above, resistant cells were produced for each of the three different cell lines tested and all the selected compounds, both in the presence and absence of mangiferin. The effects of these treatments on the activation of NFĸB when applied to normal and resistant cell lines were also investigated. All the compounds induced apoptosis in the cell lines tested as well as alter the DNA cycle at one or more phase. Additionally, combination of these compounds with mangiferin enhanced the above-mentioned effects. Mangiferin decreases the IC50 values of the platinum drugs by up to 3.4 times and, although mangiferin alone did not induce cell cycle arrest, the presence of mangiferin in combination with oxaliplatin and Yol 25 shows an earlier and greatly enhanced delay in the S-phase, while cells treated with CPA7, Yol 29.1 and Mar 4.1.4 in combination with mangiferin showed a later, but greatly enhanced delay in the S-phase. It was also found that mangiferin acts as an NFĸB inhibitor when applied in combination with these drugs, which, in turn, reduces the occurrence of resistance in the cell lines. Resistance to oxaliplatin was counteracted by the combination with mangiferin in HeLa and HT29, but not in MCF7 cells, while resistance to CPA7 was only counteracted in the MCF7 cell line. - Description: The citrus industry is one of South Africa's largest agricultural sectors in terms of export earnings with lemon fruits and juice as a trendsetter due to their high grade quality. According to growers, the Eastern Cape Province of South Africa alone produces an excess of about 10-14,000 tons of lemon juice which is presently of no economic value due to the sour taste and “bitterness”. As a result of this excess and in order to make use of the polyprotic acids naturally occurring in the lemon juice, four fertilizer nutrient mixtures are formulated, using lemon juice as base. From a conceptual scientific approach, characterization (physico-chemical and functional properties determinations) of Eureka Lemon fruit juices were undertaken, followed by smaller scale batch formulation experiments. On the basis that these lemon juice-based fertilizer mixtures are prepared following standard liquid fertilizer formulation guidelines, a field test was conducted to evaluate their potential effectiveness to influence plant growth. A growth chamber testing on tomato plants revealed high growth response (> 99.9 % certainty) potential in two of the semi-organic mixtures formulated while the organic mixture showed a relatively good growth rate as compared to the control (pure tap water). According to statistical analysis (ANOVA) comparison, two of the semi-organic mixtures performed considerably better than the two commercial samples evaluated. Potential benefits profoundly associated with these nutrient mixtures as compared to similar liquid fertilizer products on the market is that most nutrients are chelated and dissolved in solution. Also, the mixtures contain all necessary nutrients including plant growth substances required for healthier plant growth. The most important socioeconomic impact is the value addition to the technology chain in the citrus industry. The use of fluid fertilizers in significant quantities is less than twenty years old. Nevertheless, growth has been so rapid that in South Africa demand for mixed liquid fertilizer has greatly increased from 90 000 tons NPK & blended micronutrients in 1955 to more than 600 000 per annum tons today (Report 41/2003, Department of Minerals and Energy). The liquid fertilizers market is sparsely specialized with major competitors like Omnia, Kynoch and Foskor supplying more than 50 % of the market demand. Amongst the nutrient mixtures formulated, mixture one is an NPK (1-1-2) based nutrient mixture containing both secondary nutrients (0.5 % Mg & 1.0 % Ca) and seven micronutrients (0.1 % Fe, 0.05 % Cu, 0.05 % Zn, 0.05 % Mn, 0.02 % B, 0.0005 % Mo and 0.0005 % Co). The composition of this mixture offers the formula a potential to be used as a general purpose (all stages of plant growth) fertilization mixture in view of its balanced composition (containing all essential plant nutrients). Mixture two contains essentially the micronutrients and in higher concentrations (0.3 % Fe, 0.3 % Cu, 0.1 % Zn, 0.2 % Mn, 0.02 % B, 0.0005 % Mo and 0.0005 % Co) as compared to mixture one except for boron, molybdenum and cobalt. The concentration of the micronutrients contained in this mixture is adequately high which offers a potential for it to be used in supplementing nutrition in plants with critical micronutrient-deficient symptoms. Mixture three is very similar to mixture two (1.0 % Fe, 0.05 % Cu, 0.05 % Zn, 0.05 Mn, 0.05 % B, 0.0005 % Mo and 0.0005 % Co) except that the concentrations of all seven micronutrients are considerably less than those of contained in mixture two. However, the concentration of iron in this mixture is as high as 1.0 %. The mixture has a potential to be used in high iron-deficient situations. Mixture four is an organic formula with relatively low nutrient concentrations (NPK-0.02-0.02-1, 0.27 % Mg, 0.02 % Ca, 0.008 % Fe, 0.26 % Cu, 0.012 % Zn, 0.009 % Mn). Nevertheless, this mixture is appealing for organically grown crops where the use of chemicals is prohibited by standards. These lemon juice-based nutrient mixtures were further characterized and tested for stability and storability over a period of eight weeks. This study revealed no major change in the physical quality (colour, pH and “salt out” effect). The basic formulation methodology is a two-step procedure that involves filtration of the lemon juice to remove membranous materials, mixing at ambient temperature and stabilization of the nutrient mixtures. However, for the organic nutrient formula mix, filtration follows after extraction of nutrients from plant materials using the lemon juice.

Amongst the nutrient mixtures formulated, mixture one is an NPK (1-1-2) based nutrient mixture containing both secondary nutrients (0.5 % Mg & 1.0 % Ca) and seven micronutrients (0.1 % Fe, 0.05 % Cu, 0.05 % Zn, 0.05 % Mn, 0.02 % B, 0.0005 % Mo and 0.0005 % Co). The composition of this mixture offers the formula a potential to be used as a general purpose (all stages of plant growth) fertilization mixture in view of its balanced composition (containing all essential plant nutrients). Mixture two contains essentially the micronutrients and in higher concentrations (0.3 % Fe, 0.3 % Cu, 0.1 % Zn, 0.2 % Mn, 0.02 % B, 0.0005 % Mo and 0.0005 % Co) as compared to mixture one except for boron, molybdenum and cobalt. The concentration of the micronutrients contained in this mixture is adequately high which offers a potential for it to be used in supplementing nutrition in plants with critical micronutrient-deficient symptoms. Mixture three is very similar to mixture two (1.0 % Fe, 0.05 % Cu, 0.05 % Zn, 0.05 Mn, 0.05 % B, 0.0005 % Mo and 0.0005 % Co) except that the concentrations of all seven micronutrients are considerably less than those of contained in mixture two. However, the concentration of iron in this mixture is as high as 1.0 %. The mixture has a potential to be used in high iron-deficient situations. Mixture four is an organic formula with relatively low nutrient concentrations (NPK-0.02-0.02-1, 0.27 % Mg, 0.02 % Ca, 0.008 % Fe, 0.26 % Cu, 0.012 % Zn, 0.009 % Mn). Nevertheless, this mixture is appealing for organically grown crops where the use of chemicals is prohibited by standards. These lemon juice-based nutrient mixtures were further characterized and tested for stability and storability over a period of eight weeks. This study revealed no major change in the physical quality (colour, pH and “salt out” effect). The basic formulation methodology is a two-step procedure that involves filtration of the lemon juice to remove membranous materials, mixing at ambient temperature and stabilization of the nutrient mixtures. However, for the organic nutrient formula mix, filtration follows after extraction of nutrients from plant materials using the lemon juice.

This thesis reveals that three major forms of market abuse, namely insider trading, prohibited trading practices (trade-based market manipulation) and the making or publication of false, misleading or deceptive statements, promises and forecasts relating to listed securities (disclosure-based market manipulation) are prohibited in South Africa. However, although South Africa has had market abuse legislation for about 30 years, and must be commended for its great effort to enhance market integrity by combating market abuse practices, the enforcement of such legislation is still problematic. Moreover, in spite of the fact that there is no empirical data or accurate figures quantifying the occurrence and extent of market abuse activities in the South African financial markets, this thesis submits that market abuse practices are still to be completely eradicated. Accordingly, this thesis suggests that the aforementioned problem might have been aggravated by inter alia, various gaps, flaws and/or inconsistent implementation and enforcement of the market abuse legislation in South Africa. To this end, the anti-market abuse enforcement framework under the Securities Services Act 36 of 2004 is analysed to investigate its adequacy. The co-operation and role of the Financial Services Board, the courts, the Directorate of Market Abuse and other relevant stakeholders is also examined and discussed. Moreover, the co-operation between the Financial Services Board and similar international agencies is discussed to gauge its effectiveness in relation to the combating of cross-border market abuse practices. The adequacy of the awareness and preventative measures in place to curb market abuse practices is also investigated to determine whether such measures are robust enough to combat other new challenges that were posed by the 2007 to 2009 global financial crisis. Furthermore, a comparative analysis is undertaken of the enforcement of the market abuse prohibition in other jurisdictions, namely the United States of America, the United Kingdom, the European Union and Australia. This was done to investigate the relevant lessons that can be learnt or adopted from these jurisdictions. The thesis further discusses the adequacy of the recently introduced provisions of the Financial Markets Bill as well as the subsequent market abuse provisions of the Financial Markets Bill 2012. The thesis highlights that the aforementioned Bills are positive attempts by the policy makers to improve the enforcement of the market abuse provisions in South Africa. Nonetheless, the thesis reveals that most of the shortcomings contained in the Securities Services Act 36 of 2004 were duplicated in the Financial Markets Bill and the Financial Markets Bill 2012. To date, however, the more comprehensive international standardised assessment called the Programme for International Student Assessment (PISA), has not been administered in South Africa. The main aim of this research study was to investigate and scientifically explore the real situation in terms of language, mathematics and science literacy knowledge and skills of Grade 9 learners in South Africa and to draw a comparison between Grade 9 learners from secondary schools in the Port Elizabeth district in South Africa and their international counterparts, using the PISA standardised international assessment. In addition, the aim of the study was to determine the actual language, mathematics and science literacy skills and knowledge acquired by participants in this study. Quantitative data collection was done by administering a modified version of the Program for International Student Assessment (PISA) to learners from eight schools in Port Elizabeth, supplemented with questionnaires completed by participating learners and school principals. Findings revealed that the South African learners sampled, ranked in the bottom percentile of participating countries in reading, mathematics and science. The study also exposed the glaring inequalities still prevalent in South African education today, 17 years into democracy. The implications point to a serious investigation into the societal and political factors responsible for the discrepancies in the South African educational system at present.

- Description: The restructuring of higher education through incorporations and mergers has attracted a lot of attention over the past few years in South Africa. These incorporations and mergers have displaced institutions of higher education and positioned them in new organisational homes, thus subjecting faculties, schools and departments to a process of relocation, new knowledge acquisition, identity change and meaning-making processes. The merger has resulted in three types of universities; i.e. traditional universities, comprehensive universities and universities of technology. The introduction of the comprehensive university as a new institutional type has brought with it questions about the idea of the university and the purpose of higher education in general. Mergers in particular have initiated conversations about sense-making and meaning during change. Amidst all this, people within the merging institution have been confronted with a new organisation with which they have to identify. At universities in particular, questions about academic identity and organisational identity have become unavoidable. The boundaries that gave definition to a university have been (re)moved. The structure of the university, as it was known, has changed. Hence, in the newly merged NMMU, academics are in the process of internalising and giving meaning to the new organisational values and norms of a comprehensive university. Needless to say, the challenges facing the newly merged NMMU are cultural, structural and geographic. Bringing together different institutional and personal cultures involves a human dimension that needs to be nurtured by trying to form a coherent and cohesive organisation that is created from culturally diverse and uncomplementary institutions. Another challenge is bringing together different organisational structures, systems and programmes that are informed by different institutional cultures. Furthermore, the challenge of having multiple campuses that are geographically separated exacerbates the situation. Along with all these challenges, the NMMU has the task of constructing an integrated institutional identity through organisational forms and programme models that will embody the multiple functions that are typical of a comprehensive university. The aim of the current study was to explore how the meanings that academics assign to the notion of a comprehensive university are instrumental in constructing an organisational identity; describing in detail how at the NMMU academics make meaning of the comprehensive university and how that meaning-making process influences the construction of an organisational identity; and formulating recommendations based on the qualitative findings and quantitative results of the research. In an effort to achieve the aim alluded to above, this study employed the mixed methods approach that used a sequential, exploratory, transformative design. The complexity of the study was such that it required to be investigated through qualitative and quantitative analytical methods in order to confirm, triangulate and obtain a holistic picture of the situation under investigation. The sample for the qualitative interviews consisted of thirteen purposefully selected academics from all levels at the NMMU. The interviews were transcribed and coded into themes, categories and sub-categories. These themes were then developed and translated into statements for the questionnaire that was administered randomly to all NMMU academics. A total of 108 academics responded to the questionnaire. The responses to the questionnaire were analysed using the SPSS programme. The findings and results of the study revealed that there was a fairly common understanding of the term comprehensive university among academics. However, the details about its procedures appeared to be the privileged ownership of management. This situation mitigated the necessity for a sense-making process that would allow for negotiation, modification and alteration of already held assumptions. A pertinent concern amongst academics was the neglect of the ‘human factor’ during the change process. The management style also came under scrutiny, especially in terms of the facilitation and mediation of change. There was a consensus on the call for cohesion and unity that was believed to be one of the main features that would make the construction of the NMMU organisational identity possible. The vision, mission and values of the NMMU were believed to be central to the creation of cohesion and unity, which would subsequently result in the birth of an organisational culture that could inform the organisational identity of the NMMU. Strategies to actualise and realise the organisational identity were proposed by participants. Notwithstanding, the impact of the merger was identified as having a major influence in shaping the organisational identity of the NMMU.

There was a consensus on the call for cohesion and unity that was believed to be one of the main features that would make the construction of the NMMU organisational identity possible. The vision, mission and values of the NMMU were believed to be central to the creation of cohesion and unity, which would subsequently result in the birth of an organisational culture that could inform the organisational identity of the NMMU. Strategies to actualise and realise the organisational identity were proposed by participants. Notwithstanding, the impact of the merger was identified as having a major influence in shaping the organisational identity of the NMMU.

Customers accessing electronic product catalogue information on a mobile device face a number of challenges such as a long list of products presented on a small screen and a longer information download time. These challenges mainly originate from the limiting and dynamic nature of the mobile apps operating environment, for example, dynamic location, bandwidth fluctuations and, diverse and limited device features, collectively referred to as context. The goal of this research was to design and implement a context-aware model that can be incorporated into an m-commerce application in order to improve the presentation of product catalogue information on m-commerce storefronts. The motivation for selecting product catalogue is prompted by literature which indicates that improved presentation of information in m-commerce (and e-commerce) applications has a positive impact on usability of the websites. Usable m-commerce (and e-commerce) websites improve efficiency in consumer behaviour that impacts sales, profits and business growth. The context-aware model aimed at collecting context information within the user environment and utilising it to determine optimal retrieval and presentation of product catalogue in m-commerce. An integrated logical context sensor and Mathematical algorithms were implemented in the context-aware model. The integrated logical context sensor was responsible for the collection of different types of predetermined context information such as device specification or capabilities, connection bandwidth, location and time of the day as well as the user profile. The algorithms transformed the collected context information into usable formats and enabled optimal retrieval and presentation of product catalogue data on a specific mobile device. Open-source implementation tools were utilised to implement components of the model including: HTML5, PhP, JavaScript and MySQL database. The context-aware model was incorporated into an existing m-commerce application. Two user evaluation studies were conducted during the course of the research. The first evaluation was to evaluate the accuracy of information collected by the context sensor component of the model. This survey was conducted with a sample of 30 users from different countries across the world. In-between the context sensor and main evaluation surveys, a pilot study was conducted with a sample of 19 users with great experience in mobile application development and use from SAP Next Business and Technology, Africa. Finally an overall user evaluation study was conducted with a sample of 30 users from a remote area called Kgautswane in Limpopo Province, South Africa. The results obtained indicate that the context-aware model was able to determine accurate context information in real-time and effectively determine how much product information should be retrieved and how the information should be presented on a mobile device interface. Two main contributions emerged from the research, first the research contributed to the field of mobile Human Computer Interaction. During the research, techniques of evaluating and improving usability of mobile applications were demonstrated. Secondly, the research made a significant contribution to the upcoming field of context-aware computing. The research brought clarity with regard to context-aware computing which is lacking in existing, current research despite the field’s proven impact of improving usability of applications. Researchers can utilise contributions made in this research to develop further techniques and usable context-aware solutions.

It is vital that local government understands and contextualises these challenges, so that appropriate interventions may be developed. The widespread recent service delivery protests which, in many instances, have turned violent, have sounded an alarm that cannot be ignored. If local government is already struggling to fulfill its traditional mandate of service delivery, then it would find it difficult to spearhead social and economic transformation and development. This study proposes to provide a brief historical background on the evolution and transformation of local government in South Africa. Issues such as the new status and developmental mandate of local government, the extent to which local government has succeeded in complying with its developmental mandate, as well as the challenges it has encountered along the way, will be addressed. Recommendations are presented on how the existing status quo can be changed to enhance service delivery and development and enable low government to fulfil its developmental role more efficient and effectively, with particular reference to the Nelson Mandela Metropolitan Municipality. The empirical survey and research methodology employed in the study is described. This is followed by the operationalisation of the survey questionnaire used for gathering the data needed for analysis. The research findings of the empirical survey are then statistically analysed and reported. The concept of models is introduced, and selected models are explained. This is followed by an explanation of the proposed normative model for monitoring and evaluating service delivery and development in the Nelson Mandela Metropolitan Municipality, for possible replication in other South African municipalities. Various recommendations flowing from the results of the empirical study, namely the responses made by the respondents during the empirical survey, are proposed in the final chapter. If adopted, these recommendations will enable the Nelson Mandela Metropolitan Municipality to deal with the developmental challenges facing it, ultimately rendering the Municipality a more efficient and effective developmental agent.

Children are involved in a number of child-labour practices such as prostitution in the commercial and tourism sex industry, forced begging on the streets, and forced soldiering. They may be used as camel jockeys, domestic servants, farm labourers/herders, mine labourers, produce porters, roadside sellers/street vendors, sweetshop-industry labourers, cooks and porters for rebels. The persistent exploitation of children involved in hazardous work and conditions has become overwhelming in Uganda. Poverty as one of the major causes for the growing numbers of child labourers in the agricultural sector in Uganda has caused a number of children to engage in child-labour activities to earn extra income for household survival. Many children have opted for partial attendance in school, eventually dropping out. Parents have also frequently influenced children to work on family farms, thus contributing to the children dropping out of school. Child-labour practices have become entrenched in the social and moral fabric of Ugandan society, and for this reason, research endeavours to uncover ways and methods to reverse this situation. The main objectives of this research were to establish the impact of poverty on child labour, to assess the effect of the social and cultural setup on child labour, to find out the impact of child-labour legislation enforcement, to determine the 5 influence of the HIV and AIDS pandemic on child labour, to establish the effect of the educational system and technological advancement on child labour, to establish the level of awareness of human rights in the community, and to establish the impact of human rights activists on the prevention of child labour. The study was undertaken in Masindi District in Budongo Sub County, in three parishes, Nyabyeya, Nyantonzi and Kasongoire. The respondents used for the study included child labourers, their parents, farmers, and community leaders. The method used to get to the sample was purposive sampling. Data was collected using questionnaires for written answers and a tape recorder for oral answers. Both primary and secondary data was collected, verified, edited, checked, coded, analysed, and then exported to Excel and SPSS. Collecting the data was a challenging exercise for the researcher. Experiences were varied, in the hospitality and willingness of respondents to learn more about child issues. Although respondents were willing to participate in the data collection exercise, social and cultural values did not permit all of them to share their views with the researcher. To collect data from respondents, the researcher had to ensure that remuneration was in place at the end of the exercise. The respondents filled out the questionnaires only after learning of the availability of a reward for every questionnaire answered. More setbacks were the need to travel long distances, and enduring the poor infrastructure, poor sanitation, and epidemic outbreaks, some of which diluted the quality of data collected. During group interviews, most parents were not entirely truthful about involving their children in child-labour activities. Although most respondents had an idea of what child labour is, their ignorance levels on the topic prevented them from stopping their children from working. The major findings of the research were that the cultural, social and economic setup of the community in the study area favoured child labour, although the child-labour legislation is against using children as labourers. Various ethnicities 6 in the study area considered a person between the ages of 5 and 12 years to be a child, yet the Constitution of Uganda dictates the age of childhood to be below 18 years. This causes conflict in the definition of who a child is. Although parents were aware of the Universal Primary Education (UPE) regulation penalties for not taking children to school, they still permitted children to engage in child-labour activities, and little has been done by government to curb the culprits. Awareness of the Sub Counties and Credit Co-operatives (SACCOs) and their implementation has not helped to reduce poverty in the area studied, resulting in an increased school dropout rate among school-going-age children, as well as more child-labour activities. The major conclusion of the study was that little has been done to increase the awareness levels of the teachers, parents and their children about child labour and its legislation, their knowledge of and involvement in micro-finance institutions in the community, and the availability of vocational training institutions. Little has therefore been done to reduce child-labour activities, improve the economic status of the community, and improve their human resource skills. The major recommendations of the research to the study are that culture should not override the Constitution as far as the definition of age limit is concerned. The government should carry out stakeholder analyses, and implement a life-skills and sensitisation programme in order to improve child participation in the Universal Primary Education (UPE) programme. Government should ensure that the society is given information about basic accounting, project planning and management skills, in order to be effectively involved in the economic programmes of SACCOs.

Parents have also frequently influenced children to work on family farms, thus contributing to the children dropping out of school. Child-labour practices have become entrenched in the social and moral fabric of Ugandan society, and for this reason, research endeavours to uncover ways and methods to reverse this situation. The main objectives of this research were to establish the impact of poverty on child labour, to assess the effect of the social and cultural setup on child labour, to find out the impact of child-labour legislation enforcement, to determine the 5 influence of the HIV and AIDS pandemic on child labour, to establish the effect of the educational system and technological advancement on child labour, to establish the level of awareness of human rights in the community, and to establish the impact of human rights activists on the prevention of child labour. The study was undertaken in Masindi District in Budongo Sub County, in three parishes, Nyabyeya, Nyantonzi and Kasongoire. The respondents used for the study included child labourers, their parents, farmers, and community leaders. The method used to get to the sample was purposive sampling. Data was collected using questionnaires for written answers and a tape recorder for oral answers. Both primary and secondary data was collected, verified, edited, checked, coded, analysed, and then exported to Excel and SPSS. Collecting the data was a challenging exercise for the researcher. Experiences were varied, in the hospitality and willingness of respondents to learn more about child issues. Although respondents were willing to participate in the data collection exercise, social and cultural values did not permit all of them to share their views with the researcher. To collect data from respondents, the researcher had to ensure that remuneration was in place at the end of the exercise. The respondents filled out the questionnaires only after learning of the availability of a reward for every questionnaire answered. More setbacks were the need to travel long distances, and enduring the poor infrastructure, poor sanitation, and epidemic outbreaks, some of which diluted the quality of data collected. During group interviews, most parents were not entirely truthful about involving their children in child-labour activities. Although most respondents had an idea of what child labour is, their ignorance levels on the topic prevented them from stopping their children from working. The major findings of the research were that the cultural, social and economic setup of the community in the study area favoured child labour, although the child-labour legislation is against using children as labourers. Various ethnicities 6 in the study area considered a person between the ages of 5 and 12 years to be a child, yet the Constitution of Uganda dictates the age of childhood to be below 18 years. This causes conflict in the definition of who a child is. Although parents were aware of the Universal Primary Education (UPE) regulation penalties for not taking children to school, they still permitted children to engage in child-labour activities, and little has been done by government to curb the culprits. Awareness of the Sub Counties and Credit Co-operatives (SACCOs) and their implementation has not helped to reduce poverty in the area studied, resulting in an increased school dropout rate among school-going-age children, as well as more child-labour activities. The major conclusion of the study was that little has been done to increase the awareness levels of the teachers, parents and their children about child labour and its legislation, their knowledge of and involvement in micro-finance institutions in the community, and the availability of vocational training institutions. Little has therefore been done to reduce child-labour activities, improve the economic status of the community, and improve their human resource skills. The major recommendations of the research to the study are that culture should not override the Constitution as far as the definition of age limit is concerned. The government should carry out stakeholder analyses, and implement a life-skills and sensitisation programme in order to improve child participation in the Universal Primary Education (UPE) programme. Government should ensure that the society is given information about basic accounting, project planning and management skills, in order to be effectively involved in the economic programmes of SACCOs.

More recently approaches have focused on the role that people perform in bringing about competitive advantage. Research indicates that engaged employees contribute vastly to the financial bottom-line of the organisation (see section 3.2.2). The research problem in this study was to identify strategies that organisations can use to engage their employees. To achieve this objective a theoretical employee engagement model was presented. The presentation of the theoretical model consisted of the following sub-processes: § Firstly, a literature survey was conducted to determine the underlying drivers/constructs of employee engagement. Abstract iii § The second comprised surveying the literature dealing specifically with approaches for measuring employee engagement. § Thirdly, the literature was surveyed to identify strategies and models used by organisations for engaging employees. The theoretical employee engagement model served as a basis for the compilation of the survey questionnaire that determined the extent to which human resource practitioners and line managers agree with the theoretical model developed in this study. The questionnaire was administered to a random sample of individuals employed in the automotive cluster in the Nelson Mandela Metropolitan Municipality and the Buffalo City Metropole. The empirical results obtained from the survey indicated that respondents strongly concurred with the theoretical employee engagement model presented in the study. These results were included in the theoretical model, which lead to the development of the integrated organisational employee engagement model. The model comprises of four interrelated categories that all contribute to enhancing employees’ engagement. These categories are organisational leadership, organisational culture, organisational strategies and the manager’s role. From the literature survey and the study it became clear that the role of the manager, had the most significant impact on employee engagement of all the categories. In addition, the integrated organisational employee engagement model can be used by organisations as an applied strategy for the measurement of employee engagement. The main findings from this research are that 60 per cent of organisations that participated in the empirical study had implemented strategies to engage their employees. However, the majority of organisations reporting not having an engagement strategy were among organisations that employed less than 700 employees (smaller organisations). The study also highlighted certain variables that required special attention, especially when implementing employee engagement within the South African context. South African companies as compared to their overseas counterparts, rated organisational engagement variables such as remuneration, benefits and gain sharing lower. A further variable that was identified by the study requiring special attention was, ‘senior management shows a sincere interest in employees’ well-being’. A final point emanating from the study is that the implementation of employee engagement, as a strategy to enhance organisational competitiveness, must be viewed as a continuous process. Organisations should prior, to the implementation of an employee engagement strategy, consider whether they are prepared to share engagement results, take corrective action commensurate with the results and deal with employee expectations that may be incurred. The strategies espoused by the integrated organisational employee engagement model developed in this study, can be used by organisations to increase organisational competitiveness by improving their employees’ level of engagement.

According to the findings in this study, in African tradition women like to enforce patriarchy upon younger women. Older women feel that they have the duty of passing on cultural practices from generation to generation. Everything from manner of dress, posture, appropriate seating positions, eating patterns, performance of household chores, sexual expression, and voice tone and infection, self-esteem and self-concept, flows from the gender one is assigned at birth. From birth then, women and men are set on different physically based psychological paths. Of all the obstacles that limit the advancement of women, those touching upon knowledge and values are the most difficult to remove. When a woman lacks the independent capacity to assert her own positive truths and values, she is unable to contribute her insights and experiences to the various fields of human knowledge. When denied opportunities for higher forms of self expression, women may out of frustration attack the modes of understanding upheld by men. In this study theories such as black criticism, psychoanalysis, feminism and African womanism are relevant for discussing the portrayal of women. The descriptive method of research has been applied. Both observation and participation have been used for exposing barriers that block the development of women. This study will enable literature students and researchers to view culture in a broader perspective. It will enable them to consider conventions which determine the way human experience is presented in literature. Chapter one provides literature students and the researchers with a broad overview about how to develop an introductory perspective. Chapter two aims at developing a theoretical framework which serves as the basis of this study. Chapter three examines the effect of gender inequality. It opens an area of extensive examination that differentiates sexual practice from the sexual roles assigned to women and men. Chapter four examines women who are submissive or radical in some cultural aspects. Chapter five discusses women’s self assertion. Chapter six concludes this study.
