- Description: During this study, an attempt was made to explore the opinions and attitudes of members of the South African Police Service (SAPS) towards issues of diversity before, immediately after and three months after participating in diversity training workshops presented by the SAPS Training Division during 1997 in Port Elizabeth. The aim of this thesis was not to assess the diversity training itself, but to discover if the training, as currently presented, in any way influenced the attitudes of participants. In order to meet this goal, literature and empirical studies were conducted. The literature study sets the theoretical foundation pertaining to the history of the SAPS and the attitudes and prejudices of and diversity among SAPS members. During the empirical research phase, a non-probability purposive sampling procedure was adopted. Four of a range of diversity workshops presented by the Training Division of the SAPS during 1997 were selected for the purpose of this study. An internal SAPS process was used to nominate members to attend the workshops. The researcher requested the participants in each of the four workshops to complete a self-administered questionnaire before as well as after the workshop concerned. Immediately after each of the four workshops, a short interview was held with each attendee. To explore the stability of any change evident from responses on the questionnaires completed after the workshops, the attendees were again requested to complete the same questionnaire three months later. To increase the validity of any conclusion that attitudinal change was related to the workshop, a control group was used. This study has revealed that a cross-spectrum of SAPS members of both sexes and diverse racial backgrounds believe that various forms of discrimination exist within the SAPS. It confirmed that the diversity training presented by the SAPS Training Division is a useful instrument to heighten members' awareness of the different norms and customs of other cultural/ethnic groups and of the necessity that the SAPS should be constituted of a cross-spectrum of racial groups reflecting the South African society. Finally, based on the research findings, recommendations were made involving management and its supportive services and diversity training.

- Description: There is little data regarding the prevalence of dementia in South Africa. Estimating such prevalence is problematic as the most commonly used cognitive screening tests are inappropriate for use in non-western populations. For this reason researchers have explored the use of informant questionnaires where relatives provide information on cognitive functioning over the last year. In the South African context Lenger, de Villiers & Louw (1996) conducted a dementia case-ascertainment study in a Xhosa-speaking community near Cape Town using a well-known informant questionnaire, the DECO, and concurrent clinical assessment. Reflecting on the discrepancies between DECO scores and clinical diagnosis, the researchers conducted interviews to explore beliefs and expectations regarding the elderly and cognitive decline. The aims of the current research were to gather comparative data from Bothasig, an English-speaking community, in order to explore areas of commonality and difference in perceptions and attributions regarding cognitive decline in the elderly. The study found that a significant percentage of informants from both Bothasig and Langa consider forgetfulness to be normal in old age. Different discourses around illness in the elderly were identified incorporating a range of medicalised and folk attributions. The discussion showed that informant perceptions within the Langa community may increase the likelihood that observational data provided in informant questionnaires may be confounded by cultural perceptions regarding the elderly. In addition, certain items on the DECO were found to be unsuitable for use across groups and modifications were proposed. The data was analyzed using both quantitative methods and phenomenological discourse analysis. The discussion concluded with metatheoretical reflections on the tension between etic and emic perspectives in cross-cultural research.

- Description: This research situates itself in the study of stigma in mental illness. In particular, the aim is to explore the potential that art making and exhibiting has in reducing stigma for those with a diagnosis of schizophrenia. The research explores one aspect (the exhibition stage) of an "art as therapy" project. The exhibiting of one’s artwork aims to counter limiting "patient" identities by allowing those labelled as psychiatric patients to extend their self-identity to an alternative identity of the "artist". However, this idea only stands strong if the artwork created is not discriminated against as "naïve art" and is accepted or at least considered for acceptance as legitimate nonprofessional artwork. This research explores the ways in which art created by inpatients with a diagnosis of schizophrenia is received by the general art-viewing public at the National Arts Festival in Grahamstown, South Africa. The study uses a discourse analytic framework to analyse the interviews of members of the public who attended the art exhibition of patient artwork. It will examine the ways in which the public construct the artworks and how they position the makers of this art across a continuum, from patient to artist. The results of this thesis have implications for rehabilitation practices for people with a diagnosis of schizophrenia particularly with regard to opportunities to "perform" alternative identities in public spaces.

Absent or missing fathers and male sexual difficulties : their manifestations in dream symbolism and interpretation

- Description: This thesis uses the individual case method in order to demonstrate the effects of paternal deprivation including variations in patterns of inadequate fathering on a male's psychological and sexual development. More specifically, it shows that father absence is a highly significant factor in the development of serious male psychological and social difficulties. Much of the material in the text concerns the impact of father absence on male 'sex-role' development. It is, as such further concerned with showing that paternal deprivation can lead to conflicts and rigidities in the individual's sex-role adjustment, which, in turn, are frequently related to deficits in emotional, cognitive and interpersonal functioning. Due consideration is given to such family characteristics as the mother's response to the father's absence which generally manifests as pathological and debilitating intrusiveness on the affected son's masculine identity. Other family features considered are the role of older male siblings and father substitutes e.g. stepfathers in the amelioration or exacerbation of these difficulties. An assumption is made that where such substitutes are competent father absence per se can have no more than a limited influence on the son's sex role development and vice versa. Some dreams from the case are presented for the purpose of viewing how the considered difficulties manifest in dream symbolism. Attention is paid also to the question as to whether such dreams can provide any useful clues toward an understanding of the nature, type and psychological situatedness of the affected people. Consequently, a discussion of a psychotherapy attempt is made to show that a competent understanding and interpretation of such symbolism will have an invaluable healing benefit on these difficulties.

This sample composed of black South African, IsiXhosa speakers with an educational level of Grade 11 and 12, derived through DET and former DET schooling. The gender demographics were as follows; females n = 21 and males n = 12. This sample was purposefully selected based on current cross-cultural research which suggests that individuals matching these above-mentioned demographics are significantly disadvantaged when compared to available neuropsychological norms. This is due to the fact that current norms have been created in contexts with socio-cultural influences; including culture, language and quantity and quality of education distinctly dissimilar to individuals like that composed in the sample. Hence the purpose of this study was fourfold namely; 1) Describe and consider socio-cultural factors and the influence on test performance 2) Provide descriptive and preliminary normative data on this neuropsychologically underrepresented population 3) Compare test performance between age and gender through stratification of the sample and finally to 4) Evaluate the current norms of the two WMS subtests and assess their validity for black South Africans with DET and former DET schooling with comparisons to the results found in the study. Information derived from the statistical analyses indicated that a higher performance in favour of the younger group over the older age range was consistently found for both WMS subtests. With regards to gender, some higher means were evident for the male population in the sample than was produced by the female group. Lastly, due to the fact that most scores derived from the sample were considerably lower when compared to the available norms, it is felt that socio-cultural factors prevalent to this population are a significant cause of lower test performance and thus warrant the development of appropriate normative indicators.

Comparisons of performance were carried out across a spectrum of neuropsychological tests for the three rugby groups (Total Rugby, Springbok Rugby, and Under 21 Rugby) versus the performance of the non-contact sport control group (Hockey Control), as well as comparisons of performance f9r the subgroups of Rugby Forwards versus Rugby Backs. Comparisons revealed a consistent pattern of poorer performance across all rugby groups relative to the performance of the controls on tests highly sensitive to the effects of diffuse brain damage. Within rugby group comparisons (Forwards versus Backs) showed significantly poorer performance for Total Rugby Forwards and Springbok Rugby Forwards relative to the performance of the respective Total Rugby Backs and Springbok Rugby Backs on sensitive, as well as on somewhat less sensitive, neuropsychological tests. The performance of Under 21 Rugby Forwards relative to Under 21 Rugby Backs demonstrated similar trends. Brain reserve capacity theory was used as a conceptual basis for discussing the implications of these findings.

- Description: Against the background of culture bound syndromes and the universalism/relativism debate, this research set out to investigate the diagnosis of brain fag syndrome by examining case studies in a South African context. Diagnostic criteria were identified by way of an analysis of the symptoms reported by a convenience sample of 20 black students experiencing problems related to study. In-depth case studies were conducted with three of the participants from the sample who most thoroughly met the criteria for brain fag syndrome. In a comparison between the brain fag diagnoses and the DSM-IV diagnoses, it was established that while the DSM-IV was not without merit in a cross cultural context, cultural differences were found which suggested the usefulness of the brain fag syndrome diagnosis. A speculative statement on the universalism/relativism debate was proposed based on the principle of complementarity derived from Quantum physics.

While no change was experienced within the sibling relationship, the siblings’ mental illness was experienced as affecting the sibling relationship due to the socioeconomic status of the participants and stigma. The findings support and expand on the growing knowledge of adult sibling relationships and mental illness.

- Description: South Africa is a very violent society, where violence is often used as a social resource to maintain control and establish authority. Global and local research suggests that there is a high prevalence of intimate partner violence (IPV), a facet of this violence, although little research has been conducted into the effects of IPV during pregnancy in the South African non-governmental organisation (NGO) context. NGOs globally and in South Africa have attempted to address IPV and IPV during pregnancy by providing services that aim to assist (largely female) clients emotionally and logistically. In light of this phenomenon, this qualitative study presents data generated through the use of a lightly-structured narrative interview schedule. The interviews were conducted over three sessions with eight counsellors, all based at two NGOs in South Africa and experienced in counselling women who have suffered IPV and IPV during pregnancy. This study used Taylor and Littleton’s (2006) narrative-discursive analytical lens, infused with theoretical insights from Foucault about power, discourse and narrative in order to identify the discursive resources that shape the narratives that the counsellors shared in the interviews and how these translate into subject positions and (gendered) power relations of the men and women about whom they speak. Six discursive resources emerged from the narratives, namely a discourse of ‘traditional “African” culture’, ‘patriarchal masculinity’, ‘nurturing femininity’, ‘female victimhood’, ‘female survivorhood’ and ‘human rights’. These informed the three main narratives that emerged: narratives about IPV in general, IPV during pregnancy, and the counsellors’ narratives about their intervention strategies. Within these narratives (and the micro-narratives which comprised them), men were largely positioned as subscribing to violent patriarchal behaviour whilst women were mostly positioned as nurturing and victims of this violence. In reflecting upon this analysis, this study also aimed to address the questions of what is achieved or gained by using these narratives and discursive resources, what the significance or consequences are of constructing and using these particular narratives and discourses and whether different narratives or discourses would have been possible. Recommendations for further research includes incorporating more sites as well as interviewing perpetrators and IPV survivors themselves, perhaps in their home language where relevant rather than English, to gain a broader and more faceted understanding of the dynamics surrounding IPV during pregnancy. A recommendation for practice in intervention against IPV during pregnancy is to introduce more holistic/systemic intervention strategies and working with communities to address this issue.

This case-study approach involved a mixed methods approach, consisting of pre-intervention and post-intervention assessments, interviews, and performance profiles. The tools used include Vealey’s (1986) Competitive Orientation Inventory (COI), Gill and Deeter’s (1988) Sport Orientation Questionnaire (SOQ), performance profiles, and semi-structured interviews. The goal-setting intervention lasted a total of five weeks. The results from the preintervention and post-intervention assessments were compared to determine whether any changes in motivation or self-efficacy occurred during the intervention. For the first participant, significant changes in both motivation and self-efficacy were present in addition to significant changes in competitive orientation. For the second participant, there were no significant changes in either motivation or self-efficacy. However, for the second participant there was also no change in competitive orientation. Findings suggest that the focus and competitive orientation of an athlete have a significant influence on the types of goals set, and consequently on positive changes in motivation, confidence, and self-efficacy. The influence of the competitive orientation in athletes also highlighted the importance of attaining a balance between performance and outcome goal-related activities. It was also discovered through qualitative assessment that this program is suitable for particular groups of athletes such as those who are injured, display an external locus of causality or are otherwise not performing at their prime.

In order to maintain a competitive advantage it is vital that organisations manage such change and are sensitive to their human resource. It is imperative for organisations to understand and explore the factors that impact on employee satisfaction and overall organisational performance. The hospitality industry is an industry that is notorious for low levels of pay and long working hours, and often dissatisfied employees. This research focused on a hotel, that is part of an international chain of hotels, which has recently undergone a rebranding process (a change from within). This research aimed to assess and explore factors that impact on levels of employee satisfaction and organisation/hotel performance, in other words it aimed to 'diagnose' the hotel's current status. The research was conducted in two phases. Phase one made use of a widely used measure of job satisfaction, the job descriptive index (JDI), that looked at five facets of job satisfaction namely: pay, opportunity for promotion, co-workers, supervision and the nature of work. Phase two further explored the results of the JDI (staff being very dissatisfied with pay and promotions opportunity) and further explored other areas of the organisation/hotel with the use of an organisational development model, Weisbord's Six-Box Model (1990). The 'boxes' included areas of purpose, structure, relationships, leadership, rewards and helpful mechanisms. These areas were explored with staff using focus groups. Heads of departments (management) and the deputy general manager of the hotel were interviewed using a semi-structured interview format, exploring similar issues to those researched with staff. Results indicated problems in the hotel with regard to purpose, a severe lack of communication and staff feeling they have little chance for promotion as well as pay structures being perceived as unfair. The overall leadership at the hotel was described as erratic, and relationships between management revealed high levels of mistrust. Due to limited research in the South African hospitality industry, much of the literature available is based on experiences in the United States of America or the United Kingdom. For this research, the researcher had few previous published findings and was unsure of the many issues that could possibly arise. However, the intervention was enjoyable and recommendations have been provided for the hotel to consider, so the hotel can go from "good to great".

“I won’t say I feel happy or sad”: experiences of siblings of young disabled people in disadvantaged socio-economic circumstances

However, in disadvantaged socioeconomic contexts, where disability has been found to be more prevalent, and where arguably, the functioning and quality of life of a disabled person is more likely to be affected by an impairment, very little is known about how siblings of young disabled people are affected. In response, this qualitative study explores the experiences of five, isiXhosa speaking adolescents, living in Joza Township, Grahamstown, who have a brother or sister with an intellectual, physical or developmental impairment. Semi-structured interviews were conducted and an interpretative phenomenological analytic approach was utilized to ascertain how the participants make sense of their worlds in relation to their sibling’s disability. Specifically, this research aimed at developing an understanding of how the participants experienced their family climate, self-concept, interpersonal relations and daily living in relation to their disabled sibling. The results of this study reveal a prevailing sense of incongruity experienced by the participants, although there are variances between their experiences. While family climate was largely experienced as warm, the participants were ambivalent about their relationship with their mothers who are experienced more as providers than nurturers. The participants described oscillating between feelings of protectiveness and alliance, and responsibility and sacrifice toward their sibling. A high incidence of incongruity pertaining to their sense of self was noted; this was described as impacting on their interpersonal relations where an underlying sense of negative public perception in relation to the disability is perceived. Although the participants expressed feeling supported within their homes, it was evident that they experienced little support from peers or the community at large. Four out of the five participants did not report experiencing a sense of deprivation, despite their socio-economic contexts and described a day-to-day existence that allows for their needs to be met. This included adequate time during their day to pursue personal interests as opposed to their time being spent taking care of their disabled sibling or assisting their parents, who may be overburdened due to the added care and responsibilities a disabled child might require. Furthermore, it is suggested that the incongruity experienced by the participants could be the result of various factors including age, gender, birth order and the nature of their sibling’s impairment. On the basis of the findings of this research, it can be concluded that the experiences of siblings of young, disabled people living in disadvantaged socioeconomic contexts cannot necessarily be described as being positive or negative, but are highly nuanced. In addition, the participants to some extent experience disability by association and are lacking in adequate support and opportunities to discuss their unique challenges. These insights serve to better inform disability studies in disadvantaged socio-economic circumstances. These findings are in accordance with earlier research.

On the basis of the findings of this research, it can be concluded that the experiences of siblings of young, disabled people living in disadvantaged socioeconomic contexts cannot necessarily be described as being positive or negative, but are highly nuanced. In addition, the participants to some extent experience disability by association and are lacking in adequate support and opportunities to discuss their unique challenges. These insights serve to better inform disability studies in disadvantaged socio-economic circumstances. These findings are in accordance with earlier research.

- Description: There is extensive evidence that the rehabilitation of individuals with coronary heart disease needs to include psychological components to complement the exercise and dietary recommendations that are normally provided. However, psychological aspects have not been integrated into medical care in South Africa to any significant degree. Psychological interventions overseas have included the modification of the Type A behaviour pattern, stress management, cognitive restructuring, relaxation techniques, improved communication skills, the identification and expression of emotions, and emotional support. The aim of the present study was to design a short-term group intervention which incorporated these aspects and which included an exploration of the mind-body experience post infarct. In addition, the intervention aimed to increase participants' awareness of the compensatory dynamics of the Type A behaviour pattern. The intervention was tailored to South African conditions and was evaluated by means of a multiple case study design. The intervention was delivered to a group of nine coronary heart disease patients which included six survivors of myocardial infarction, the remaining participants having undergone a by-pass operation. Data included weekly feedback sheets evaluating each session, repeated measures on the Profile of Mood States, the Jenkins Activity Survey, a Spouse Rating Scale and extensive qualitative data on each participant including tape recordings of each session and data collected from a series of interviews before, during and after the programme. The feedback sheets and recordings of the sessions were used as a basis for recommendations for revising the content and structure of the programme for future use. Case narratives were written for three of the participants and provided an in-depth look at how and why individual changes did or did not occur in response to the intervention. In addition, the case narratives revealed the role played by the compensatory dynamics of the Type A behaviour pattern in complicating rehabilitation for survivors of myocardial infarction. Two participants were offered a series of individual sessions at 18-month follow-up and the material from these sessions was also used to aid in the interpretation of the data. The content of the 18-month follow-up sessions provided evidence for the importance of conducting a developmental analysis of the origins of low self-esteem and insecurity that maintain and drive the Type A behaviour pattern. In these sessions, this analysis provided the basis for a brief focused psychodynamic psychotherapy that facilitated marked changes that had not been achieved in the 12-week structured group intervention. It is recommended that future research investigate the use of brief psychodynamic psychotherapy on an individual basis as a complement to a group intervention focusing on psycho-education, building social support and management of problematic emotions in everyday situations.

The data for the study was obtained by means of three dialogical interviews which were tape recorded, transcribed and analyzed with~n a cyclical framework consisting of three interdependent levels. The interpretive procedure is modeled upon the 'reading guide' developed by Brown, Tappan, Gilligan, Miller and Argyris (1989). The analysis follows a course from the individual psychological descriptions of the experience to the generation of a general, nomothetic narrative account of the acute phase. The findings are then discussed in relation to the existing literature and evaluated on the basis of the goals of the study. The study highlights the value of some of the 'stage' ways of thinking about SCI adjustment, while simultaneously stressing the need for placing adjustment within a more personalized, and individually meaningful context. Significant differences between the psycho-physical experiences of patients in the categories of complete and -incomplete SCI were found, which suggests that a sharper distinction be made in the literature between these two groups, in order to account for the marked variations in their experiences.-- The study also contains a number of shortcomings, such as a lack of understanding about certain historical and contextual factors which may have mediated the experiences of the trauma for the individuals concerned. These shortcomings and some suggestions fro their resolution are then discussed. The study concludes with an evaluation of the research strategy and methodology and also offers some suggestions for future research.

IQ scores for black African language and white English Southern Africans were comparable with the United States of America (USA) standardization when level and quality of education were equitable. (‘White English’ is the term used to denote those of European descent whose first language is English). A limitation of the research was the lack of control for language for most of the black groups and particularly in the Private/Model C Graduate group, where sixty percent of the participants originated from Zimbabwe. These represented a particularly elite group whose education was equitable to that of the white participants throughout their education (i.e. at primary, secondary and tertiary level). In order to rectify the lack of homogeneity of language, all non- Xhosa first language participants were excluded from the black sample and sixteen additional Xhosa first language participants were tested on the WAIS-III. Data analyses found no significant differences between the original and new groups, except in the comparison between Mixed African language Private/Model C Graduates and the Xhosa first language Private/Model C Graduate/15+ years of education, where there was a lowering of WAIS-III subtest, index and IQ scores in the newly constituted group. This lowering in test performance is explained in that the new Xhosa first language 15+ years of education group was a less advantaged group than the original Mixed African Language Private/Model C Graduate group, as the new group generally had less advantaged primary school education and had generally studied less at a tertiary level. Overall, these results demonstrate an incremental increase in WAIS-III test performance for sample groups on a continuum of quality of education from least to most advantaged education. This was true for both verbal and non-verbal subtests.

In order to rectify the lack of homogeneity of language, all non- Xhosa first language participants were excluded from the black sample and sixteen additional Xhosa first language participants were tested on the WAIS-III. Data analyses found no significant differences between the original and new groups, except in the comparison between Mixed African language Private/Model C Graduates and the Xhosa first language Private/Model C Graduate/15+ years of education, where there was a lowering of WAIS-III subtest, index and IQ scores in the newly constituted group. This lowering in test performance is explained in that the new Xhosa first language 15+ years of education group was a less advantaged group than the original Mixed African Language Private/Model C Graduate group, as the new group generally had less advantaged primary school education and had generally studied less at a tertiary level. Overall, these results demonstrate an incremental increase in WAIS-III test performance for sample groups on a continuum of quality of education from least to most advantaged education. This was true for both verbal and non-verbal subtests.
