Peer group supervision as an adjunct to individual supervision: an investigation of models of learning

- Description: Supervision of practice makes an important contribution to the development of psychotherapeutic skills in the training of psychologists (Bernard and Goodyear, 1998). Much research has, until recently, focussed on dyadic, hierarchical models of supervision, even though other forms of supervision have been developed. Peer group supervision has had little attention in the literature, although it is a common form of supervision utilised by psychologists in practice (Lewis, Greenburg and Hatch, 1988). A review of the literature considers the purposes of supervision; elements of dyadic supervision; various forms of group, peer and peer group supervision; and the leaming process in supervision. The development and implementation of a peer supervision group (pSG) of intern psychologists within the training setting of a University is described in this study. The PSG model was developed from the model proposed by Wilbur, Roberts-Wilbur, Morris, Betz and Hart (1991). Transcripts from nine audio-taped PSG sessions were analysed, and a comparison with four audio-taped dyadic supervision sessions was then undertaken. Grounded Theory methodology was employed in the design of the study and analysis of the data. The form and content of the two models of supervision were examined, with particular attention to the perspective of the trainees' learning experiences. The relative merits of both forms of supervision were assessed, and this analysis clearly demonstrates that peer group supervision has the potential to complement dyadic supervision by contributing differing learning experiences. A model of key influences upon, and effects of, participation in the two forms of supervision has been developed. Suggestions are made of ways in which dyadic supervision may be optimised, and recommendations for further development of the PSG emerge. The results were then considered from a neo-Vygotskian perspective. This enabled the findings to be linked to a comprehensive theory of learning, pointing to the key role of speech in thinking, and the contributions of the various forms of dialogue to deepened understandings. The discussion includes: consideration of techniques which enable trainees to obtain assistance from both more experienced practitioners as well as from their peers; an exploration of aspects of subjectivity and intersubjectivity; and contextual influences which have bearing on the study. This study identifies the need for further consideration of the supervision process in South Africa, and makes recommendations for the training of supervisors. The neo-Vygotskian model offers great promise both as a framework for understanding the leaming process in. supervision, and for developing guidelines for enhancing supervisory practice.

The general approach involved four major phases: (1) the clustering of various variables and identification of underlying key factors purported to be related to psychological health based on the writer's clinical experience and review of the mental health literature, (2) the formulation of an ~ priori concept of psychological well-being based on the operational definition of those factors, (3) the construction of an inventory designed to examine the proposed concept, and (4) the interpretation of the results and their implications for the development of the overall concept based on the examination of the factorial structure, validity and reliability of the inventory. The"a posteriori concept" evolved out of this process. The results of the present study indicate that the most valid and reliable factorial components of psychological well-being are self regard,interpersonal relationship, independence, problem-solving, assertiveness,reality testing, stress tolerance, self-actualization and happiness; social responsibility and flexibility emerged as questionable components of the a posteriori concept. The inventory which was designed to study the concept was successful in significantly differentiating various levels of psychological health. This way of conceptualizing and assessing psychological well-being has potential applicability for mental health practitioners and researchers.

Five students each gave an account of an occasion when they procrastinated. The resultant protocols were analysed and the Situated Structure of each individual’s experience was reported. From these, the General Structure of procrastinating behaviour was determined. A further, novel step was added to the standard methodology, whereby ‘themes’ were extracted from participant protocols and a ‘Composite Reality’ of everyday-life procrastination was rendered. Participants’ accounts suggest they are concerned the results of intellectual tasks they undertake will be seen as equivalent to their quality of being-as-an-individual: poor work results will be interpreted by important-others as evidence of participants’ poor quality of self – which is to be avoided. This study suggests that procrastination is a ploy used by individuals to avoid criticism, by deflecting assessment of their capacity to complete a task well, to instead, what they are capable of when only a limited time is available. Conclusions drawn by the important-others of participants’ true ability are thereby confounded. The results achieved in the phenomenological study were compared with others originating from various quantitative studies, and considerable overlap was found. The experiential richness of the phenomenological results point to a worthwhile methodological strategy for future procrastination research.

"I felt that I deserved it" : an Investigation into HIV-related PTSD, traumatic life events, and the personal experiences of living with HIV : a mixed-method study

- Description: There appears to be a growing body of literature focusing on PTSD and HIV-related PTSD (the diagnosis of HIV being the significant traumatic event) amongst HIV-positive samples, but only a few African studies that attempt to estimate the prevalence of PTSD amongst HIV-positive people, and even fewer that attempt to estimate the prevalence of HIV-related PTSD. The systemic review presented in this study is currently fully inclusive and is the most up-to-date available. Estimates of the prevalence of PTSD and HIV-related PTSD in South Africa range from 0.7 to 54.1% and, 4.2 to 40% respectively. The current cross-sectional study made use of a mixed-method approach to investigate traumatic life events, PTSD and HIV-related PTSD within a primary health-care centre in KwaZulu-Natal. The quantitative sample consisted of 159 adults (18-50 years) who were compliant on ARV medication. Using the CIDI-PTSD module, the adapted CIDI-PTSD module for HIV, and IES-R, findings indicated that 62% had reported some kind of traumatic event in their lifetime, with 29.6% of participants meeting the criteria for lifetime PTSD, and 40.9% meeting the criteria for lifetime HIV-related PTSD. Altogether, 57.9% of individuals met the criteria for some form of PTSD (either regular PTSD or HIV-related PTSD), and 12.6% met the criteria for both PTSD and HIV-related PTSD. Of the different categories of traumatic events, interpersonal violence has the highest rate of PTSD, followed by a diagnosis of and living with HIV, and then disaster. Furthermore, the IES-R was compared for its usefulness as a screening measure for PTSD against both the CIDI, but results suggest that it is an inferior screening measure to the PDS. The qualitative study consisted of six participants who were examined using IPA methodology informed by the Ehlers and Clark (2000) Model of trauma. Their experiences revealed experiences of stigma, a number of negative appraisals, negative emotions and coping behaviours. Some of the latter might serve as compensatory mechanisms to avoid negative judgements. Hypervigilance seems to be a feature of ARV-compliance that might confer added vulnerability to PTSD and other anxiety disorders.

Jung in the light of existential phenomenology, thereby to lay the foundations for an integrated phenomenological analytical psychology. It was recognised that although Jung introduced a poetic understanding of psychological life he tended to adhere theoretically to a Cartesian and natural scientific epistemology and ontology, in which the knower is separated from the known, and psychological life is encapsulated inside the human subject. Thus the main task, which defined generally the study's scope and limitations, was to undercut the lingering Cartesianism in Jung's thought, thereby to recover the world in which one lives as intrinsically and authentically psychological, and one's psychological life as irreducibly world-related. The ontological guidelines for this endeavour were taken primarily from Heidegger and Merleau-Ponty, but it was consistently argued that this hermeneutic movement towards an existential understanding is given within Jung's work itself. Thus: Jung's method is primarily hermeneutic-phenomenological; the psyche is not "mind" or an inner realm more or less linked to the body, but is the embodied life-world, and Jung's descriptions of it - of its autonomy, spatiality and bodiliness, for instance - achieve ontological clarity when it is articulated as Dasein; the self as the totality of the psyche is interpretated in terms of Dasein, and individuation involves differentiation, personalisation and appropriation within existence itself; the complexes, archetypally grounded, are the vital densities of incarnate life, ambiguously conscious and unconscious, known and lived; the archetypes are the fundamental necessities of psychological life, autonomous imaginal structures within which both body and world are founded. Imaginal autonomy is revealed ontically as the metaphorical reality of things, but since imaginal autonomy has no ground thought about psychological life is ultimately poetic. Where relevant, recent theoretical developments in analytical psychology were discussed, particularly the Developmental and Archetypal movements. A clinical study was presented to illustrate some of the main themes of the thesis. In conclusion, the main themes of an integrated phenomenological analytical psychology were outlined, and the central contributions of analytical psychology and existential phenomenology were highlighted.

- Description: In South Africa access to mental healthcare resources is restricted for a number of reasons including language barriers that prevent suitable communication between mental healthcare professionals and African language speaking South Africans. The translation of psychometric tools into African languages has been identified as one method in improving access to psychological services for African language speakers. The Clinical Outcomes in Routine Evaluation - Outcome Measure (CORE-OM) has demonstrated its clinical utility within the United Kingdom (UK) National Healthcare Service (NHS) as a standardised psychotherapy outcome measure that evaluates the degree of psychological distress individuals present with at the start of psychotherapy treatment, and the degree of change that has been effected at the termination of therapy. A measure like the CORE-OM holds valuable clinical utility for the South African context. This thesis argues that the availability of a valid Xhosa version of the CORE-OM would allow for improved access to psychotherapy resources for Xhosa speaking individuals, and allow for the evaluation of the effectiveness of psychotherapy interventions conducted in Xhosa. The CORE-OM developers have provided a translation design and set of guidelines to standardise the translation of the CORE-OM into different languages. However this thesis argues that these guidelines are incomplete. Instead International Test Commission (ITC) guidelines are recommended as a culturally sensitive method to supplement current CORE-OM translation guidelines, in order to generate a valid Xhosa measure of distress. A mixed methods approach is applied which first investigates the construct equivalence and bias of the CORE-OM English version within a South African student population sample, both qualitatively and quantitatively, in order to establish the degree of adaptation required to generate a valid Xhosa version of distress. Next the CORE-OM English version is translated into Xhosa using the five-stage translation design prescribed by the CORE System Trust, supplemented by ITC guidelines. All changes made to the CORE-OM during translation into Xhosa are documented. The CORE-OM Xhosa version is then investigated for reliability and validity. This investigation reveals low internal reliability within the subjective wellbeing domain indicating that these items are less meaningful as depictions of distress within the Xhosa language. A reduced version of the CORE-OM demonstrates strong psychometric properties as a valid Xhosa measure of distress.

All changes made to the CORE-OM during translation into Xhosa are documented. The CORE-OM Xhosa version is then investigated for reliability and validity. This investigation reveals low internal reliability within the subjective wellbeing domain indicating that these items are less meaningful as depictions of distress within the Xhosa language. A reduced version of the CORE-OM demonstrates strong psychometric properties as a valid Xhosa measure of distress.

I then consider the role of personality characteristics and psychopathology in individuals who have committed such offences. With the basic features of the offender and act itself outlined, the following section reviews key areas of debate regarding the psychodynamics of violence and the intrapsychic make-up of the rage-type murderer. I first explore the nature of aggression as debated in psychoanalysis and conclude that the views expressed are often unn,ecessarily polarized regarding the origins of aggression and suggest that the specifics of particular types of aggression require consideration in order to assess their intrapsychic nature. The specifics of rage and violence are discussed with this in mind. In the second chapter of this section I develop a number of intrapsychic dimensions to be used in understanding how different types of violence are constituted. Psychodynamic contributions towards understanding rage-type murder, as a specific form of violence, are then discussed. Following this review, a number of directive ~uestions are formulated regarding (1) the intrapsychic dimension of rage-type murder; (2) the pJ;esence of the borderline personality in such offender~ and its intrapsychic nature; and (3) the_ specific psychodynamics that lie behind what is argued to be a defensive act of murder. A multiple case study approach, using nine imprisoned rage-type offenders, is used to further explore the above issues. Court summary reports, the Thematic Apperception Test and the Psychoanalytic Research Interview comprised the research material, with particular emphasis placed on the interview material. The interview is approached from a psychoanalytic perspective and I develop some theoretical, technical and analytical guidelines to try to broaden Jhe use of psychoanalysis in the research domain. Findings of the research reveal a specific kind of defensive organization that is characterized by a constellation of object relations that I term the 'narcissistic exoskeleton'. I suggest that these findings best fit the description of a particular kind of borderline personality organization typified by apparent 'normality'. Other prominent aspects of the dimensions of violence observed in these cases include: (1) a poor representational capacity; (2) an interactional style characterized by uncontainable projective exchanges between victim and offender; (3) a collusive primary object relationship combined with the absence of an internalized 'third object'; (4) a 'two-faced' superego structure; (5) the internalization of traumatic experience that has become associated with a bad object system; (6) phantasies of restoring ideal good in external objects alongside conscious fantasies of annihilation. Within the context of these factors the intrapsychic events that lead to the act itself are discussed. It is found that a collapse of the 'narcissistic exoskeleton', the intrusion of the bad object system and the unbearable shame that this evokes in the offender are prominent features of what culminates in an act of explosive rage and projective identification. Some of the implications of my research are briefly discussed in the concluding chapter.

A narrative-discursive analysis of abortion decision-making in Zimbabwe

Research on abortion decision-making has rarely examined how women who undergo a termination of pregnancy (TOP) construct micro-narratives of the decision to terminate the pregnancy and also how these women are positioned by the service providers who interact with them. Using a Foucauldian postcolonial feminist approach and narrative-discursive analysis, this study explores abortion decision-making narratives in a Zimbabwean context where abortion laws are restrictive. In this study I elicited the narratives of women who had undergone an abortion about how they came to make the decision and proceeded to terminate the pregnancy. I highlight the discourses employed in constructing these narratives and how women position themselves in these narratives and discourses. These are then compared to the subject positions enabled in health service providers’ narratives on the same topic. These narratives are then linked to the social discourses and power relations that work to enable or constrain reproductive justice. The data were collected from three sites in Harare, Zimbabwe. The three sites were Harare Hospital, Epworth and Mufakose. An adapted version of Wengraf’s (2001) narrative interview was used to elicit narratives from 18 women who had terminated pregnancies (six at each site). Semi-structured interviews were conducted with six service providers (two nurses at Harare Hospital, two village health workers in Epworth and two nurses in Mufakose). All the service providers interviewed have experience working with women who have terminated pregnancies. In narrating their stories about their abortions, the women employed discursive resources around shame, stigma, religion, health and culture. These discursive resources were drawn upon in the construction of the women’s micro-narratives. The women spoke in a socially sanctioned manner where stories were enabled and constrained by particular religious, cultural and gendered discursive resources. In these stories, cultural constructions, gendered understandings of motherhood and femininity constrained reproductive justice for women who have terminated pregnancies. Comparisons of the way women positioned themselves and how they were positioned by health service providers point to the existence of social discourses and power relations that work to constrain reproductive justice. While the women saw themselves as having ‘unsupportable pregnancies’, the service providers positioned them as being evil, selfish and irresponsible. The negative positions deployed by the service providers point to the vilification and blaming of women who have undergone a termination of pregnancy. In these positions, the woman is at fault and there is silence on the role of men in abortion decision-making. In the women’s narratives and the health service providers positioning of the women a ‘reproductive rights’ discourse was absent. This was significant as much of the activism around abortion has centred on the woman’s rights to her body. Where rights were mentioned, it was in reference to foetal rights (using cultural, moralistic religious understandings of abortion as killing). The missing ‘reproductive rights’ discourse points to a need to move from a reproductive rights framework to a reproductive justice framework that can be applied through local understandings of hunhu/ubuntu. By doing this, abortion is not seen as a ‘choice’ that a woman makes but rather as involving broader social and environmental circumstances that make a pregnancy ‘unsupportable’.

The independent and dependent comparative analyses in respect of both nonconcussed and concussed groups, provided cross-validation of poorer acute and/or chronic neuropsychological outcomes for the rugby groups on the ImPACT Reaction Time, Visual Motor Speed, Impulse Control and Symptom composites, and the WMS-III Verbal Paired Associates. The finding of significantly poorer scores on Verbal Paired Associates up to 24 days post concussion for the rugby players versus controls, was longer than the 7 – 10 day recovery period frequently cited in the literature. The overall implication of the study is that even in a group with high cognitive reserve such as these provincial level athletes, there may be prolonged acute recovery, as well as permanent deleterious neuropsychological consequences of cumulative concussive injury in association with a sport such as rugby. Accordingly, the move towards careful individualised postconcussion monitoring of neurocognitive functioning is endorsed, including early identification of any significant permanent reductions in cognitive reserve. Sensitivity of the ImPACT test might be enhanced via inclusion of a verbal associate learning task.

- Description: Clinical psychology’s relevance and future viability depend on its ability to render services that are relevant and sensitive to multicultural and minority issues. Lesbian, gay, bisexual and transgendered people are one such group that professional psychology – both in South Africa and abroad - has identified as having unique treatment needs for which psychologists require specialised knowledge and skills in order to render appropriate treatment. Competence to treat non-heterosexual patients has been framed in terms of a gay affirmative paradigm which has as its basic tenet the recognition that same-gender orientation is not pathological but rather a healthy alternative to heterosexuality. From this perspective being “gay friendly” or “gay accepting” is not enough. To implement a gay affirmative approach in practice, practitioners must have resolved their possible prejudice and heterosexist bias and have the requisite knowledge of concerns unique to lesbian, gay, and bisexual (LGB) individuals to be able to apply their skills in a culturally sensitive manner. Although more American post-graduate psychology programmes are addressing sexual diversity, their failure to produce psychologists who feel competent to treat lesbian/gay or bisexual individuals has highlighted the need to develop effective training strategies based on empirical nvestigation. The dearth of comparable data about local South African psychology training prompted this inquiry which had four broad aims namely, (i) to establish to what extent trainees’ prior training had equipped them with the knowledge, awareness, and skills to approach their work with non-heterosexual patients in a gay affirmative manner, and (ii) to implement and (iii) to evaluate to what extent a brief structured training programme is effective in engaging the trainees; in increasing knowledge, in raising awareness, and in changing specific attitudes and imparting specific skills required for treating lesbian and gay patients.; and (iv) what, if any, recommendations should be made for the future with respect to training of sychologists in this area? The field of sexual orientation research has been expanded to include issues pertaining to bisexual, transgendered and intersexed people, but serious time constraints meant that issues pertaining to these groups could not be addressed in depth. Although the exclusion of these groups is problematic and may be seen as reinforcing their invisibility, it was decided to focus primarily on gay and lesbian issues s an introduction to same-sex orientation. It is envisaged that bisexual and transgender issues would be dealt with in depth in more advanced training. Nine trainee psychologists employed at hospitals in the greater Cape Town area volunteered to participate in the programme which comprised a series of two-hour experiential workshops offered once a week over six weeks. The study employed both quantitative and qualitative data analysis methods. The first stage entailed gathering information to better understand trainees’ existing level of competence. Individual interviews were conducted prior to the course to obtain data about their attitudes and perceptions regarding the need for such specialised training, and how qualified they considered themselves to be to treat LGB patients, and their experience in this regard. For the purpose of the over-all analysis information was also gathered about pertinent personal and social characteristics of the trainees, as well as their contact with lesbian/gay persons. In addition, an attitude survey and the Lesbian, Gay, and Bisexual Counselling Self-efficacy Scale (LGB-CSI) were administered to obtain benchmarks against which change could be measured. The second stage involved the implementation of the educational programme and gathering information about trainees’ responses to its various components. This stage concentrated on discovering how individual trainees reacted to material on lesbian, gay, and bisexual issues and how they used the programme to improve their self-awareness and skills. The results indicate that local psychology training might not address same-gender orientation adequately, thus reinforcing trainees’ belief that sexual orientation is irrelevant, and that their generalist training equips them to work with gay/lesbian/bisexual patients. While the training strengthened existing positive attitudes, it was less effective in changing blatant antigay prejudice. However, both quantitative and qualitative data suggest that the programme increased individuals’ awareness and insight into their previously unrecognised heterosexist biases and created greater understanding of the effects of stigmatisation on sexual minority individuals. In addition, the training increased trainee’s sense of competence to provide affirmative treatment as evidenced by the significant differences between the pre- and post-training mean scores on the Relationship, Knowledge, and Advocacy Scales and between the mid- and post-training means scores on the Assessment and Awareness Scales of the LGB-CSI. Despite the limited generalisability of these findings on account of possible sampling bias, the need and value of such training was confirmed by trainees’ recommendation that this programme should be a mandatory offering in the first year of clinical psychology training.

In addition, the training increased trainee’s sense of competence to provide affirmative treatment as evidenced by the significant differences between the pre- and post-training mean scores on the Relationship, Knowledge, and Advocacy Scales and between the mid- and post-training means scores on the Assessment and Awareness Scales of the LGB-CSI. Despite the limited generalisability of these findings on account of possible sampling bias, the need and value of such training was confirmed by trainees’ recommendation that this programme should be a mandatory offering in the first year of clinical psychology training.

The explication also revealed that participants identified with archetypal aspects of the father that their fathers' did not express, and desired archetypal aspects of the mother that their mother's did not express. Finally, in so much as the method distinguished ~ sex, sexual identity and sexual orientation from gender, gender identity, gender identification and gender orientation, it may prove useful for exploring gender in heterosexual relating.

The traditional healing system, and biomedicine, were compared for effectiveness, through the course of illness events. While biomedicine was more effective in containing acute psychotic episodes than treatment by the traditional healer, lack of appropriate resources within the biomedical setting had disastrous results for patient compliance and long-term management of the illness, particularly in people with obvious symptoms of bipolar disorder. The mental hospital emerged as an agent of control. While Xhosa culture provided a more tolerant setting for people with a mental illness, the course of severe mental illness was by no means benign, despite research suggesting a more positive outcome for such conditions in the developing world.

The appropriate central research question, formulated to elicit the experience of this phenomenon, emerged through the process of enquiry during the pilot study. Thirteen experienced, psychoanalytically-oriented psychotherapists were interviewed and the five protocols considered most revelatory of the phenomenon under investigation were analyzed in detail. The remaining eight protocols were used to illuminate central themes and to clarify areas of uncertainty during the phase of formal explication. The central findings revealed that the oscillating process of the therapist as he shifts from being immersed in the world of his patient to being in a position of observation and self reflection is the fulcrum around which he evaluates the nature of his patient's communications. During this ongoing process of discrimination, living in duality, the therapist comes to experience himself as a patient scrutinized by his own and his patient's confrontations. His journey of disentanglement, the endeavour to differentiate his responses from his patient's actions, is dependent on his ability to engage in honest selfreflection and to access his pre-theoretical and articulated cognitions of his patient. This allows him to acknowledge his own role in what has unfolded interpersonally and to appropriate his previously denied feelings for and attitudes towards his patient, a prerequisite for the accurate and full appraisal of the nature of his patient's communications. Forsaking fixed judgements, the therapist becomes open to the confluence between the reality-oriented responses and transference-based reactions of his patient. This salient discovery, when dialogued with the literature, reinforced the theories of Greenson and Langs that not all the interactions between the patient and the analyst/therapist are transference-based and that it is therefore imperative that the analyst/therapist reflect on his participation in the analytic situation.

Four research questions were formulated to interrogate the individual and collective experiences of three male patients whose internal worlds seem to be governed by rigid intrapychic structures organized around the dictatorship of a constellation of seemingly impenetrable defensive strategies. These questions were as follows: 1. What emotional states, actions and experiences of self and others characterize the clinical phenomenon designed as destructive narcissism and distinguish it from other forms of character pathology? 2. What early developmental experiences and relationships may have pre-disposed individuals to the development of this type of character pathology? 3. How are the psychodynamic processes of destructive narcissism structured and configured in the psychotherapeutic process and progress? 4. What are the transference/countertransference psychotherapeutic manifestations of the psychodynamics of destructive narcissism? The illustrative-didactic case study method was utilized to discuss pertinent aspects of each patient. This included their early developmental histories, inter- and intrapersonal relationships, their current mental state, defensive strategies and their stated reasons for commencing psychotherapy. In addition, the structure of the psychotherapeutic process with these patients was reviewed in depth. Various psychic and personality features, as unveiled through this process were discussed, as well as the implications of these for the therapeutic endeavor. The features chosen for discussion were: Firstly, the constellation of the internal object world, the capacity for symbolic thought and defensive organizations. Secondly, therapeutic ambivalence, which made psychotherapy untenable, was explored in conjunction with transference/countertransference issues. Thirdly, the shadow sides of psychotherapeutic change with these patients were considered and the issues of therapeutic failure and other treatment possibilities were examined. It was concluded that there need be an important shift with regard to the psychotherapeutic goals for those patients whose condition may be chronic, and for whom it appears that psychotherapy is of little benefit. In essence, the intent of psychotherapy with these patients is to reach the healthy sane patient of the patient within the pathological organization. Attempts to unravel the perverse gratification and protection derived from the domination of the narcissistic structure may not be enough, and the patient's collusion with the internal destructive gang should also be exposed. If this can be achieved, the patient may come to accept the existence of a part of himself as truly destructive. This, in turn, cannot be disowned, therefore the patient has to live with it. It did this utilizing a qualitative method, by exposing four respondents (patients), and two therapists to an art therapy situation in which images were created out of clay. A cross-cultural study of interpersonal distance and orientation schemata

- Description: Expectations about interpersonal distance during social encounters (distance schemata) and body orientation (orientation schemata) were investigated among White English-speakers and Xhosa groups which included illiterate traditionalists (Reds), poorly educated urban dwellers, and highly literate students and nurses. In a series of six experiments a doll placement task was used in which subjects represented dyadic encounters by placing pairs of standing dolls. For each situation represented the responses of each group of subjects were summarised in the form of a profile which showed the mean of the distance and three angle measures (IPOS profile). Independent variables included culture of subject, type of situation represented (friendly encounter, quarrel, accusation and denial, request) and the sex, age or relationship of the persons represented. In the culminating experiment (Experiment 6), females from three Xhosa groups (Reds or XR, poorly educated urban or XU, and urban nurses or XN) made twenty-three placements. In some respects the schemata of the four groups were very similar, while in others both distance and orientation schemata were a function of cultural group. The experiments allowed an assessment of the validity of the doll placement method to be made, and results were discussed in terms of the effects on interpersonal distance and body orientation of cultural norms concerning the showing of respect and the nature and strength of the emotions present in the various types of situation. It was concluded that with cultural movement away from the traditionalist pattern the schemata of the urban Xhosa showed a transition towards those found among the Whites in some respects. However, while the schemata of the XN group showed features of both those of the XR and White groups, those of the XU group showed features found in neither which seemed to reflect the insecurity of the cultural milieu of the urban poor.

The design and evaluation of a short-term group psychotherapy model for survivors of a first myocardial infarction

- Description: There is extensive evidence that the rehabilitation of individuals with coronary heart disease needs to include psychological components to complement the exercise and dietary recommendations that are normally provided. However, psychological aspects have not been integrated into medical care in South Africa to any significant degree. Psychological interventions overseas have included the modification of the Type A behaviour pattern, stress management, cognitive restructuring, relaxation techniques, improved communication skills, the identification and expression of emotions, and emotional support. The aim of the present study was to design a short-term group intervention which incorporated these aspects and which included an exploration of the mind-body experience post infarct. In addition, the intervention aimed to increase participants' awareness of the compensatory dynamics of the Type A behaviour pattern. The intervention was tailored to South African conditions and was evaluated by means of a multiple case study design. The intervention was delivered to a group of nine coronary heart disease patients which included six survivors of myocardial infarction, the remaining participants having undergone a by-pass operation. Data included weekly feedback sheets evaluating each session, repeated measures on the Profile of Mood States, the Jenkins Activity Survey, a Spouse Rating Scale and extensive qualitative data on each participant including tape recordings of each session and data collected from a series of interviews before, during and after the programme. The feedback sheets and recordings of the sessions were used as a basis for recommendations for revising the content and structure of the programme for future use. Case narratives were written for three of the participants and provided an in-depth look at how and why individual changes did or did not occur in response to the intervention. In addition, the case narratives revealed the role played by the compensatory dynamics of the Type A behaviour pattern in complicating rehabilitation for survivors of myocardial infarction. Two participants were offered a series of individual sessions at 18-month follow-up and the material from these sessions was also used to aid in the interpretation of the data. The content of the 18-month follow-up sessions provided evidence for the importance of conducting a developmental analysis of the origins of low self-esteem and insecurity that maintain and drive the Type A behaviour pattern. In these sessions, this analysis provided the basis for a brief focused psychodynamic psychotherapy that facilitated marked changes that had not been achieved in the 12-week structured group intervention. It is recommended that future research investigate the use of brief psychodynamic psychotherapy on an individual basis as a complement to a group intervention focusing on psycho-education, building social support and management of problematic emotions in everyday situations.

5-hydroxytryptamine and sexual behaviour in rhesus monkeys

- Description: Selective inhibition of 5-hydroxytryptamine by parachlorophenylalanine (PCPA) is able to restore sexual receptivity in female rhesus monkeys made unreceptive by bilateral adrenalectomy. PCPA in the doses used reduces the levels of 5-hydroxyindoleacetic acid (5HIAA) in the cerebrospinal fluid to 40 per cent of the normal oestradioltreated condition. Both the increased sexual receptivity and the lowered 5HIAA levels "in the CSF are in turn reversed by 5-hydroxytryptophal (5HTP), the irrmediate precursor of 5HT and the substance whose synthesis is inhibited by PCPA. 5HTP on its own reduces sexual receptivity and increases 5HIAA levels in the CSF of ovariectomised, oestradiol-treated (but otherwise intact) female rhesus monkeys. A substance other than an adrenal androgen has therefore been shown to restore sexual receptivity in adrenalectomised female monkeys . Testosterone propionate and oestradiol benzoate both lower the turnover rates of 5HT in the brains of ovariectomised female monkeys, as measured by the 2 hour probenecid test. Taken together, these findings suggest that adrenal androgens could act on specific sites in the female monkey brain via 5HT-containing neural systems, to control (or at least influence) sexual receptivity. All the results of administering oestradiol to ovariectomised monkeys in these experiments are consistent with the established roles of this hormone in female sexual attractiveness and in the gonadotrophin- controlling systems of the hypothalamo-hypophyseal axis. In contrast to these findings on 5HT and sexual receptivity in female monkeys, no clear role for 5HT- containing neural systems could be demonstrated in the grooming, aggressive or social behaviours of female monkeys. No clear role for 5HT could be demonstrated in the refractory period following ejaculation in male monkeys , or when testosterone replacement is given to castrated male monkeys.
