Management of medical records for healthcare service delivery at the Victoria Public Hospital in the Eastern Cape Province :South Africa

- Description: The study sought to investigate the management of medical records for healthcare service at the Victoria Public Hospital in the Eastern Cape Province. The objectives of the study were to describe the present records management practices in Victoria Hospital; find out the existing infrastructure for the management of patient medical records at the Victoria Hospital; determine the compliance of patient medical records management in Victoria Hospital with relevant national legislative and regulatory framework; find out the security of patient medical records at the Victoria Hospital. Quantitative and qualitative approaches were employed. The sample was drawn from the service providers and from the healthcare service users. Questionnaires, interviews and observation were used to collect data. The findings showed that Victoria Hospital uses manual records management system in the creation, maintenance and usage of records. In the findings, there were challenges related to misfiling and missing patient folders which sometimes lead to the creation of new patient folders. Also, the study discovered that the time spent in the retrieval of patient folders could negatively affect the timely delivery of healthcare services. The study recommended the adoption of electronic records management system as most public healthcare institutions in the country are rapidly shifting to electronic records management system. The use of electronic records management system is believed to be efficiently and effectively promoting easy accessibility, retrieval of patient medical records and allows easy communication amongst the healthcare service institutions and healthcare practitioners.

Recycled water usage in peri-urban agriculture : an examination of its socio-economic and environmental benefits in Port Elizabeth

- Description: This study examines whether recycled water (specifically grey water) is used in peri-urban agriculture in greater Port Elizabeth. It also investigates the socio-economic as well as environmental benefits of using recycled water in peri-urban agriculture. Community perceptions and narratives about the use of recycled water for farming purposes were also investigated. Study respondents were purposively selected from Motherwell and Uitenhage in Port Elizabeth. A qualitative research methodology and methods were used. This involved using purposive sampling to sample, select and interview 22 respondents and eight key informants. All the respondents were interviewed using in-depth semi-structured interviews. The study found that while many respondents have heard about water recycling, they are however not using recycled grey water for their peri-urban farming activities. Instead, peri-urban farmers continue to use water supplied by the municipality. Recycled water is mostly used for watering sport fields around Port Elizabeth. This continued usage of municipal water (instead of recycled water) is not only costly to the municipality but the individual farmers as well. The respondents were however aware of the benefits of using recycled water. These include economic (money saved from not paying irrigation water bills), environmental (nutrient contents found in recycled water) and socio-economic (conserving fresh available for use in other community purposes). The study recommends that the Nelson Mandela Metro Municipality should do more (through educational and environmental campaigns) to encourage peri-urban farmers to use recycled water in their farming activities.

- Description: The primary aim of this thesis is to interrogate existing psychoanalytical theory, as well as to conduct research on ‘the Burtonesque’ in order to examine two of Tim Burton's filmic adaptations of classic literary works for children, namely: Alice in Wonderland (2010), an adaptation inspired by Lewis Carroll's Alice books, and Charlie and the Chocolate Factory (2005), based on Roald Dahl's novel of the same title. Firstly, the chapters will explore the psychological elements found in the original texts, which Burton amplifies in his onscreen adaptations by means of various stylistic and thematic techniques that have become known as ‘the Burtonesque’. Secondly, the ‘Burtonesque’ itself will be explored, and thirdly, by means of comparative analysis, the investigation will focus on the manner in which such aesthetics aid Burton in highlighting psychological concerns in his films, as well as interrogate his cinematic alterations to existing texts in order to further his purpose. As a keen enthusiast of psychology and having won awards within the field, Tim Burton, a foremost contemporary Hollywood filmmaker, in his adaptations of children’s classics, deliberately magnifies the psychoanalytic components that are often found in the tales. Through his unique interpretation, Burton brings the psychoanalytic approach to children’s stories to a new level. He explores certain themes such as death, childhood fantasies, psychological development and parental relationships, and makes use of specific artistic techniques that create a sense of the uncanny and defamiliarisation in the viewer. Due to his gothic cinematic approach, Burton is one of the most recognisable artists in his field, with a consistent vision that is concerned with cinematic and thematic aspects of his work, branding his films with a unique ‘Burtonesque’ quality that has become instantly identifiable. As a result of Burton’s psychological interest in children’s stories, Tim Burton’s film adaptations of the Dahl and Carroll classic children’s stories can be interrogated under a psychoanalytic lens. Psychoanalytic literary criticism is a method of literary analysis that is informed by the tradition of psychoanalysis established by Freud. In Burton scholarship, critical exploration should take the form of not only a close reading of the original text and analysis of the film version in order to come to a psychoanalytic interpretation, but by means of comparison of film and text, highlight the methods applied by psychoanalytic theory. A close comparative analysis of original literary works by Roald Dahl and Lewis Carroll, and Tim Burton’s filmic adaptations will be conducted in this study. Thereby, the manner in which Tim Burton employs specific techniques in order to emphasise the stories’ psychological components will be explored. Biographical information of Dahl and Lewis, as well as a short synopsis of their stories will be included in order to provide the context for psychoanalytical criticism. A discussion of ‘the Burtonesque’ will be provided, as well as a brief exploration of his adaptation history and Burton’s specific interest in psychology. Research methods to be applied in the analysis of the original texts and their film versions are psychoanalytic literary theory, drawing predominantly on the work of theorists: Sigmund Freud, Jacques Lacan and Carl Jung. A close examination of auteur theory, the unique voice Burton has established through his iconic cinematic techniques, now coined ‘the Burtonesque’, will also be applied to provide a fuller and rounder interrogation. The level of knowledge, the sources thereof, and the girls’ attitudes and perceptions toward contraception, abortion and their actual use as possible barriers in the prevention of unplanned pregnancies. 3. The possible interventions in curbing the phenomenon of unplanned pregnancies. Data for the study was gathered through in-depth-interviews with ten female undergraduate students at the University of Fort Hare in East London campus, who had the experience of unplanned pregnancy while studying. Selection of participants was done through snowball sampling. A number of factors responsible for the unplanned pregnancy are revealed, including: Inadequate and inconsistent use of contraceptives, ignorance about contraception or rejection of contraceptive use due to religious beliefs. Influential factors regarding unplanned child bearing include: The role of the mother and the boyfriend, traditional and religious beliefs, and the desire for child bearing and motherhood. Although the majority of the girls are knowledgeable about contraception, and their right to abortion, a number of barriers hinder them from putting this knowledge into practical use. WISC-IV test performance of grade 3 Xhosa-speaking children : an extension of a prior South African normative database

A well-recognized problem exists when commonly employed tests developed in the United States (US) or United Kingdom (UK) are accepted unconditionally for use on local relatively disadvantaged populations, as these tests have questionable validity and therefore incur a high risk of misdiagnosis. Cross-cultural normative research has been conducted in South Africa on some of the Wechsler intelligence scales with respect to participants stratified for level and quality of education, age, race and language, including a study on the Wechsler Intelligence Scale for Children – Fourth Edition (WISC-IV) for learners with Grade 7 education (age 12 to 23). The aim of the current study was to conduct a partial duplication of the earlier WISC-IV South African norming study that specifically targeted a younger group of educationally disadvantaged children at a lower level of education. Method. Participants were Black Xhosa-speaking Grade 3 learners in the age range 8 to 9 (N =32 ), who were being schooled in the disadvantaged educational setting of the former Department of Education and Training (former-DET) schools. The WISC-IV results of the current study were statistically compared with the WISC-IV results from Shuttleworth-Edwards, Van der Merwe et al. (2013) study. Results. There was a significant lowering of between 20 to 30 IQ points relative to the UK standardisation on WISC-IV scores for this sample of Grade 3 Xhosa-speaking learners, replicating the earlier outcome for Grade 7 Xhosa-speaking learners relative to the UK standardisation. No differences were in evidence within the WISC-IV sample for the female and male participants on any of the indices. There was equivalence between the Grade 3 and Grade 7 Xhosa-speaking learners on all subtest and Index scores with the exception of the Digit Span subtest. Conclusions. The results confirm prior research indications of the negative impact of educational disadvantage on IQ test results and the need for this to be taken into account by the availability of socio- culturally relevant norms.

- Description: The aim of this study is to establish the employment-readiness perception of prospective University of Fort Hare Alice campus graduates. To do this, the study determined the capability of these prospective graduates to face the demands of the labor market. In addition, the study also wanted inter alia to find out whether there exist a relationship between prospective graduates perceived level of skills and their readiness to engage in the work environment. Astin’s Input-Environment-Outcome model of student development, which posits that students’ academic development in universities is affected by their own inputs as learners in addition to the institutional environment, has been utilized as the theoretical framework for this study. Hence a number of hypothesis informed by Astin’s model about whether there exist a relationship between perceived output, skills and readiness of prospective graduates for the workplace, on the one hand, and university education/training, on the other, were derived. To test these hypotheses the study utilized a quantitative research approach anchored around a survey design. In fact ample studies, which have investigated the issues of employability skills and prospective graduates’ readiness for work, have adopted this type of research design with a questionnaire as a preferred research instrument. After collecting data through the use of a mainly Likert scale based survey questionnaire, the findings of the study indicated inter alia that the university environment has a direct influence on the input of students to their study and skills acquisition in addition to the fact that faculty influence is directly correlated to students’ output, skills and readiness for employment. Hence the conclusion that for any tertiary institution like the University of Fort Hare to function effectively or optimally, the institutional environment, faculty influence and student input have to satisfy the best assessment criteria at the exit level, such that an output of quality prospective graduates imbued with all employment readiness characteristics can be guaranteed. Self-driven individuals with willingness to involve themselves in lifelong learning, who are capable of self-improvement and of taking advantages of innovative opportunities, are what the country’s economy need. These findings are in agreement with Astin’s model, which proposes that learning development is an interacting system of student input, institutional settings, and the students’ output. Results and conclusions of a significant number of other empirical studies concur with the results of this study. Furthermore the findings of the current study contribute to the graduate work readiness literature in a number of ways. For instance, the work process in modern organizations has been greatly transformed and as such that only high levels of skills, abilities and talented graduates are qualified to fit in such a labor market. The changing demands of the education competitive market have generated a belief that the educational sector will react in ways that are innovative with the aim of both the learning needs and the career objectives for its learners to be reached. - Description: The aim of this study is to establish the employment-readiness perception of prospective University of Fort Hare Alice campus graduates. To do this, the study determined the capability of these prospective graduates to face the demands of the labor market. In addition, the study also wanted inter alia to find out whether there exist a relationship between prospective graduates perceived level of skills and their readiness to engage in the work environment. Astin’s Input-Environment-Outcome model of student development, which posits that students’ academic development in universities is affected by their own inputs as learners in addition to the institutional environment, has been utilized as the theoretical framework for this study. Hence a number of hypothesis informed by Astin’s model about whether there exist a relationship between perceived output, skills and readiness of prospective graduates for the workplace, on the one hand, and university education/training, on the other, were derived. To test these hypotheses the study utilized a quantitative research approach anchored around a survey design. In fact ample studies, which have investigated the issues of employability skills and prospective graduates’ readiness for work, have adopted this type of research design with a questionnaire as a preferred research instrument. After collecting data through the use of a mainly Likert scale based survey questionnaire, the findings of the study indicated inter alia that the university environment has a direct influence on the input of students to their study and skills acquisition in addition to the fact that faculty influence is directly correlated to students’ output, skills and readiness for employment. Hence the conclusion that for any tertiary institution like the University of Fort Hare to function effectively or optimally, the institutional environment, faculty influence and student input have to satisfy the best assessment criteria at the exit level, such that an output of quality prospective graduates imbued with all employment readiness characteristics can be guaranteed. - Description: This dissertation is concerned with the phenomenological question of lack of care in the face of Martin Heidegger’s placing care as a base for being. More specifically with the question: How is Heidegger’s ontological notion of care to be understood from within the contexts of healthcare, in general, and nursing in particular. Furthermore, deep within this notion of care there is always the option to not care which, although care is always contained in the various modes of Heidegger's Dasein, can be a contemporary enigma demanding investigation. In approaching the interpretation of what it is to care, the question will be confronted on three fronts a to interrogate, in the context of healthcare, Heidegger’s conception of the phenomenological situation of care in his writings up to and including his Being and Time; b then to delve into the phenomenon of lack of care that seems to have appeared in the provision of healthcare in recent times; and, in an attempt to explain this lack, c to expand on Heidegger’s early conception of care more broadly out into the world by postulating a diachronic emphasis by introducing elements from the developmental psychology of Erik Erikson. It is argued that this is necessary in order to begin to understand provenance of the notion of lack of care within the sphere of healthcare. As nursing is considered an epitome of caring, the profession will be used as a vehicle to illustrate the phenomenon of lack of care and how this is possible when care is the basis of Being in the world. Thus the final section will bring out through the lens of lack of care the predicates of caring as they apply to the healthcare professions, and, just as importantly, other areas of human endeavour, for that matter. Thes predicates, it is postulated, are an accretion of five elements: development of the care-of, assumption of some level of authority, introduction of curiosity into the engagement with the world of people and things, an understanding of the role of empathy, and, finally, advocacy in the face of disturbance. The secondary objective is to look at the pastoral methods that he employed based on the challenges of the day. It is acknowledged that there have been autobiographies and biographies written on the experiences of well-known clergy like Dr. Frank Chikane, Bishop Desmond Tutu, Reverend Beyes Naude and others, but there is puzzling silence about other Christian permanent persuaders who played a pivotal role in the public life in South Africa. This individual is one of those like many who were able to trounce the inferior and oppressive education that was offered to black ministers in the Baptist Union of Southern Africa. This dissertation is a narrative of a heart-breaking but fascinating account of a black minister who was detained and tortured by security forces; sadly, he was even forsaken by his own brothers and sisters in Christ (Baptist church members). - Description: This research study explored the relationship between economic dependency and the reporting behaviours of victims of intimate partner violence. The study was conducted using qualitative methods. Data was gathered using semi-structured interviews and a focus group discussion. Data analysis was done using thematic analysis where themes were allowed to emerge from the raw data findings. The study intended to explore the experiences; reporting behaviours; coping strategies and available sources of support for victims of intimate partner violence. The study drew mainly from literature and the tenets of the Resource Theory as well as the Learned Helplessness Theory. The study established that economic dependency supported the existence of intimate partner violence owing to the fact that victims of intimate partner violence feared financial vulnerability should they report the perpetrator resulting in his arrest. It was also established that the relationship between economic dependency and the reporting behaviours of victims of intimate partner violence was compounded by the number of children the victim had including the number of family members dependent on the victims besides her own children. More dependents were linked to a lesser likelihood of reporting intimate partner violence. This was due to the fact that more dependents translated into a greater financial burden in the absence of the source of income; the perpetrator. On the other hand, the study also established that even those victims who were employed, though earning lesser money than their partners (the perpetrators), did not leave abusive relationships. This was linked to the possibility of learned helplessness as well as the possibility of their individual incomes being too little to afford them a life away from the perpetrator. The study also established that victims of intimate partner violence experienced physical violence more than all other forms of violence. Availability of support, frequency and severity of abuse were identified as determinants of the timing for reporting. The study established that families were the most active sources of social support for victims of intimate partner violence whilst all professional networks of support were discovered to be functional. The study made several following recommendations including awareness raising, introduction of policy amendments and the inclusion of victims of intimate partner violence in income generating projects. This problem mainly affect old, women and children as they are mostly accused of witchcraft practice in many societies. Witchcraft beliefs and fear is a major problem in Africa as compared to Europe and America. Although Malawi boasts of over 75% of Christians, but witchcraft beliefs and fear still poses a big challenge. There are so many factors that contribute to witchcraft beliefs and fear in Malawi. Some of the major factors include; cultural factors, legal factors, religious factors, social factors and economic factors. This study aimed at establishing a theological and ethical approach to witchcraft beliefs and fear within the Church of Central Africa Presbyterian (CCAP) Blantyre Synod in Malawi. The study established that old people particularly women and the children largely are the victims of witchcraft accusations in Malawi. This revealed that these victimised people needs security support, spiritual support, economic support, legal support as well as social support. The study also established that those that were victimised due to witchcraft accusations are stigmatized by the society and the church as well. This problem mainly affect old, women and children as they are mostly accused of witchcraft practice in many societies. Witchcraft beliefs and fear is a major problem in Africa as compared to Europe and America. Although Malawi boasts of over 75% of Christians, but witchcraft beliefs and fear still poses a big challenge. There are so many factors that contribute to witchcraft beliefs and fear in Malawi. Some of the major factors include; cultural factors, legal factors, religious factors, social factors and economic factors. This study aimed at establishing a theological and ethical approach to witchcraft beliefs and fear within the Church of Central Africa Presbyterian (CCAP) Blantyre Synod in Malawi. The study established that old people particularly women and the children largely are the victims of witchcraft accusations in Malawi. This revealed that these victimised people needs security support, spiritual support, economic support, legal support as well as social support. The study also established that those that were victimised due to witchcraft accusations are stigmatized by the society and the church as well. This investigation used a case study approach. The methodology used was a qualitative research approach which according to Leedy (1985: 101) and Merriam (2002) is used to answer questions about the complex nature of the phenomenon being studied. He further states that its purpose is to describe and understand the phenomenon from the point of view of the participants. The researcher used questionnaires, interviews, and observation processes to collect data in order to analyze it, and discuss the findings. The study revealed that the theses & dissertations collection are the only GL materials that were actively acquired and processed at the HPL and these were accessed by consulting the OPAC. However, other than T&D there is a bulk of unprocessed GL that remains inaccessible. Despite being invaluable research resource the GL is under threat of deterioration because of lack of preservation mechanisms. As a result of inaccessibility it cannot be shared or integrated within the SEALS consortium. The online sphere has become a platform for teenagers to connect with peers in different locales, thus increasing their exposure to large volumes of information including that which their parents, guardians and school authorities would debar them. Founded on the uses and gratifications theory and social cognitive theory, the study investigates the influence of social media on antisocial behaviour among teenagers. Uses and gratification theory was used in this study to understand how teenagers use social media whereas the social cognitive theory brought an in depth understanding as to how teenagers learn antisocial behaviour from inappropriate social media content. The social cognitive theory argues that children can acquire new behaviour through observation and learning from the environment as well as the media. The study was conducted using the mixed method design which focused on triangulation. Triangulation enables the researcher to combine theories, data sources and methods in a single study; this makes the information obtained in such a study to be verified, thus increasing validity of the study. A case study strategy of Jabavu High school learners as well as teachers was used in the study. Data was collected from Jabavu high school learners and their respective teachers to obtain information from various perspectives. Simple random sampling was used to select 150 grade 11 and 12 learners who participated in the study. Semi structured interviews were used to collect data from Jabavu high school teachers while questionnaires were used to collect data from learners at Jabavu high school located in Eastern Cape, South Africa. The study found that Jabavu High school learners attribute social media to antisocial behaviour. Thus, the study concluded that social media influences teenagers to participate in various forms of antisocial behaviour through observing, learning and imitating what they are exposed to on social media. - Description: The debate on how the communication function can contribute at a strategic level to organisational success is an on-going one because communicators still need to prove their relevance at the top management level. In general, the contribution of communication as a management function in corporate strategy is not known. It is either considered an afterthought or as an element within other functional departments. In this regard, the purpose of this study was to analyse critically the role of communication in strategic planning at the selected South African universities. To achieve this main objective, institutional theory was applied, as it seeks to affirm the role of communication in decision-making with the intention to be legitimised. In-depth interviews were conducted with senior communicators and strategy planners at six universities on how they approach strategic planning in their respective universities. Significant from the findings was the fact that the South African universities follow the same process for strategic planning. Regarding the role of strategic communication, the findings confirmed literature in the sense that communication practice at functional level has been institutionalised but not at the top management level. It was this study’s finding that university reputation and positioning did not prominently feature in the strategic planning processes. Furthermore, this study found that communicators should play a leading in environmental scanning to identify society, industry and commerce with a view to fine-tune the university’s qualification mix. That is, communicators should provide intelligence on changes in the environment which serve as the basis of the development of university strategies. In other words, they should provide vital information to top management to enable them to take informed strategic decisions. Lastly, it was this study’s finding that communicators should conduct evaluation research to assist top management in comparing the set strategic objectives with actual performance. In addition, the communicators should assist top management in determining corrective actions if need be. In view of these findings, this study developed guidelines that will empower communicators to make a more meaningful contribution in the strategic planning process. Given the paucity of research evidence of strategic communication in a university context, the study provides a better understanding of the theory and practice thereof.

The results indicate that the challenges of SAP child protection unit in providing services to vulnerable children in Alice, Eastern Cape Province are mainly financial based, emotionally stressful, labour related, and can be caused by parental oversight, legal constraints that begets lack of trust and also policing illegal child immigrants. These factors make the police child protection unit’s work challenging thereby hampering its effectiveness. It has therefore been recommended that the police child protection unit should be given the support due to them so as to make service delivery to vulnerable children in Alice effective. Also it has been recommended that children be educated that the police are there to enforce the law, assist and not to harm them.

This study is also significant for the factors which contribute towards school children being prevented from receiving the full range of benefits which libraries have to offer and the recommendations which it makes to improve the effectiveness of the role played by libraries in this respect. The study could also be of benefit to the staff of libraries, as it could serve to inform them with respect to what needs to be done in order to improve the ways in which libraries make information available to school children. In this respect the research could also provide valuable insights into the strategies which need to be developed and adopted in order to make public libraries more effective providers of various types and forms of information to school children. It is to be hoped that the study will also make a valuable contribution to the existing body of knowledge concerning public libraries as a means of providing information to school children. It is the fervent wish of this researcher to make a credible and articulate case for the great value which public libraries have for school children, despite the fact that general perceptions no longer accord libraries the value which they were once commonly held to possess.
